As part of the activities for Academia San Jos , the fund-raising committee is planning a Mothers Bazaar, which will

be held as follows: Thursday, April 24, 2014 9:00 AM to 7:00 PM San Rafael Auditorium Villa Caparra, Guaynabo, Puerto Rico We would like to know if you or someone you know would be interested in participating with a booth for this Bazaar. First choice on booth location will be given to ASJ parents and relatives, but other persons are welcome to participate. Since spaces are limited, we are asking you to call us or visit us as soon as possible to reserve your booth. Prices will be as follows: One table (72 x 30) Two tables (144 x 30) One table (36 x 36) $200.00 $300.00 $100.00 Now $150.00 Now $250.00 Now $ 50.00

A deposit of $100.00 is required in order to reserve your booth (deducted from the total booth price). The signed contract and balance are due no later than April 7, 2014. If you are interested, please complete this preliminary form and either bring it to the school, or sent it with a student, along with the deposit. You may also call the school for further details at the above telephone numbers. This agreement authorizes Academia San Jos to rent a space at the San Rafael Auditorium on April 24, 2014 from 9:00 AM to 7:00 PM for the sole purpose of having a booth (exhibition table) representing your company in which you may sell and advertise your products subject to the following conditions: TERMS & CONDITIONS: 1. 2. 3. 4. 5. 6. 7. Tables and merchandise may not be left unattended. Academia San Jos and Parroquia San Jos are not responsible for lost, stolen, or damaged articles. 8. All merchandise and decorations must be removed after 7:00 PM by the end of the activity. 9. A security guard will be on duty during the entire activity. 10. Proper behavior and attire will be required at all times. 11. No smoking is allowed inside the auditorium. 12. No liquor or other substances forbidden by law will be allowed at any time. 13. Academia San Jos reserved the right to accept or deny any type of merchandise not in keeping with the philosophy of the school.
