You must bring two forms of identification in order to be admitted to the Consular Section passport, driving license, national identity card, carte de sjour, etc.. A passport. If more than one person is shown in the passport, a separate visa application is needed for each person traveling to the U.S. If you have a French passport and it will expire in less than 6 months: Your passport may be used, but it is recommended that it have at least 6 months of validity remaining. A temporary "Laissez-passer" is not a valid document to receive a visa.
Complete the Nonimmigrant Visa Electronic Application, Form DS-160. After completing the DS-160, you must print the confirmation page. This page should contain a bar code which you will have to bring with you to the interview. Please visit https://ceac.state.gov/genniv/ to electronically complete DS-160.
Please fill out the DS-160 form at least 48 hours before your appointment. This will allow a faster process of your application.

A passport photograph taken within the last 6 months (5 cm X 5 cm) for each applicant, regardless of age, clearly showing the full face against a white background, ears are exposed, no dark glasses nor head covering (unless the applicant's religion prohibits the absence of a head covering).
