Somerset, PA 15501, was contacted and provided a compact disc (CD)
representing a copy of DVD No. 60 dated 09/11/2001 entitled "The Crash"
United Flight #93, containing telephone calls received by the emergency
center. In addition, Baker furnished a copy of a diskette containing
telephone call summaries dated 09/11/2001.
On 05/29/2002, the CD received from Baker was reviewed and it
was ascertained that a malfunction had occurred resulting in not all of
the telephone calls received by the emergency center on
09/11/2001 were contained on the CD. Baker was telephonically contacted,
appraised of the problem and agreed to re-copy the DVD.
On 06/04/2002, the second copy of DVD No. 60 was obtained from
Baker.

On 06/13/2062, Carol Peretin, Training Supervisor, Cambria


County 911 Center, 401\Candlelight Drive, Ebensburg, PA,! provided
pertinent information relative to the 09/11/2001 crash of United Airlines
Flight 93 (UA93). Peretin provided the following: !

Threat at Cambria County Airport,., 479 Airport Road, Johnstowh, PA, received
at 10:02:27 on 09/11/2001. \n also provided a cassette tape containing

telephone calls specifically related to UA93 received by Cambria County


911: The first call was from Joe McKeiyey, Director, Cambria pounty Airport,
advising that the Johnstown tower had been evacuated and the airport closed
due to a report of an unresponsive 75.7 heading in the direction of the
airport. The second call placed from \ cell phone was ff;om an unknown
person who reported the crash of an airplane near Shanksville, PA. The
Cambria County 911 Center transferred the call to the Somerset County
911 Center. The Cambria County 911 Cent'er did not obtairj the identity
of the caller. Peretin added that since the call was placed from a cell
phone, only a cell tower site would have been available, iwith no other
identifying information. \

PA. | ~l'heard a roar and/looked up an.d observed a large airplane
fly overhead at 150 to 200 feet. I "^described the] roar as coming
from the jet engines and being abnormally loud. I I ~~\d see black
smoke coming from the engines, which he described as being like when the
engines are throttled up. The airplane was traveling very fast and heading
in an easterly direction. .The airplane was so low that he could see the
rivets and the colors and s/tripes on the plane. I "Icontinued watching
the airplane, which was .rolling back and forth. The airplane rolled to
the right until the right wing pointed toward the ground. It then went
back to a level position, then rolled to the left until the left wing
pointed toward the grp'und. The plane continued rolling in this manner
as it traveled in an/easterly direction.

| |-told the 911 operator that he could'still see the/ajlrplane and
that it was continuing to travel toward,the' east. i / 1 advised that
from his location in Stahlstown, he was..a'ble to see for about 15 to 20
miles and could see the airplane for.-'several minuses. As he continued
to watch, the airplane disappeared"'over the horizon. At the time the
airplane disappeared, it was rolling hard to the right with the right
wing going well past perpendic-ular to the ground. I I was able to
hear the airplane crash, butx'wa.s' unable to see a fireball or cloud of
smoke.
At the time .of the crash, | r'was not aware of the events
that had occurred at ..the World Trade Center or Pentagon. It was not until
after the airplane,.crashed that he was informed that airplanes had crashed
into the World Trade ..Center and the Pentagon.
I .-••''' ~|was able to observe the airplane for several minutes
prior to the crash. During that time, the airplane appeared to be intact.

regarding his observations concerning the crash of United Airlines
Flight 93 (UA93) on September 11, 2001. Thereafter, | ]provided
the following information:
advised on the morning of September 11, 2001, he and
his father"7"\e watching the news concerning the events
occurring at the World Trade Center and the Pentagon. | | hear
big booming sound and looked outside to see a large cloud of smoke and
the roof of a nearby cabin on fire. While his father called 911 to
report that a plane had crashed nearby, | ~~| took a four-wheel
all terrain vehicle (ATV) and drove over to site of the crash and fire.
I I assisted others in putting the cabin fire out. Afterwards, he
drove over to the crater caused by the plane crash. I Iwas not
able to see very much, only pieces. Upon the arrival of emergency
units, including the Pennsylvania State Police (PSP), | | was
asked to leave.

Center, 401 Candlelight Drive, Suite 100, Ebensburg, Pennsylvania (PA),
15931, telephone number 814-472-2055, provided pertinent information
relative to the September 11, 2001 crash of United Airlines Flight 93
(UA93). PERETIN was advised of the identity of the contacting Agent
and thereafter provided copies of two incident reports and a cassette
tape containing recordings of two separate calls received by the
Cambria County 911 Center regarding the crash of UA93 at Shanksville,
PA, on September 11, 2001.
PERETIN provided an incident report detailing pertinent Pre-
Arrival Notes for Mass Transportation Incident at Lambert Road,
Shanksville, received at 10:18:43 on September 11, 2001, a copy of
which is attached and made a part hereto.
PERETIN provided a second incident report detailing pertinent
Pre-Arrival Notes for Bomb Threat at Cambria County Airport, 479
Airport Road, Johnstown, PA, received at 10:02:27 on September 11,
2001, a copy of which is attached and made a part hereto.
PERETIN advised that Cambria County 911 received two
telephone calls specifically related to UA93: The first call was from

report of an unresponsive 757 heading in the direction of the airport.
The second call placed from a cell phone was from an unknown person who
reported the crash of an airplane near Shanksville, PA. The Cambria
County 911 Center transferred the call to the Somerset County 911
Center. The Cambria County 911 Center did not obtain the identity of
the caller. PERETIN added that since the call was placed from a cell
phone, only a cell tower site would have been available, with no other
identifying information.
PERETIN provided a cassette tape containing a recording of
the two calls received by Cambria County 911 Center, as outlined above.
