Faculty Heather Davis, MS, EdD(c), NREMT-P Paramedic Education Program Director Ms Davis is a Board member for the National Registry of EMTs and a former member of the Board of Directors for the National Association of EMS Educators. She is currently earning a doctorate in educational psychology from USC and is a published author and national speaker. Steven J. Rottman, MD, FACEP Medical Director, Paramedic Education Program Dr. Rottman is a senior attending physician in the UCLA Emergency Medicine Center and a professor of Emergency Medicine & Community Health Sciences at the David Geffen School of Medicine and the School of Public Health at UCLA.

Baxter Larmon, PhD, MICP UCLA Center for Prehospital Care Director Dr. Larmon is also co-founder of the Center for Prehospital Care; Professor of Medicine at the UCLA School of Medicine, and is a cofounder of the UCLA CPC Prehospital Care Research Forum. Dr. Larmon is a nationally renowned EMS expert and presents at many national and international conferences yearly.

Atilla ner, MD, MPH, FAAEM Associate Medical Director, Paramedic Education Program Dr. ner is the Medical Director for the UCLA EMS radio base station and participates regularly in training with FEMA Urban Search and Rescue team CA TF-1. Dr. ner is an Assistant Professor Medicine at the David Geffen School of Medicine at UCLA, and is an Attending Emergency Physician at the UCLA Medical Center and at Antelope Valley Hospital.

Justin McCullough, BS, EMT-P Paramedic Program Clinical Skills & Clinical Internship Coordinator (UCLA alumni 99) Mr. McCullough has a bachelors degree in Biology from Loyola Marymount University. He also continues to work as a paramedic and paramedic supervisor.

Brian E. Wheeler, AGS, EMT-P, CCEMT-P Paramedic Educator, Paramedic Education Program Mr. Wheeler is a licensed Paramedic and a certified Critical Care Paramedic. He has worked in both rural and urban systems for over 15 years as a ground and flight paramedic, critical care paramedic and EMS Systems Manager.

Nanci Medina, BS, NREMT- P Paramedic Education Field Coordinator (DFH alumni 1985) Ms. Medina attended Loma Linda University where she received her Bachelors degree in Emergency Medical Care. Ms. Medina continues to work as an active paramedic in the City of Sierra Madre.

Significant Dates/History
Dr. Walter Graf shared with Los Angeles County Supervisor Kenneth Hahn his unique idea of having County firemen trained for a coronary unit on wheels. They would provide vital assistance and bring heart attack victims to a hospital minutes after an attack. January 1970: The Board of Supervisors approved two pilot studies on the training and use of Mobile Intensive Care Paramedics: one study was implemented at Daniel Freeman and one at Harbor General Hospital. . Dr. Walter S. Graf May 1995: Daniel Freeman Hospital Paramedic School celebrates 25 years of paramedic training.

January 1999: Daniel Freeman Hospital and the UCLA Center for Prehospital Care unite forces to offer a joint Paramedic Education Program for the Los Angeles City Fire Department.

July 1970: Paramedic training in California became possible when Governor Ronald Reagan signed the historic Wedworth-Townsend Act on July 15th

September 2000: The UCLA Paramedic Education Program hosts the first class with Los Angeles County Fire Department firefighters.

August 1970: Paramedic Training in California is initiated at Daniel Freeman Memorial Hospital under the direction of Dr. Walter S. Graf. The first class graduates are certified as Mobile Intensive Care Paramedics. June 2006: The Paramedic Programs celebrates its 35th Year Anniversary as part of the graduation for the September 2005 held on June 10, 2006.

October 2007: The UCLA Paramedic Education Program successfully continues its accredited status from the Commission on Accreditation of Allied Health Education Programs. January 2009: The UCLA Paramedic Education Program begins its 30th program.

May 1973: The State of Utah elects to have Daniel Freeman Hospital Paramedic School train the first paramedics to function in that state.

January 1975: Daniel Freeman Hospital Paramedic School initiates training on an on-going basis, offering five classes yearly.

May 2010: Dr. Walter S. Graf, founder of the Daniel Freeman Paramedic Program, was honored as one of the pioneers of paramedicine in the United States at the inaugural "Lifetime Achievement Award Ceremony" in Los Angeles.

October 1980: Daniel Freeman Hospital Paramedic School, with graduates serving on emergency medicine teams around the world, becomes the first paramedic school in the country to be accredited by the American Medical Association.

July 1990: Daniel Freeman Hospital Paramedic School celebrates the 20th anniversary of paramedic training and honors Walter S. Graf, M.D. and Supervisor Kenneth Hahn, for his contributions to the emergency medical services system.

August 2010: The UCLA Paramedic Education Program celebrates 40 years of delivering exceptional paramedic education. December 2010: Program Director Heather Davis named EMS Educator of the Year by the State of California EMS Authority.

July 2014: UCLA delivers the very first paramedic hybrid program in Los Angeles County. Present Day: The UCLA Paramedic Education Program continues to successfully graduate over 120 paramedics per year. .

About the Program


The UCLA Paramedic Education Program is part of the UCLA Center for Prehospital Care and is located within the Walter S. Graf Center, in its original Southern California location. Our program was the first accredited program in the country. The UCLA Paramedic Education Program is accredited by the Commission on Accreditation of Allied Health Education Programs upon the recommendation of the Committee on Accreditation of Educational Programs for the Emergency Medical Services Professions (CoAEMSP). Course Descriptions
Our paramedic education course is offered in 2 formats: a traditional in-person course and our brand new hybrid course designated by this symbol ( ). Both courses are divided into three phases: didactic (classroom), clinical (in-hospital) and field internship (assigned to an active paramedic unit) for a total of 1230+ hours. Didactic (540 hours) During the phase, cognitive and psychomotor learning occurs through lectures, interactive presentations, skills labs, and simulations. During the didactic phase of the traditional course, you will attend class in person on Mondays, Tuesdays, Thursdays, and Fridays (some Wednesdays too). Classes typically meet as scheduled from 8:00am to 5:00pm on lecture days and 9:00 am to 6:00 pm on skills days over 16 consecutive weeks. During the didactic phase of the part-time hybrid course, learning also occurs via interactive on-line modules, on-line instructional sessions, and on-line discussions. Synchronous online sessions will occur each Wednesday from 6-9pm PST. On-line participation is required. Interactive online modules and quizzes can be completed anytime, but are due before midnight each Monday night. Students will come to campus for one long weekend per month consisting of Thursday, Friday, Saturday and Sunday (dates as scheduled). The meeting times for on-campus days are 8am-6pm). Didactic (combined online and on-campus sessions) for the hybrid course runs for 32 consecutive weeks. Successful completion of this section requires a minimum of 80% on all written exams. The goals of the classroom phase include development of core foundations in cognitive and psychomotor knowledge of anatomy, physiology, pathophysiology, assessment, pharmacology, trauma, and medical complaints, and treatment modalities in order to prepare students to interface with real patients in the remainder of the program. Please note that there is no advanced placement available for the course. We also do not accept transfer credits into our program nor can we offer credit for experiential learning. You must attend the entire course. Clinical (212 hours: 172 hours minimum plus 40 hours of call back time) In the clinical segment, students are rotated through affiliated hospital clinical areas that may include an emergency department, anesthesiology rotation, labor & delivery, pediatrics, etc. Students function on a one-to-one basis under direct supervision of a nurse or physician. The clinical phase is approximately 6-8 weeks in duration. The assigned tours of duty include day, evening, and night shifts so that the student will receive training and exposure that will closely simulate actual prehospital care functions. Field Internship (480 hours minimum) During the field internship phase you are assigned to an active affiliate paramedic unit in Los Angeles County for a minimum of twenty 24-hour shifts spread out over 8 weeks on a typical fire department

schedule. (Extensions due to student performance are only for a maximum of 10 shifts.) While in this rotation, you will perform the full scope of practice of an EMT-Paramedic, under the direct supervision of at least one certified EMT- Paramedic with at least two years field experience. Field internships are arranged under the auspices of, in conjunction with, and are monitored by our Paramedic Program.
Please note: 1. Dependable transportation is a must. Students accepted into our Program will be assigned to work in one or more hospitals in Los Angeles County, which may be a distance from your residence. 2. Our students are counseled against outside employment during the didactic phase of training. In addition to eight hours of instruction per day, the home study load is tremendous. 3. Field internship assignments will occur on a traditional fire department schedule. Hybrid students may need to plan for vacation or leave of absence from their employer in order to complete the field internship portion of the program. 4. Online coursework challenges students both academically and motivationally. Students should ensure they have the self-discipline to complete a challenging course online before enrolling in the paramedic program.

The Application Process


Come to our Student Information Session to learn about how to be the best prepare for applying to the program including tips about the requirements, interviewing, and more. The application process for our Program includes: 1. Entrance Exam 2. Your Application 3. An Oral Application Review 4. The Decision: Accepting and Enrolling The application and enrollment process for each individual program typically begins 5 to 6 months prior to the start date of each program. For specific details, read more below. 1. The first step is for you to take the entrance exam. You must take the entrance exam BEFORE the application deadline. To register on-line for our entrance exam (EMT test & WONDELIC assessment exam) and to find out how to order a practice test, visit www.cpc.mednet.ucla.edu/course/paramedicentrance-exam . Your test scores are e-mailed to you usually 1 week after the test date. Along with your test scores, you will be notified that you: You successfully completed of the entrance exam or That you failed to meet the minimal passing test scores. You can retest up to 2 more times in a 12month period. 2. In addition to the entrance exam you must submit a complete on-line application along with submitting all supporting documents by the designated deadline. (See page 2 of this application packet for current application filing periods). If you take and pass the entrance exam, submit a complete application packet by the designated deadline (and meet the minimal requirements) you will be notified by e-mail about one of the following two possible scenarios: 4. Your meeting (date and time) with our faculty to discuss your application and qualifications. That your application was not selected for a meeting with the faculty for an in-person application review. You can visit with us to learn more about how to improve your future application & test scores!

If you attend the oral application review will typically be notified of your status in the program within 30 to 60 days prior to the start of the program. Possible notifications include: That you are accepted! You will get your precourse packet and tuition payment deadline! Due to the overwhelming number of excellent candidates, not all eligible applicants are offered admission. If you are placed on the alternate list, you will be given a specific deadline by which we will give you the notification of your final status for the program. Applicants that are not offered enrollment into the program will also be notified. Our notification includes specific instructions and deadlines on how to apply to upcoming available programs. Remember that you can visit with us to learn more about how to improve your future application!

Before the Course Begins If you are enrolling, you will be given the pre-course study materials approximately four (4) weeks before classroom instruction begins. You will have to attend the mandatory Program Orientation intended to ready you and your support network (e.g., family, friends, etc.) for the program. The orientation is typically held on a Saturday afternoon or evening approximately one month before the start of the course. What about Early Decision? The Early Decision process allows you to apply during the January or May filing periods of any given calendar year and if accepted, be able to reserve a seat in any of the programs offered during the remainder of that same calendar year. Applicants for Early Decision must meet all application requirements and deadlines for that filing period. The number of seats available for the Early Decision applicants in each program is limited to half of the total number of seats available to private students in each class, typically four seats per class. Notification of decisions regarding Early Decision applicants will be made at the same time as regular applicants.

About the Other Costs Tuition does not include additional fees for required textbooks, laboratory materials, uniforms, certifications, identification, and optional college credit. Required textbooks are approximately $320.00 for textbook/workbook/supplement packages. Additional optional recommended textbooks are available for purchase with prices ranging from $17.00 to $170.00 (specific information on textbooks is included as part of a students admission packet). Specific electronic book and on-line resources will be announced soon for the hybrid course. Clinical equipment: Students must bring their own stethoscope and goggles as of the first day of class. The typical price for stethoscopes varies from $25.00 to $200.00 and goggles typically range from $10.00 to $60.00. Students must also furnish an appropriate uniform for clinical and field internships. The clinical uniform consists of Program-approved scrubs. The scrubs are available for purchase during the didactic portion of the program and cost approximately $35.00 for a set (top and bottom). Students must also furnish an appropriate uniform for field internship. The blue top and trousers typically range in price from $20.00 each piece to $100.00 each piece for items made with fire-retardant materials (e.g., Nomex). Students are required to purchase a program uniform patch during didactic to be worn during the field internship. The patch costs $5.00 a piece. Optional wearable items bearing the program logo are available for purchase during the didactic portion of the program. Electronic skill database access and testing fees (FISDAP) are required to record laboratory, clinical and field skills attempts. This same service is required for the Online Summative Paramedic Exam (OSPE) in preparation for the National Registry Exam. This fee is $100. UCLA Photo Identification Fee: $37.50 Certification fees for paramedics are approximately $742.00. The following fees are typically due towards the end of the didactic portion of the Program: $225.00 National Registry test site fee for UCLA Paramedic student for the practical exam, $110 National Registry Application Fee for the computer-based written exam, and $125.00 Los Angeles County Accreditation. Additional fees are typically required and payable to the designated agency: $250.00 State of California Licensing Fee, estimated $32.00 LiveScan Fee, etc. College credit is available to students successfully completing the traditional program. The cost for for this optional service is $280.00 (see the Student Service section for additional details). College credit is not available at this time for the hybrid course. Reliable Internet Access is required for some course assignments. Computer and internet access available on campus, or home or public computers can be utilized. Reliable internet access is absolutely mandatory when attending our hybrid course as this will allow you easy access to the course, the course companion web site, grades, on-line office hours, and much more. Access to our course companion website will be distributed 1 week prior to the start of the class. You will also need to have a headphone and microphone for VOIP (voice over internet IP) to participate in the weekly on-line class sessions.

Our Refund Policy


The refund policy is also listed in the Course Policy Manual available on-line and given to students at the time they are accepted into the program. Should the Paramedic Education Program cancel a class after a student has been accepted, all monies paid by that student shall be refunded in full. Any tuition monies paid, not including the non-refundable deposit of $950 will be refunded if a student cancels or is terminated no later than 30 days before the start of the course. If a student cancels or is terminated from the course 29-15 days before the start of the course, the UCLA Paramedic Program will in good faith make every effort to backfill the canceled/terminated enrollment (spot). If the spot is filled, the student will be refunded any tuition balance paid minus the non-refundable deposit of $950. It is to be clearly understood, that the student remains financially responsible for the tuition balance if the spot remains unfilled despite the Programs efforts to fill it. There is no refund if a student is terminated or withdraws from the Program 14 days or less prior to the start of the course There is no refund if a student is terminated or withdraws from the Program after the start of the course including the clinical or field internship portions of the program. Notice of a cancellation or withdrawal being made before the start of the Program must be made in writing to the Registration Coordinator (Michael Gudger). Notice of a cancellation or withdrawal during the Program must be made in writing to the Program Director, Ms. Heather Davis. Textbooks, laboratory materials, testing and certification fees are non-refundable for any student. There is no refund if the student is terminated for academic or administrative reasons at any juncture of the program.

The Disclosure
We reserve the right to cancel a specific course at any time due to under enrollment or other extraneous circumstances. The Program is not responsible for any expenses or inconvenience that may be incurred by an enrolled or prospective student as a result of a specific course being cancelled. Should the Paramedic Program cancel a class after a student has been accepted, all monies paid by that student will be refunded in full.

Your Banking You are encouraged to contact your own employer, local credit union or bank and inquire about what private loan services they offer you as a member.

Payment Plans
For the traditional paramedic course: If you are accepted to the Program, you have the option to pay your tuition in full or use our new payment plan option. Either tuition payment option includes the nonrefundable deposit of $950. To enroll into the payment plan you have to pay a down payment of 1,570.00 plus a $35 enrollment fee. The remaining balance can be paid in 1 to 5 monthly payments during the first months of program. For the hybrid paramedic course: If you are accepted to the Program, you have the option to pay your tuition in full or use our new payment plan option. Either tuition payment option includes the nonrefundable deposit of $1,600. To enroll into the payment plan you have to pay a down payment of $7,425 plus a $35 enrollment fee one month before the course begins. The remaining balance can be paid in 1 to 7 monthly payments during the first months of the program. Veterans Assistance Benefits (GI Bill)


The traditional paramedic course is approved for the GI Bill. The hybrid paramedic course is not approved for the GI Bill at this time.

Our Paramedic Program is approved by the Department of Veterans Affairs (DVA) to certify enrollment for eligible veterans. We have a long history of supporting the educational endeavors of eligible veterans through the DVAs Montgomery GI Bill (Chapter 30) and other eligible GI Bills including the new Post9/11 GI Bill (Chapter 33). For purposes of the VA, the Paramedic Program is considered a full-time (clock hour) program. As a veteran, you must follow and meet the Programs established eligibility, enrollment, and tuition payment requirements. Program Scholarship
Michihito Mitch Sekine was a graduate of the September 2005 UCLA Paramedic Program. To honor Mitchs memory, his family and friends established The Mitch Sekine Memorial Scholarship Fund to help support the education of future paramedic students at the UCLA Paramedic Program, Mitchs beloved alma mater. The Fund will award a scholarship, in the form of a partial tuition waiver, to a student in each UCLA paramedic course. Bound Tree Medical has assisted children of EMTs and Firefighters interested in becoming EMTs, or furthering their certification by becoming Paramedics. Scholarships are awarded nationwide for those who fit the eligibility criteria and are interested in pursuing a state approved certification program. Applicants must show financial need, hold an EMT Certificate with plans to become a paramedic and currently live in Ventura, Santa Barbara, Los Angeles, San Bernardino, Riverside, Orange or San Diego counties with preference given to Ventura County residents. Private Lenders A list of approved lenders for private loans can be obtained when visiting the WLAC Federal Financial Office. Remember that you need the on-line application (FAFSA) in order to apply for private lender loans. Consider personal loans through your bank and credit union which are independent of the financial aid process.

Federal Financial Aid


The traditional paramedic course is approved for Federal Financial Aid. The hybrid paramedic course is not approved for Federal Financial Aid at this time.

Through our relationship with West Los Angeles College (WLAC), our Center is able to offer federal financial aid as one possible option to assist eligible students of the UCLA Paramedic Program to recover some of the costs of tuition. Be sure to come to our Student Information Session for more information about how the financial aid process works in the Program! If you enroll with West Los Angeles College (WLAC) for college credit, once you are accepted to the Paramedic Program, you are then eligible to apply and receive Federal Financial Aid. You are absolutely encouraged to secure other funding either prior to or when applying to the Program as your eligibility for receiving Federal Financial Aid, the amount received, and the schedule of payments to you will absolutely vary. If you are accepted into the Program you will be expected and required to meet deposit and tuition deadlines without exception. Please note that the tuition deadlines established by the Program are firm and do not coincide with the distribution of financial aid funds so you should NOT count on it to meet your tuition deadline as financial aid can take 16 weeks from the start of the program to start your payments. So What Can You Do About Federal Financial Aid Right Now? You should apply for financial aid at the same time that you apply for the paramedic program. If you wait until you are accepted into the program to apply for a loan, this will further delay the process for you. Keep in mind that you need the on-line application (FAFSA) in order to apply for federal and private lender loans. For Paramedic Programs beginning between July 1, 2014, and June 30, 2015, students can submit a FAFSA application for the 2014-2015 Financial Award Year. You apply for financial aid by completing the official financial aid application [Free Application for Federal Student Aid (FAFSA)] available at www.fafsa.ed.gov The WLAC FEDERAL SCHOOL CODE IS: 008596. Do not use the UCLA federal school code your financial aid application will not be processed under the UCLA school code. A Word About Other Student Services


College Credit The traditional paramedic course is approved for College Credit. The hybrid paramedic course is not approved for College Credit at this time. Through a relationship between the UCLA Center for Prehospital Care (CPC) and the West Los Angeles College (WLAC), students enrolled in the Paramedic Program and who successfully completing the program are eligible to receive academic credit (up to 36 units). WLACs fee for this service is $280.00 for the total program ($35 per each of the 8 courses that make up the Paramedic Program), which is in addition to the course fees you pay to UCLA (UC Regents). If you enroll for the college credit option you are eligible to apply for Federal Financial Aid through the WLAC Financial Aid Office (see Financial Resources section on the next page for more details).

Students with Disabilities Our Paramedic Program complies with Federal and State laws and University of California policies by providing reasonable accommodations during the programs entrance exam and in the course itself to students with disabilities, including students with learning disabilities. Reasonable accommodations are defined as accommodations that do not fundamentally alter the program. Eligibility Requirements
Come to our Student Information Session to learn about how to be the best prepare for applying to the program including tips about the requirements, interviewing, and more. To be eligible for enrollment into the Program, you have to: 1. Be currently certified as an EMT (EMT-1/EMT-II/AEMT) in the state of California and maintain EMT certification throughout the Program. Have at least six (6) months of work experience as an EMT-1 in a prehospital setting within the past two years. You can apply with 5 months but you must have a minimum of 6 months if you are invited to an interview. Work experience may include full-time, part-time or volunteer work. A majority of successful applicants have a minimum of one year of full time experience (or equivalent). [ **Note: You can apply with only 5 months of EMT work experience at the time you submit an application - as long as you keep working and have a minimum of 6 months by 4 weeks after the application deadline.] Receive a qualifying score on the UCLA Paramedic Education Emergency Medical Technician written examination and WONDERLIC exam. WONDERLIC is a computer-based examination that tests a student expertise in math and reading comprehension. Test scores are valid for two years from the date the exam is completed. Submit a complete program on-line application and all supporting documents. Show a positive appearance, motivation, and attitude. (In the clinical and field internship segments, the students will be involved with patients, nurses, and physicians in both hospital and field settings, and must maintain a high degree of professionalism). Receive a qualifying score on the application review. Meet with the program faculty, for eligible applicants.

What Else Will You Need to Do?


Only if you are ACCEPTED into the Program, will you need to provide proof of the meeting specific medical and background check requirements. The medical requirements include having health insurance, proof of a basic physical and a series of immunizations. The background check requirements include a criminal background check by the FBI and DOJ (LiveScan), and verification against the General Services List of Parties Excluded from Federal Programs and the OIGs List of Excluded Individuals/Entities.

APPLICATION REQUIREMENTS

The Paramedic Program application is available on-line at: www.cpc.mednet.ucla.edu/paramedic-application You will need to upload these documents with your on-line application:
1. A copy of your current CA EMT-1 certification, renewed within the past 12 months. (EMT certification must be maintained current throughout the paramedic training.) 2. A copy of your current American Heart Association (AHA) Basic Life Support for Healthcare Providers CPR card, renewed within the past 12 months. (CPR certification must be maintained current throughout the paramedic training.) 3. A copy of your valid state drivers license (Class C) or state identification card. 4. A copy of your high school diploma, certificate of completion, GED, or transcripts. 5. Official college transcripts or a copy of college diploma, if applicable. 6. Verification of experience in the form of an original letter(s) on letterhead from your former and current EMS employer(s), with original signature(s). (A SAMPLE LETTER IS AVAILABLE ON-LINE.)

In addition to the items listed above, you will need to complete the UCLA Paramedic Education Program Entrance Exam before the application deadline and three (3) recommendations to the Program are needed. You will submit your references contact information as part of the on-line application. We encourage you to contact your references in advance and ask them to complete the on-line reference form when they receive it from the Program. The recommendations must be provided by one person from each of the following categories: 1. Your current EMS employer 2. Your current or former professor or EMS instructor you must have been an enrolled student in the instructors class and taught by this instructor. 3. Another professional or educational reference (e.g., paramedic, instructor, etc.)
