Critically analyze the instrument, tool, or form you use when conducting an initial patient or resident admission assessment at a hospital, or other health care agency in terms of its relevance to the health and nursing needs of persons from diverse cultures. From a transcultural perspective, identify the strengths and weakness of the admissions instrument. What suggestions would you make to enhance the effectives of the instrument in assessing clients? Be sure to consider the practical constraints that you face in the current managed care environment, such as time limitations, external factors that require you to care for increasingly large number of patients, and other constraints before you suggest modifications.

Since I am not affiliated to any hospitals at the moment, I decided to search for patient admission form in the internet and I found the form utilized by Grace Hospital in the United States. The patient admission form is very comprehensive wherein it includes the demographic profile of the patient, a questionnaire regarding past medical history and allergies to food, etc., and consent for admission, which is very essential in the course of treatment of the patient. Everything that is included in the patient admission form is relevant because these data are needed so as to provide appropriate/ accurate interventions to patient. For example, data about allergies; it is essential to prevent any allergic reactions toward the patient. If no data is collected about allergies of the patient, then surely, allergies will be imminent to the patient- instead of treating the patient, it will add up to the patients burden. It is also noticeable that ethnicity is included in the demographic profile of the patient and this connotes that the institution is geared towards cultural competence. It is good to know the ethnicity of the patient so that proper communication will be observed and stereotyping and prejudices will be avoided. Through this, quality and culturally sensitive care will be rendered to patients. The strength of the patient admission form is that ethnicity is included in the demographic profile of the patient. However, it is still lacking which is one of its weaknesses. I therefore suggest that the institution should focus more on the ethnicity of patients, gives emphasis on it, and have a thorough assessment about it. Cultural practices and beliefs should also be included as part of the patient admission assessment so as to understand patients holistically and respect them fully. Lastly, another weakness of the patient admission form is that it is only written in English language. Yes, it is indeed that English is the international language but we cannot remove the fact that other people cannot fully understand and know how to speak English. Because of this, there is a tendency that patients will not be able to give accurate answers because they dont understand/ comprehend the language well. Thus, I suggest that the admission form should also be written in different language that is culturally sensitive.
