Research On Dietary Intervention as a Therapy for Behaviour Problems in Children with Gastrointestinal Allergy

Abstrak This brief study describes the reports of parents of dietery intervention as a therapy for 50 children with gastroenterointestinal symptom in food allergy. All of whom have been utilising a programme involving the exclusion of allergies foods from the diet an to evaluate of behaviour problem in children under 2 years old. Method The parents of 91 children with gastrointestinal allergy (aged 0 months to 2 years) who were utilising this form of dietary intervention and who were prepared to be interviewed were recruited. Diagnosis of CMA was based upon clinical criteria described by Goldman and measuring of total Ig E. The majority of the subjects were collect at Children Allergy Center Bunda Hospital jakarta on February 1, 2003 untill June 31, 2003.

Evaluation of clinical course done at 3rd weeks and 4th weeks after dieteary intervention. All parents had, by the time of interview, removed food allergy containing products from the diet with Rowe diets and evaluated for sleep patterns, aggressive, tantrum hyperactivity, socialize, enuresis, emotional and speech. Results The parents of all 50 subjects were aware of behaviour problems before the official diagnosis was made. Fourty children excluded of this study because had been infection, need some oral medicine and drop out. Conclusion Quite clearly this stydy can only be of a preliminary nature in behaviour problem in children with gastroenterointestinal allergy. Dietary intervention seems plays an important role in the management of certain that behaviour disorders. Behaviour problem in children with systemic features particularly with

gastroenterointestinal allergy may be approached from a complementary medicine model that recognizes the role of the abdominal nervous system with regard to etiology and treatment. 1. Preliminary There is a new research field in psikneuroimunologi aspect , the research reveals how the immune system and central nervous system bergubungan each other in affecting human health .. In a recent study it was found that allergies can affect various systems of the body including the brain . Gastrointestinal symptoms of food allergy can be explained due to a change in the transport system of the gastrointestinal tract wall (increased and decreased function skretori absorption ) , an increase permeabilizas and motility of the gastrointestinal tract . 2. Ways and methods Observation carried 91 children who received treatment at Children's Hospital Allergy clinic Bunda Jakarta with gastrointestinal allergy ages 0 months to 2 years , on February 1, 2003 to June 31, 2003 . Have conducted interviews with the parents and given appropriate instructions are given on the form for dietary interventions that will be given to their children. Gastrointestinal allergy diagnosis is made based on clinical criteria and modification Goldman total IgE measurements . Dietary intervention for all patients and clinical evaluation when the third week and fourth week . Dietary intervention was carried out while avoiding certain foods according Rowes Diet modification . As for the inevitable is cow's milk , certain fruits , eggs , chicken , fish and nuts . Evaluation by performing physical examinations and interviews of the parents to the symptoms of gastrointestinal disorders and behavioral disorders in children such as sleep , aggression , tantrums , enuresis , and speech disorders .

3. Results and Discussion Finally obtained from 91 children to 50 children entered the study , 41 children were excluded because of taking drugs , violate dietary interventions , pain control and not anymore .. Clinical manifestation is dermatitis found 48 ( 96 % ) , rhinitis 38 ( 76 % ) , cough 6 ( 12 % ) and wheezing 4 ( 4 % ) . While manifestations of gastrointestinal disorders obtained were diarrhea 28 ( 64 % ) , constipation 22 ( 44 % ) , vomiting 24 ( 48 % ) , sialore / " drool " 22 ( 44 % ) , halitosis 19 ( 38 % ) and GER 19 ( 36 % ) . Dermatitis patients as much as 96 % in patients with gastrointestinal allergy seems to indicate that there is a relationship between gastrointestinal disorders and dermatitis . In addition to direct targeted disruption of the skin and allergic to the gastrointestinal tract , metabolic disorders are suspected of sulfate in patients with gastrointestinal disorders . The sulfur metabolism disorder that can produce sulfite menggganggu skin disorders such as dermatitis . Obtained 36 ( 76 % ) children with impaired weight gain , while the nutritional status of malnourished obtained were 23 ( 66 % ) , good nutrition 10 ( 20 % ) and overweight and obesity were 17 ( 24 % ) children . Evaluation of the clinical manifestations of gastrointestinal disorders and behavioral disorders in children were performed at the third and fourth weeks after dietary intervention . The study as a preliminary study to reveal more clearly the relationship between children's behavior and gastrointestinal disorders due to allergies . Allergic dietary intervention seems very important role in the handling of developmental and behavioral disorders in children .
