You need to print out the completed visa application from the website, sign it, and submit it with other required documents for visa application as well as visa fee to the GPP office, located in BRI 310 by 4.30 P.M. on January 13, 2012.

NOTE 1. In the visa application, you must choose money order as your payment method, but you need to pay your visa fee in the amount that appears in your Indian Visa Summary page (the first page of printed-out visa application) by personal check or credit card the travel agency that the school uses to facilitate your visa application. The school pays the service fee for you. 2. $5 credit card processing fee need to be added to the visa fee amount if you pay by credit card. Personal check need to make payable to Crown International Travel 3. If you are still less than 18 years old on January 13, 2012, both of your parents or legal guardians must also co-sign your visa application.
