The Bharat%as !istributor will issue a booklet called the !omestic Consumer Gas Card> popularly known as Blue Book or the $ass Book where the particulars of your #$G connection> the details of supply and other ser&ices are entered by the distributor;s staff. A. 0t is not compulsory to buy hot plate from the !istributors. A customer can buy B0S appro&ed Sto&e from any source of his choice. /owe&er !istributors are ad&ised to keep hi%h

Authorisation Code on production of Subscription 6oucher 2S64. The e CTA is &alid for C months from the date of issue. B The S6 with the Authorisation Code should be produced to the new distributor> who will enrol the customer D return the ori%inal S6 after endorsement. The S6 is an important document and should be retained safely. The T6 is &alid for one year from the date of issue. B !o not surrender your !omestic Gas Consumer Card. The same card can be used at the new location after endorsement by the new distributor. (ather> 5other> Brother Sister> Son> !au%hter4 is permitted. B 3ri%inal S6 holder has to submit his written consent> in Annexure A> to transfer his connection to a specific family member. B Additionally> the family member recei&in% the connection has to submit an affida&it> in Annexure B> indemnifyin% /$C# re%ardin% this connection. B (urther> completed 98C form with &alid $roof of 0dentity 2$3l4 and $roof of Address 2$3A4 must be submitted.
