NAME OF DRUG Generic: Flupentixol MECHANISM OF ACTION Acts as an antagonist at various dopamine (D1-D5), serotonin (5-HT2), adrenaline(1), and histamine (H1) receptors, without affecting the muscarinic acetylcholine receptors. Its antipsychotic effects are likely caused by D2 and/or 5-HT2A antagonism, whereas its antidepressant effects at lower doses may be mediated by preferential D2/D3 auto receptor blockade, resulting in increased postsynaptic activation. INDICATION The maintenance therapy of chronic schizophrenic patients whose main manifestation s do not include excitement, agitation or hyperactivity. CONTRA INDICATION Flupenthixol should not be given to patients who are hypersensitive to the thioxanthenes. It is also contraindicated in patients who have suspected or diagnosed subcortical brain damage, blood dyscrasias, pheochromocyto ma, liver damage, cerebrovascular or renal insufficiency, severe cardiovascular disorders. Patients with confusion/agitati on should not use Flupenthixol. ADVERSE REACTION CNS: NEUROLEPTIC MALIGNANT SYNDROME, sedation, extrapyramidal reactions, TD. EENT: Blurred vision, dry eyes, lens opacities. GI: Constipation, dry mouth, anorexia, hepatitis, ileus, priapism. GU: Urinary retention. Derm: photosensitivity, pigment changes, rashes. Endo: Galactorrhea, amenorrhea. Hemat: AGRANULOCYT OSIS, leukopenia. Metab: Hyperthermia. Misc: Allergic reactions. NURSING RESPONSIBILITY Usual: Remind patient IM (Adults): that before 20mg/amp q having any monthly surgery, give 1 dose including prior to dental or discharge emergency treatment, tell Actual: the surgeon, 20mg/amp q doctor or monthly dentist that you are taking flupentixol. Inform client that Flupentixol can occasionally cause a dry mouth. DOSAGE RATIONALE To prepare the patient for the procedure.

If patient experiences this, try chewing sugar-free gum, sucking sugarfree sweets or pieces of ice. Avoid strong sunlight and sunbeds until you know how your skin reacts and use a suncream higher than factor 15.

Flupentixol can cause some people's skin to become more sensitive to sunlight than it usually is.

If client experience 'flu like' symptoms such as stiffness, high temperature, abnormal paleness, leaking bladder and a racing heartbeat. To inform the client for the possible side effects of the medication.

Educate the patient that the symptoms of overdose may include seizers, muscle spasms, weakness, fast heartbeat, fever, difficult breathing, severe dizziness, drowsiness, convulsions, irregular heartbeat, disturbed concentration, constipation and coma.
