MINUTES ON LLCS-MIS 03 March 2014 On behalf of the presence of the Provincial Directors of respective Provinces of Region IV-A, representatives

attended the meeting for further information as regards the procedure on synchronization of assessments into the system and appropriate guidelines on the submission and approval of Authority to Assess, as well as, other related matters. Note the following accordingly: 1. Provincial Offices are strictly directed to ensure that messengers or representatives shall exclusively prepare and submit the Authority to Assess only to TSSD II, at least one (1) week in advance preceding the scheduled date of assessment, preferably every Monday, through its receiving personnel, Mr. Alexander Gaela. It shall be subject to evaluation and shall be immediately assigned by authorized personnel of TSSD II; thereafter, forwarding the same for signature of the Regional Director or Assistant Regional Director, as the case may be. 2. Labor Laws Compliance Officer(s) shall only conduct assessment only after said Authority to Assess had been officially entered into the server. No inspection/assessment shall be conducted without going through this initial assignment, as approved by the Assistant Regional Director. 3. LLCOs shall regularly check their mobile system application (LLCS-MIS) installed in their devices for new assignments of establishment for assessment. If an LLCO does not have a tablet device, he can log on through a PC, using his assigned username and password, to access the system using this link https://webbapp.og90.com. 4. LLCOs are advised to carefully determine the information regarding the establishment prior to encoding into the system application installed in their tablets. If, at some instances of typographical errors, LLCOs may edit the information on their tablets.

5. LLCOs are strongly encouraged to observe proper subscript on case numbers for immediate determination and/or classification of cases. Please follow the present format as prescribed by the LLCS.

6. If, at some point, the establishment has no violations on General Labor Standards (GLS) and Occupational Safety and Health Standards (OSHS), it is advised that only one case number shall be used. However, if there are violations on either one of the two or both, please separate their case folders, as well as, the case number. Example: One case folder: RO4A-LPO-JA-0214-001-GLS/OSHS (No violation on GLS and OSHS) Two separate case folders: RO4A-LPO-JA-0214-001-001-OSHS (With violation on OSHS) RO4A-LPO-JA-0214-001-001-GLS (With violation on GLS) 7. Please use the current form of Authority to Assess. 8. TSSD II shall create its own e-mail address for specific purposes of LLCS implementation. Provincial Offices are advised to routinely check their e-mails for any updates and information. 9. It shall be the duty and responsibility of all representatives, who attended this conference, to communicate with the employees of their Provincial Offices; and, to guide them on matters that have been undertaken as of this date. Finally, please strictly observe the Memorandum, dated 27 February 2014, with regard the implementation of the new Labor Law Compliance System, as well as, the documents distributed during this discussion. We had gone through the discussion and fully understood all the concepts and guidelines of LLCS-MIS. Thus, being representatives of our Provincial Offices, we will provide thorough guidance and assistance to other LLCOs as regards the same.
