PARENTAL ACKNOWLEDGEMENT OF RISK AND INFORMED CONSENT FOR PARTICIPATION IN BSS INTERSCHOOL ATHLETICS

Practices are held Monday and Thursday mornings at St. Michaels College 7:00 am and one other day at BSS pending practice space. Please check the BSS Athletics Weebly for schedules. All students in grade 9 12 are expected to be part of the BSS Athletics Google Community to receive athletics updates for scheduling. April 2 - CISAA Indoor meet at York University April 11 - Snowball Relays at St. Mike's May 2 & 3 - Sacred Heart Classic at Centennial May 6 - CDS meet at Country Day May 9 - Bill Crothers at Bill Crothers* - TBC May 13 & 14 - CISAA Finals in Oshawa May 16 - Bill Crothers at Bill Crothers* - TBC Week of May 20 - 2 days will be Metros at TBA (top 4 eligible from CISAA) June 5-7 - OFSAA at St. Marcellinus (top 4 from Metros) Note: Students are required to attend at least three meets. Students must attend the practices prior to the meets to be eligible to compete for BSS. Any student who finishes in the top four at CISAA championships will qualify for Metros. This consent form must be returned before competing on April 2 at York University. Below is the explanation of the risks involved in participating in Interschool Athletic Events.
The risk of injury exists in every athletic activity. Falls, collisions and other incidents may occur and cause injury. Due to the nature of some activities, injuries may range from minor sprains and strains to more serious injuries affecting the head, neck or back. Some injuries can be life impacting or prove to be life threatening. Injuries as a result of participation in an activity can occur without fault on either the part of the student, or The Bishop Strachan School and its employees or agents, or the facility where the activity is taking place. A student choosing to participate in the activity assumes the risk of an injury occurring. The chances of an injury can be reduced by carefully following instructions at all times while engaged in the activity. Students playing an outdoor sport are also subject to environmental conditions (ie: temperature, weather, air quality, humidity, UV rays, insects). All participants are reminded to have adequate liquid replacement and carry personal water bottles for hydration before, during and after physical activity. Mouth guards are mandatory for softball, field and ice hockey players. Shin Guards are mandatory for field hockey and soccer players. Ice hockey helmets must be CSA approved helmet. Ski helmets must meet the requirements of Alpine Ontario regulations for racing. Students will be transported to athletic events in either a taxi, school bus or coach bus. There is an inherent risk to bus and taxi travel. The Bishop Strachan School and CISAA attempt to manage, as effectively as possible the risk involved for students while participating in school athletics.

In the interest of safety, we strongly recommend:


Students have an annual medical examination. Students bring emergency medications (e.g., asthma inhalers, epinephrine auto injector) to all practices and competitions. Students remove eyeglasses during practices or games. If eyeglasses cannot be removed, the student must wear an eyeglass strap or shatterproof lenses. Students must be prepared to made aware of ways to protect themselves from environmental conditions (e.g., use of hats, sunscreen, sunglasses, access to liquid replacement, insect repellent, appropriate clothing) A safety inspection is carried out at home of any equipment brought to school for personal use in interschool practices and competitions (e.g., skis, skates, helmets).

Parents or guardians who consent to a student participating in a School activity should understand that such injuries can occur during School activities because of the inherent nature of the activity, and without neglect on the part of either the student or The Bishop Strachan School. IMPORTANT MEDICAL INFORMATION Students are responsible for bringing allergy medication such as Epipens or inhalers and any other prescribed medication required to all games and practices. (Add important medical information relevant to the particular excursion.) MEDICAL SERVICES AUTHORIZATION Should the student suffer injury or illness while participating in this activity, the undersigned hereby authorizes any teacher or third party contractor accompanying the student to authorize such medical attention for the student as may be deemed appropriate in the circumstances. The undersigned authorizes medical personnel to administer medical and/or surgical services, including anaesthesia and other drugs. In the event of the students illness or accident, the parents or guardians will be notified as soon as possible. STUDENT ACCIDENT INSURANCE The Bishop Strachan School does not provide accidental death, disability, medical or dental insurance on behalf of students participating in the described activity. The undersigned agrees to maintain appropriate medical insurance coverage for the student. Should the student suffer injury or illness while participating in this event, the undersigned agrees to bear the medical and dental costs required. IMPROPER BEHAVIOUR The chances of injury occurring during School activities will be reduced when the student carefully follows instructions at all times, and wears appropriate clothing and equipment. Should the student fail to obey the rules and regulations prescribed by the School and by the BSS Code of Conduct while participating in the activity, the School may, at its sole and absolute discretion, terminate the students participation without refund. Any additional costs incurred as a result of the student being sent home will be the responsibility of the undersigned. PERMISSION TO PARTICIPATE The undersigned understands the risks inherent in the activity and gives permission for the student to participate in the described activity.
