For good and valuable consideration, receipt of which is hereby acknowledged, I irrevocably authorize Produc-
er to make use of my appearance in connection with the motion picture production, (Production) to be pro-
duced by Producer. Performer gives dreamthree, inc., the absolute right and permission to use video(s),
photograph(s) and or audio recording(s) in its promotional materials and publicity efforts and understands
that the photographs may be used in a publication, print ad, direct-mail piece, electronic media (e.g., video,
cdrom, Internet/WWW), or other form of promotion. Performer releases dreamthree, inc., and its suc-
cessors, licensees and assigns, as well as its photographers, videographers, production team, their ofces,
employees, agents, and designees from any and all claims, actions and damages relating to or arising out of
the use in any context and in any form by the Producer, its successors, licensees and assigns and releases
same from liability for any violation of any personal or proprietary right Performer may have in connection
with such use. Performer is 18 years of age or older. Performer acknowledges that Producer is under no
obligation to use produce, release, broadcast or to otherwise exploit the Program, or to use Performers
services or the rights granted hereunder in connection with the Program.

Performer agrees that Producer shall own, in perpetuity, throughout the universe, all right, title and interest
in and to the Program, the elements thereof, and the results and proceeds of the Performers services hereun-
der and all the materials produced thereby or furnished by the Performer, of any kind whatsoever and free and clear
of any and all claims for royalties and other compensation except as specifically set forth in this Agreement. Perform-
er acknowledges that any and all results and proceeds of Performers services hereunder shall be a work made for hire
for Producer, specifically commissioned for use as part of a motion picture or other audiovisual work. Producer shall
have the right to adapt, change, revise, delete from, add to or rearrange the Program or any part thereof, and Per-
former waives throughout the universe the benefit of any law, doctrine or principal known as droit moral or moral
rights of authors or any similar law, doctrine or principle however denominated, to the maximum extent permitted in
each applicable jurisdiction. Producer shall own the Program produced hereunder and all rights whatsoever therein,
including, but not limited to, all copyrights throughout the universe and in perpetuity and in all elements thereof and
shall have the right to sell, lease, license and otherise exploit such right and elements, as Producer may determine in
its sole discretion. Performers grant includes all rights regarding the renting, lending, fixing, reproducing and other
exploitation of the Program conferred under any applicable law, directions or regulations, including without limitation
whatsoever.
Performers sole remedy in the event of any default by Producer hereunder shall be an action against Producer to re-
cover monetary damages, if any. Performer agrees that Performer shall have no right to enjoin the exhibition of the
Program or any other production based thereupon or to terminate or rescind any rights granted to Producer hereun-
der, or to obtain any other form of equitable or injunctive relief, any right to which Performer hereby waives. In the
event Performer shall attempt in any way to terminate or cancel Producers rights hereunder, Producer shall be enti-
tled to seek injunctive relief in addition to all other remedies available either at law or in equity.

In order to induce Producer to enter the foregoing agreement, I represent and warrant that I am the parent or
guardian of minor _________________________________(Minor). I have read the foregoing Personal Release
and am familiar with all of the terms and conditions thereof and I consent to its execution by the Minor. I agree
that neither I nor the Minor will revoke or disaffirm the foregoing Personal Release at any time. I agree to indemni-
fy and hold Producer and its successors, licensees and assigns harmless from and against any and all claims, liabili-
ties, costs or expenses, including attorneys fees and costs which may arise from the breach or alleged breach by the
Minor or me of the foregoing Personal Release or this consent.
