Important Notes for Applicant:


1. Please submit the following supporting documents with your AQ application:
i) Photocopy of higher or postgraduate qualification. The original must be produced for verification either at the application stage or at the registration
stage;
ii) Processing fee is $65 per AQ application. Fee paid for each AQ application is non-refundable. Payment at counter is acceptable only in cheques. Nets
or Credit cards services are available via internet only.
iii) A letter of verification (LV) sent directly to the SMC by the awarding institution. LV submitted by you will not be accepted by the SMC. AQ application
will be considered cancelled if SMC does not receive your LV from the awarding institution after 2 months from date of submission.
2. Fill in the accompanying checklist (Annex A) by ticking in the appropriate box therein for each additional title /qualification. An individual checklist must be
duly completed & submitted for each AQ applied. E.g. If you are applying to register 2 AQs, the application form must be accompanied by 2
checklists duly completed.
3. If the AQ has not been approved for display previously, you must also submit:
• Course structure, syllabus duration; information about the awarding institution & its recognition by local authorities;
• All relevant information on format of assessments and examination & recognition as a specialty qualification in country where AQ was awarded.
• You are responsible for providing documentary evidence of all the criteria in the checklist. Absence of relevant evidence will result in rejection of your
application.
4. If any of your declarations is ‘NO’, it is veto factor and you do not need to submit this application form. Your title or AQ is not approved for display.
5. If you have filled in ‘YES’ to all sections, submit this form to SMC where a further evaluation will be made, and a Specialist Training Committee (STC) or the
College of Family Physicians, Singapore (CFPS) as appropriate will be asked to give its opinion. Your title or AQ will be approved for display if all criteria are
deemed met and the relevant STC/CFPS also assents.
6. All decisions made by the SMC are final.

Name of medical practitioner must be the same as in the Register of Medical Practitioners. Evidence such as NRIC or Deed poll must be furnished for any change in the name of the
medical practitioner.

ANNEX A - CHECKLIST FOR APPLICATION FOR APPROVAL FOR DISPLAY OF ADDITIONAL TITLE/
QUALIFICATION (AQ)

1. If a clinical qualification - is it directly related to and consistent with
your registered area of medical practice?
OR
Is the academic degree relevant to your clinical practice?
The subject should directly advance clinical practice and not be peripheral or
theoretical.
2. Is this title OTHER than an honorary title?

The title should not be merely conferred, but must be based on training courses.
Honorary titles are not approved for display, except those that have historically
been allowed.
3. Is the issuing body of the clinical qualification accredited by the
Ministry of Health or Medical council or their equivalents in that
country as the appropriate authority for the relevant area of practice?

Is the University that is conferring the degree of good international
standing?

4. Does the clinical title indicate an advance in GP, specialty or sub-


specialty interest, which is NOT procedure specific?
OR
Does the academic degree indicate an advance in knowledge beyond
your highest clinical qualification?
The degree should not represent a lower order of knowledge than your highest
clinical qualification and generally should not be obtained prior to achieving your
highest clinical qualification.

There must be stated minimum qualifications and minimum duration of practice
before entry into a program.
