Before Hospitalization According to the client, The client is not choosy when it comes to food. Whatever food is available, she eats it. The client has no ability to eat all by herself due to her condition her mother is the one who help her in eating.

During Hospitalization

Interpretation and Analysis

The client is on Diet as Interpretation: Tolerated. The client is not choosy when it comes to eating She usually eats fruits foods. and foods that the hospital provides. Analysis: An individuals food preferences and habits are often a major factor affecting actual food intake. Habits about eating are influenced by developmental considerations , gender , ethnicity and culture, beliefs about food, personal preferences , religious practices, lifestyle economics, medications and therapy, health , alcohol , consumption, advertising and physiological factors.

Elimination

The client urinates more than 5-6 times a day with no difficulty.

During hospitalization she only urinates twice by using her diaper. And three days she didnt defecate, but on

She defecates once a day every afternoon with no difficulty. When it comes to bowel movement, the client defecates 2-3 times a week.

Analysis: Successful elimination in human beings depends on individual having an intact and fully functional urinary tract, gastrointestinal tract and nervous system. Exercise

The patient stated that her form of exercise is by simply cleaning their house before. She also does waking outside the house. The patient was asked to rate each area of self-care on a scale of 0 to 4; 0 being completely independent and 4 being dependent. She can bathe, dress, ambulate, prepare food and feed herself; at times her complaints of pain at her hips so whenever pain occurs

The client doesnt engage to any form of exercise since she has the inability to stand and walk.

Hygiene

According to the client Her Family members She usually takes a bath perform bed bath for the twice every day. client as her form of hygienic measures. The client brushes her teeth twice a day.

Interpretation: The client maintains her hygiene by the help of her family members in providing the client bed bath.

The client trims her nails every 5 days. The client use deodorant and cologne.

Analysis: Hygiene is the science of health and its maintenance. Personal hygiene is the self-care by which people attend to such functions as bathing, toileting, general body weight and grooming. Hygiene is highly a personal matter determined by individual values and practices. (Fundamentals of nursing 7th edition p.1164, by Kozier)

Sleep and Rest

Her usual sleeping She has an intermittent habits before admission sleep pattern because were 8 PM to 5 AM. she usually wakes up around 2am or 3am She woke up in the almost 7- 8 hours a day middle of her sleep and and she feels rested had difficulties getting upon waking up. She back to it. seldom takes a nap during the afternoon. She has a difficulty going to sleep since she was hospitalized and due to her conditions. The clients sleeping pattern is being disturbed due to doctors / nurses visits and also because of the hospital environment.

Interpretation: The sleep pattern of the client is normal since an adults hours of sleep should be 7-8 hours. And the clients hours of sleep is 7-8 hours. Analysis: Sleep is the basic human need. It is biological process common to all people. We require sleep to cope with daily stresses, to prevent fatigue, to conserve energy, to restore the mind and body and to enjoy life more fully. Sleep enhances daytime functioning. It is vital for not optimal

psychological functioning but also psychological functioning as rate of healing of damaged tissue is greatest during sleep. It is an important factor in persons quality of life. (Fundamentals of nursing 7th edition p.1164, by Kozier) Sexual Activity The client stated that she has a good relationship with her husband. The patient verbalizes that she and her husband were no longer active in sex since after her operation. The patient always shows that she is someone who gives everyone hope and she always ensure the condition of every family member. She also added that in a family it is not only the father who has to be strong, it should be both partners, because every person has their limits and we shouldnt depend on only one person every time. The client doesnt engage through sexual intercourse even before the hospitalization. Interpretation: The client doesnt in engage anymore in any sexual activity. Analysis: Sexuality is an individually expressed and highly personal phenomenon whose meaning evolves from life experiences. Physiologic, psychosocial, and cultural factors influence a persons sexuality and lead to wide range of attitudes and behaviors seen in humans. There are no normal, universal sexual behaviors.
