Apego January 23, 2014 N-40 Patients name: Antonieto, Buron Name of Drug Generic Name: metformin hy drochloride Trade Name: Glucophage, Glucophage X R Availability: Tablets500, 850, 1,000 mg; ER tablets 500 mg Classification of Drug Therapeutic: Antidiabetic Pregnancy Category B Mechanism of Action Exact mechanism is not understood; possibly increases peripheral utilization of glucose, decreases hepatic glucose production and alters intestinal absorption of glucose. Indication Adjunct to diet to lower blood glucose with type 2 (noninsulindependent) diabetes mellitus in patients Contraindications Contraindicated with allergy to metformin; CHF; diabetes complicated by fever, severe infections, severe trauma, major surgery, ketosis, acidosis, coma (use insulin); type 1 (insulindependent), serious hepatic impairment, serious renal impairment, uremia, thyroid or endocrine impairment, glyco suria, hyperglycemia associated with primary renal disease; Side Effects Nursing Responsibilities

Endocrine: Hypogly o History: Allergy to metformin; diabetes cemia, lactic complicated by fever, acidosis severe infections, severe GI: Anorexia, trauma, major surgery, nausea, vomiting, e ketosis, acidosis, coma; pigastric discomfort type 1 diabetes, serious , heartburn, hepatic or renal diarrhea, flatulence impairment, uremia, Hypersensitivity: Al thyroid or endocrine lergic skin impairment, glycosuria, reactions, eczema, hyperglycemia pruritus, erythema, associated with primary urticaria renal disease, CHF, pregnancy, lactation. Do not discontinue this medication without consulting your health care provider. o Monitor urine or blood for glucose and ketones as prescribed. o Swallow ER tablets whole; do not cut, crush, or chew.

o Do not use this drug during pregnancy; if you become pregnant, consult with your health care provider for appropriate therapy. o Avoid using alcohol while taking this drug. o Report fever, sore throat, unusual bleeding or bruising, rash, dark urine, light-colored stools, hypo- or hyperglycemic reactions. Chief complaint: Right cerebral tumor left sided weakness
