Foreword
This report covers mortality that occurred in 2007. At that time, Zimbabwe had Combating Human Immuno Deficiency Virus (HIV) and Acquired Immune- Deficiency Syndrome (AIDS), Malaria and other diseases as one of its top three priority Millennium Development Goals (MDGs). The other two priority MDGs were eradication of extreme hunger and poverty and promotion of gender equality and empowering women, both related to the health of a population. The goal of combating HIV and AIDS, Malaria and other diseases by the year 2015 has the following guiding targets: Have halted, by 2015, and begun to reverse the spread of HIV and AIDS Have halted, by 2015, and begun to reverse the increasing incidence of Malaria, Tuberculosis and diarrhoeal diseases

As at 2013, the country had reprioritised the MDGs to: MDG 1:- Eradicate Extreme Hunger and Poverty MDG 2:- Achieve Universal Primary Education MDG 4:- Reduce Child Mortality MDG 5:- Improve Maternal Health

Timely and reliable information on mortality is vital for planning, monitoring and evaluation of these and other health outcomes. The sources of mortality data include population censuses and surveys, routine health information (public health institutions inpatient mortality data) and registered deaths from the Registrar Generals Office (RGO) in the Ministry of Home Affairs. The main source of data for this report is the Registrar Generals Office. The Ministry of Health and Child Welfare (MoHCW) and the Zimbabwe Republic Police (ZRP) play a part in the registration process by providing the Registrar Generals Office with deceased inpatients medical documents. The registration coverage in rural areas is not as good as in urban areas mainly due to the transport costs involved in travelling to register the death, shortage of human resources at registration centres and the lack of understanding of the importance of registering deaths. The Zimbabwe National Statistics Agency (ZIMSTAT) acknowledges with thanks the financial support given by the United Nations Population Fund to code data at the Central Registry. The United Nations Childrens Fund provided financial support to code the data at Provincial capitals in cases where the data was not easily available at the Central Registry. This report includes data on suicide deaths which were published separately up to 1995 in the Suicide Reports.

Introduction
The registration of deaths in Zimbabwe is compulsory in terms of the Births and Deaths Registration Act, Number 11 of 1986 which requires notification of a death within thirty days after occurring. This requirement can be more easily complied with in urban centres than in rural areas. To register a death, a Notice of Death Form has to be completed and submitted to the local Registrar of the Registrar Generals Office (RGO). Each of the fifty-four districts in Zimbabwe has a District Registration Office. However, in rural areas the registration office may be located in distant places where one would incur high transport costs in order to register an event. This report presents data on causes of death extracted from the death notification form and classified according to the International Classification of Diseases Tenth Revision (ICD10). According to ICD10, the cause of death refers to the underlying cause of death which is defined as the disease or injury which initiated the chain of events leading directly to death or the circumstances of the accident or violence which produced the fatal injury. The data is further disaggregated by age and sex in an attempt to assess age groups which are vulnerable to certain diseases and those diseases which are gender specific. Although suicides, deaths due to injuries, maternal deaths and AIDS deaths are included in the total deaths, special attention has been given to them in this report. Unlike the International Classification of Diseases Ninth Revision (ICD9), the ICD10 provides for the classification of deaths resulting from HIV.

Methodology
Data for this report consist of deaths which occurred and were registered in 2007. However, late registrations (i.e. deaths registered after 2007), are accounted for by using the late registration data for the previous year (i.e. number of deaths for 2006 but registered in 2007) as a proxy for 2007 late registration. A major limitation of this data is that registration of deaths in rural areas is not complete in contrast to registration in urban areas which is almost 100 percent. Registration of Deaths For a death occurring in a health institution, a doctor completes a Medical Certificate of Cause of Death which is then used by health institution staff to prepare the Notice of Death. Copies of the Notice of Death of a person and Medical Certificate of Cause of Death are shown in Appendix Documents I and II, respectively. The two documents are sent to the District Registrar Generals Office for preparation of a Death Certificate. Notices of Deaths for persons under police investigation including suicide cases, motor vehicle accidents and murder among others are prepared by the police who in turn submit notification forms to the District Registrar. The District Registrar sends the forms to the Provincial Registrar who then sends them to the Central Registrar in Harare. In urban areas, police have to be notified of a death that occurs outside a health institution and it becomes their responsibility to make arrangements for post mortems, where necessary, and to prepare the notification of death which is sent to the Registrar nearest to where the death

occurred. In rural areas, relatives of the deceased have the responsibility of registering the death that occurs outside a health institution. Coding The ZIMSTAT staff housed in the RGO, for easy access to the registration documents, code deaths data using the coding sheet shown as Appendix Document III. Three causes of deaths for general mortality study can be identified as immediate, contributing and underlying. This report is based on the underlying cause of death. Other variables include sex, age, place of death, month of death and occupation. The codes for these other variables are the same as those used during the 2002 Population Census. Data Processing and Analysis After completing the coding process, the coding sheets are sent for data processing. Data entry is done using the Census and Surveys Processing System (CSPro) while the Statistical Analysis Software (SAS) is used for tabulation.
