Carnes Weeks Center Residential Counselor Summary: A dynamic opportunity to help individuals with substance abuse and mental health disorders in a great team environment. Clinical Supervision is available for both MSW and CAC s. !rimary responsibilities include individual and group counseling sessions" with documentation and reports. Duties and Responsibilities: Assess clients by identifying and evaluating individual strengths" wea#nesses" problems and needs for the development of an individuali$ed recovery plan. %evelops recovery plans with clients by identifying problems needing resolution& establishing goals& determining a treatment process and resources to be used& and initiating discharge planning. 'eviews recovery plans according to established timelines. Conducts individual and group counseling. Duties and Responsibilities:

Assess clients by identifying and evaluating individual strengths" wea#nesses" problems and needs for the development of an individuali$ed service plan. %evelop service plans with clients by identifying and ran#ing problems needing resolution& establishing reasonable and achievable goals& determining a recovery process and resources to be used& and initiating discharge planning. Assist clients in gaining access to addiction treatment services" medical" social" educational" vocational" housing and other services essential to meeting basic human needs and achieving optimal *uality of life and community living. Advocate for clients in various systems to assure *uality treatment and e*uity of care !rovide crisis prevention and intervention by identifying and helping clients to identify triggers for and2or symptoms of relapse" and early intervention to prevent further setbac#s. Create and facilitate prevention presentations for middle school" high school" college and community groups. 3ead substance abuse psycho educational groups in school settings. 3ead evidence based parent and family s#ills groups" wor#ing with both youth and parents to improve family s#ills and strengthen relationships. Create media materials aimed at promoting healthy youth behavior and positive community change. Train and mentor youth and peer leaders. !rogram specific data collection and reporting. Successful e,perience wor#ing with community coalitions and environmental prevention strategies is preferred. 4,cellent communication and documentation s#ills" attention to detail" and time management s#ills are re*uired. Strong computer and organi$ational s#ills with a customer service orientation and the ability to function in a team environment.
