Disaster Recovery plan (DRP) Plan Purpose /Overview: In the event of a major occurrence, event, or major disaster (whether connection loss or natural disasters), this document will outline and dictate the necessary action to various contingency plans that will guide what protocols are critical and high-priority recovery requirements. This plan is used for both Information Systems (including WAN, LAN, Workstations, Network Devices, etc.) and other facilities. This plan is categorized into separate sections, each divided into sub-sections: * Disaster Recovery Management and Leads * Network and Information Systems Infrastructure Plan * Business Operations Plan * Office Facility and Structure Plan * Technology Priorities * Assigned Personnel * System Profiles (Each Remote Office) * Communications Section I: |Disaster Recovery Management and Leads| The Disaster Recovery Management Leads will provide direction and initiate the contingency plans are part of primary directives, given the nature of the situation. Below are the key leads that will facilitate and carry-out the recovery plan: * Disaster Recovery Coordinator/Manager * Network and IT Recovery Lead * Business Operations Lead * Facility and Construction Lead Section 2: |Network and Information Systems Infrastructure Plan| This section will detail on what measures need to be document, the proper protocols and procedures needed during the event of a disaster, and reporting methods. The network is crucial to the overall operation of the business, without it the partnership will not be able to function and meet its own internal objectives and goals.

Info-Network Solutions, Inc. has devised a well structured and detailed contingency plan in the event of a disaster. S-2.1: Network Lead The essential responsibility of the Network and IT Recovery Lead is to oversee the recovery of both the Internal LANs (Local Area Networks) and the WAN Infrastructures, bringing them both to the normal operation state. It is also his/her responsibility to counteract and report any unauthorized entry into the partnership infrastructure. Network and IT Recovery Lead (Responsibilities and Actions) | Objectives/Goals: | * Review Information Systems recovery plan and initiate * Notify and relay details of Recovery Plan to Information Systems Team * Assemble all necessary resources to ensure network restoration * Assign personnel of Information Systems Team necessary tasks to be carried out * Keep track of IS team status updates and documentation * Ensure all data backups are active for network configuration restoration * Inventory all network equipment and functional status * Assess the immediate situation and determine recovery priorities * Determine, calculate, and assess the overall damage of network * Communicate with Vendors, Network Technicians/Administrators, and Partnership of Disaster Recovery status updates * Determine and provide additional resources (if needed) to all Information Systems Support Members. * Provide status of Network Telecommunications Room * Provide an estimation of network restoration * Revise Recovery Plan and promote changes in plan to team members and Business Recovery Coordinator | S-2.2: Possible Network Disaster Scenarios S-2.2.1: WAN Connectivity Loss / Network Outage In the event of a network outage on the primary connection, a failover has been implemented to take over without notification to both the Users and Network Administrator. Although in a state that both WAN links have been lost, all network traffic will be sent over a private VPN (Virtual Private Network) setup by the Network Administrator, so confidential and crucial data maybe exchanged and/or backed up. A Virtual Private Network (VPN) is a virtual connection from one point to another by using the public internet. This is done by using a tunneling protocol to establish the connection. S-2.2.2: DHCP (Dynamic Host Configuration Protocol) State is down The role of the DHCP Server is to assign IP Addresses to all hosts/devices on the network from the designated address pools. The overall IP address assignment is centralized and located in the

Central Office. The remote offices utilize DHCP Relay Agents to assign IP Addresses from Central Office. If the primary WAN Link goes down, the failover will take over until primary is restored. In the event that both links are down, then each remote office will have DHCP configured; which will allow users to log into the local network and still accomplish necessary tasks. The use of the VPN through the remote offices dedicated Internet Connection will be utilized to exchange information between file servers. This will provide up-to-date patient information and will be able to be accessed while the WAN links are down. S-2.2.3: Domain Controller Status State is down The Domain Controller in each remote office is a critical piece of equipment that is vital to the overall operation of the partnership. Without the Domain Controller, users will not be able to log into the network. That is why a standby server will be readily available to use and replace the current domain controller if damaged or until it is up and running. The standby server will have all of the latest updates and a replica of Active Directory with all users and computers in its catalog. This will allow easy transition from the primary D.C. (Domain Controller) to the secondary/backup D.C. without much interference. Section 3: |Business Operations Plan| S-3.1: Business Operations Lead The essential responsibility of the Business Operations Lead it to coordinate and dictate the necessary action that is needed to bring the entire state of the business back online and to its normal functioning status. The overall objectives, goals, and responsibilities are listed below. Business Operations Recovery Lead / Recovery Coordinator(Responsibilities and Actions) | Objectives/Goals: | * Communicate with Partnership the overall status of damage assessment * Discuss the Business Recovery Plan with Operations Team. * Inventory all Patient Information and ensure the storage at a secure location, outside of the partnership. * Communicate with Information Systems Team on total damage expenses and additional resources needed to fulfill the Information Systems Recovery Plan. * Communicate with Facility and Construction Team on total damage expenses and any additional resources/funds needed to fulfill the Facility and Construction Recovery Plan. * Adjust changes and revise Business Operations Recovery Plan. Section 4: |Facility and Construction Plan| S-4.1: Facility and Construction Lead The essential responsibility of the Facility and Construction Lead is to perform the necessary building damage assessment from natural disasters. It is also their responsibility to ensure that the business is back up and running in a timely manner without direct loss to the overall partnership. Facility and Construction Recovery Lead

(Responsibilities and Actions) | Objectives/Goals: | * Obtain and gather the overall building damage assessment * Develop the total cost matrix of expenses and communicate with the Partnership * Purchase vendor services to reconstruct the damaged parts of the facility * Communicate with Business Operations Recovery Lead on any additional services required * Communicate with Network and IT Recovery Lead on state of network infrastructure and necessary materials (wiring, wall plates, etc) needed. |
