All of the required items listed below must accompany the application, which must be returned to your neighborhood school. Please mark all items you have included. Parent is responsible for submitting copies of all required data at the time of application. Previous years final report card, May/June 2012: Grades 3-12 applicants End of semester report card, January, 2013: Grades K, 1st, 2nd applicants Nationally-normed Achievement Test (Stanford/Aprenda or ITBS) within the last 12 months) Naglieri Non-verbal Ability Test-2 (NNAT-2) (current years score) Only one teacher recommendation form may be submitted per application. Only the first teacher recommendation form will be used and the online form takes precedence. *All current HISD Kindergarten and 5th graders will have their form completed online by their classroom teacher. For students currently in Kindergarten and 5th grade, this form will be completed online by the classroom teacher with collaboration from all teachers who instruct this student. For students in all other grades, this form should be completed by the current teacher, or teacher from the previous school year only.
