SUBJECT: MEETING WITH PUBUC WORKS STAFF AND UNION REPRESENTATIVE
On October 23, 2002, Donald Freedland, Director of Public Works, Bernard Adams, Grounds Maintenance
Manager, Rickey Reed, Grounds Maintenance Manager, William Jackson, Equipment Operator I, Basil Farrington,
Union Representative, and William Gault, Union Representative, and I met to discuss an incident wltich took place at
the Public Works facility involving Melissa Woodling, Equipment Operator I. We bad requested that Ms. Woodling
attend the meeting so both sides could air their views, however, Ms. Woodling declined the invitation and asked that
William Gault represent her. Ms. Woodling bad reported to William Gault that William Jackson was standing near
the telephone in the crew quaners and his presence made her uncomfortable.
Mr. Jackson said that be was using the telephone in the crew quarters when Ms. Woodling entered the room and he
continued using the telephone. Mr. Jackson stated that he did not look at Melissa and could not understand why he
was making her upset when be took every measure to make sure that be bad no contact with her.
After a discussion about the situation, it was established that Ms. Woodling was not accusing Mr. Jackson of doing
anything to her, however, in the morning when she punched in on the time clock, she preferred if Mr. Jackson would
not be in the area since it is a confined space.
I explained to the group that Mr. Jackson was also an employee and had a right to use the facility as any other
employee. If Mr. Jackson was not threatening or bothering Ms. Woodling, then the City could not stop him from
punching the time clock, or using the telephone or facility. I believed that as a team, we needed to bring some
balance to the issue. I felt that there were some actions Ms. Woodling could take that would improve the situation
with Mr. Jackson. Again. we asked that Mr. Jackson use the time clock and leave the area immediately since Ms.
Woodling had a roblem with him standing there.

Written Reprimand
On Thursday, January 2, 2014, I was notified that you had just turned in approximately 15-20
"Dump Tickets" dating back to February, 2013, that had obviously not been submitted to the
Recycling and Solid Waste Department.
Your failure to deliver these Dump Tickets to Recycling/Solid Waste in a timely manner resulted
in an unnecessary burden on Recycling/Solid Waste staff to reconcile these tickets, especially in
regard to the tickets for Fiscal Year 2012/2013, i.e., the Fiscal Year ended on September 30,
2013.
As you are/were well aware, dump tickets are to be turned in daily. Your negligence in failing to
perform this minimally-demanding duty over such a protracted period of time suggests that the
non-performance of this duty was both deliberate and insubordinate.
Accordingly, you are being issued this Written Reprimand for substandard/unsatisfactory work
in violation of the following provisions of Article 21 (Employee Discipline), Section 2
- An Equal Opportunity Employer -
(Grounds for Employee Discipline) of the Collective Bargaining Agreement between the
Deerfield Beach Unit of Local 101 0/Fiorida District Council 78 of the International Union of
Painters and Allied Trades (IUPAT) and the City of Deerfield Beach (City):
(c) Has violated any lawful and reasonable regulation or order, or failed to obey any lawful
or reasonable direction made and given by a superior;
(g) Has been guilty of acts which amount to insubordination or of disgraceful conduct;

(k) Incompetence, inefficiency or repeated neglect of assigned duties; and
( q) That the employee is antagonistic in his attitude toward supervisory or fellow employees
... to the detriment of efficient public service.
If your substandard/unsatisfactory work performance continues, further disciplinary action (up
to, and including, termination from employment for cause) will be taken.
Please contact your Immediate Supervisor should you have any questions regarding this
Written Reprimand.

mowing along Dixie Highway & SW 5th Street "without" wearing hearing protection.
You are fully aware that personal protective equipment (PPE) is mandatory for city employees.
Please wear your PPE.
This is negligence in the performance of your job duties. If this sub-standard I unsatisfactory
work performance continues progressive steps of discipl inary action will occur up to and

SUBJECT: Suspension Without Pay for Preventable Accident
On May 16, 2005, you were involved in an accident that was determined to be preventable. You did not
exercise the proper safety practices and skills necessary when operating City vehicle #8814, the aerial
truck. By not raising the driver's side out rigger and driving down MLK Jr Avenue, the outrigger struck the
median curb, breaking the curb, bending the outrigger and even breaking out the rear window of the
vehicle causing an estimated total of $13,604 in damages to the vehicle. In addition if we lease a
replacement vehicle for the four weeks it is estimated to take for repairs the City faces an additional
expense of $3,600 in lease fees. In addition your work partner was in the bucket while you were moving
the vehicle and could have been even more seriously injured.
You have been involved in three accidents (December 2004, September 2004 and December 1999) with
City vehicles with trailers attached where you were backing up, jackknifed the trailer and indicated you
"forgot" the trailers were attached. When we met to discuss this accident you indicated you "forgot to
raise the outrigger. Your forgetfulness is dangerous as well as expensive. This accident is a violation of
Personnel Rules and Regulations, Article IX, Disciplinary Action, 9.03 (b) Violations of safety rules and
practices.
You are an experienced operator with eight years on this vehicle and should have a better understanding
of the vehicle you are operating than most In addition, you need to pay attention and be more aware of
your surroundings as you operate City equipment. We expect that in the future, you will make better
judgments while operating City vehicles and equipment
At this time because of the severity of the damage done to the vehicle, I am fecommending to the City
Manager that you be suspended for five (5) days without pay. If you fail to show satisfactory improvement

problem I instructed you to report to the site with out the truck or trailer which you never arrived
nor communicated to me that you incur any other problems with a vehicle. As stated in the City of
Deerfield Beach employee hand book page 52 9.03 section C says failed to obey any reasonable
direction made and given by a superior.

activity went on during a 40-hour workweek in the past four weeks we have no records
of your work sheets in our file. This is unsatisfactory in the city Employee's handbook
section 9.03 page 53 (k) incompetence inefficiency of assigned duties. This must be
corrected immediately work sheet must turn in every Monday of every week if this not

SUBJECT: Incident Report
On May 16, 2005 at 1 :1 0 pm William Jackson operator three was involve in accident at
S.W. 3rd Ave & Hillsboro with truck 8814 (aerial truck) our investigation found that
William Jackson has eight years of experience on this truck William should have double
check to see that the outriggers was in up position. Estimate cost for repair is $ 13.604
It could take up to four weeks to compete repairs leasing a truck of this size would cost
$350.00 a day or 900.00 dollars a week. Gloria Jones operator one was in the bucket at
the time of incident both employee' s was driven to minor emergency center with
supervisor Trent Lacy. Will iam Jackson check out ok Gloria Jones is under going
treatment for tenderness in the neck. Our recommendation is five days suspension
without pay.
