Instructions for Filling Up of OMR Application Form B

OMR Form B is to be filled up for the programs offered by the following Faculties/Centres:

Guidelines for filling up the OMR Form are given in Annexure III (Page 60) of the Prospectus. Instructions for Filling Up the Front Page (Side 1) of the OMR Form 1 Program Code: Fill the 3-letter program code in the boxes and also darken the appropriate ovals. Program code may be seen in column 3 of the tables given in Section 10 (Pages 16-24) of the prospectus. They are also written in the first column of the tables in Annexure I (Pages 25-57)..

Program Code for B.A. (Hons) Arabic is B01 Program Code for B.F.A (Art Education) is B12 Program Code for M.Sc. Bioinformatics is M28 (Pl see. Pages: 16 & 42 of the Prospectus 2014-15) (Pl see. Pages: 22 & 45 of the Prospectus 2014-15) (Pl see. Pages: 19 & 32 of the Prospectus 2014-15)

Preference: This field is applicable to programs with Combined Test (M58, G02, B01, B02, B03, B04, B05, B06, and B07) only. For such tests, the available programs have been shown with their corresponding Preference Code in Annexure I (Page 25-57) of the Prospectus. The candidate of a combined test program must fill in his/her preference(s) for admission in this field. Candidates of other programs may leave this field as blank. Example: Suppose you wish to apply for B01 group of programs. Look at the the entries shown in Annexure I (Page 42) of the Prospectus. If B.A. (Hons) Urdu is your First Preference, B.A. (Hons) Arabic is the Second Preference, B.A. (Hons) Persian is Third, and B.A. (Hons) Islamic Studies is Fourth, fill the preference column in the following way:

This field is applicable to candidates of B.F.A (Art Education) B12Program only. Indicate your choice for medium as English, Hindi or Urdu by filling the appropriate oval. Also write E or H or U accordingly in the box. Please refer to pages 8 and 59 of the Prospectus for specific details. Relaxation Under Special Provisions: Candidates claiming relaxation under NCC or NSS certification may fill the appropriate oval. Also write C or S accordingly in the box. The candidate should specifically read sections 4.4 and 4.5 (Page 8) of the Prospectus for details.

Instructions for Filling Up the Front Page (Side 1) of the OMR Form 5 6 7 8 9 Applicant's Category: Fill in the Category Code in the Box and darken appropriate oval. Candidates should specifically read section 3 (Pages 6-7) of the Prospectus for details. Foreign National: Fill 'Y', if the applicant is a Foreign National, else fill 'N' in the Box and darken appropriate oval. Kashmiri Migrant: Fill 'Y', if the applicant is a Kashmiri Migrant, else fill 'N' in the Box and darken appropriate oval. (Applicant has to provide proper certificate, at the time of verification of documents, to claim this category) Gender: Fill in your Gender Code and darken the appropriate oval. (M - for Male and F - for Female) Date of Birth: Enter date of birth as given in your High School (Class X) Certificate in the space provided and darken the appropriate ovals. (Example: If your date of birth is September 01,1996, fill it as shown below): D D M M Y Y Y Y 0 1 0 9 1 9 6 9 State of Domicile: Codes for Indian States and Union Territories are listed in Code Table-1 of Annexure IV (Page 61). Write the code for state of your domicile in the Boxes and darken appropriate ovals. Name of the Candidate: Write your name, in capital letters, as given in your High School (Class-X) Certificate of the Board/University. Each letter should be filled in one Box. Leave a Blank Box between first, middle and last name. You should also darken appropriate ovals under each Box. (Example: Candidate's Name : ASHISH WINSTON KHAN) A S H I S W I N S T O N K H A N H Qualifying Exam Details: Qualifying Examination Code: For each program, minimum eligibility qualification requirement is given in Annexure I (Pages 25-57) of the Prospectus. Identify the qualifying examination of your program of interest by consulting Annexure I and find out its code by referring to Code Table - 3 of Annexure IV (Page 62) of the Prospectus. In the Boxes, write code of the identified qualifying examination and darken the appropriate ovals. Example: Senior Sec. School Certificate is the qualifying examination for admission to B.A.(Hons) Mathematics(Program Code B08). As per Code Table 3 (Page 62), the code for Senior Sec. School Certificate (equivalent to XII) is 2. Write 02 in the Boxes as shown below): 0 2 Board/University: The codes for Boards/Universities are given in Code Table - 2 (i) & (ii) of Annexure IV (Pages 61, 62). Write the code of the Board/University from where you have passed the qualifying examination in the Boxes. Also darken the appropriate ovals. Example: If you are applying for M.A. (Hindi) and have passed the qualifying examination from Delhi University, the code is 223. Write this code in the Boxes as shown below): 2 2 3 Year of Passing/ Appearing: Write the year of passing the qualifying examination or if you are appearing in it, in theBoxes provided and darken the corresponding ovals. Example: If you have passed the qualifying examination or you are appearing in 2014, write as shown below: 4 2 0 1 Percentage: Fill the percentage of marks obtained in the qualifying examination which makes you eligible for the program based on the eligibility criteria given in Annexure I (Pages 25-57) of the prospectus. In case, you are eligible under more than one criteria, you may fill the higher percentage. In case of grades or Final Cumulative Performance equivalent percentage is to be filled. Example: If you have secured 507 out of 600, your percentage is 84.50%, write the percentage in the boxes as shown below: . 8 4 5 0 13 14 15 16 17 18 19 Mobile Number: Write your or parent's mobile number (For SMS Alert) in theBoxes and also darken the appropriate ovals. Pin Code: Write Pin Code of your mailing address in the Boxes and also darken the appropriate ovals. Mailing Address: Write your complete mailing address with Pin Code where you want your Admit Card to be delivered. Write in CAPITAL LETTERS only using Black Ballpoint Pen. Also write your Contact Telephone Number. Any request for change in address will not be entertained at a later stage. Photograph: Paste a recent coloured photograph of the size and specifications as described in Annexure III of the Prospectus (Page 60). Visible Mark of Identification: Write visible mark of identification. Candidate's Signature: You must sign within the Box using black ballpoint pen. Candidate's Thumb Impression: You must also put your thumb impression within the Box at the location mentioned in the Application Form.(left thumb impression for male/right thumb impression for female). Instructions for Filling Up the Back Page (Side 2) of the OMR Form 20 21 22 23 24 Mother's Name: Write your Mother's name in CAPITAL LETTERS. Each letter should be written in one Box. Leave a blank Box between first, middle & last name. Applicant should also darken appropriate ovals under each Box. Father's Name: Write your Father's name in CAPITAL LETTERS. Each letter should be written in one Box. Leave a blank Box between first, middle & last name. Applicant should also darken appropriate ovals under each Box. Educational/Professional Qualification: Applicant should write the detail of his/her qualification as per format. If result of an examination is not yet declared, the candidate should write R.A. under percentage (of aggregate). Intervening Period: Write gap period, if any, since passing the qualifying examination. (Refer to section 5.6(ii) (Page 10) of the Prospectus for details) Permanent Address: Write your complete permanent address with Pin Code. Also write your contact Telephone number. (Refer to section 5.6(ii) (Page 10) of the Prospectus for details)
