Case Presentation Form Assignment 1. Nicole Caron 2. Students first name- Student # 1, Age-15 , Grade-9th , Gender- Male , Sexual OrientationUnknown , Race/Ethnicity-Multi-Ethnic , Religion-Unknown . 3. Session Information: This is the second session I have had with this student, but first real session. First session was just meeting him, talking briefly, and giving consent. This session lasted 15 minutes. 4. The Presenting Issue: Subjective: The clients presenting issue is that he is struggling to understand what assignments are due for each class and struggling with time management. He also gets easily distracted in class and feels weak because he was recently dropped from the football team because of ineligibility. Objective: The client is well groomed. He has good attendance now, but missed school a few times in the beginning of the year, had 3 excused absences. The client is well liked by his teachers and peers. Teachers report the client is very engaged in class but often does not turn in assignment work and gets distracted easily when working in a group setting. The client currently has three Fs and one D-. Teachers are worried he is not going to pass his classes this year because his grades are so low. The client was a member of the JV football team but recently became ineligible because of his thee Fs. Assessment: My hunch is that the client has a difficult time with the transition from his previous school to this school, where the teachers move at a very quick pace and. These issues are impacting the clients grades and he is feeling so overwhelmed with his assignments that he does not know where to start. Not only are his grades suffering, but his interest of playing football is as well. I also think the clients confidence is down because of the failing grades and the ineligibility. Plan: I am going to continue to meet with this student on a weekly basis. I also referred him to a National Honor Society peer tutor for extra support with his academics. I also think it might benefit the student to have teachers give him a copy of the in class notes for each class period. I also created a tutorial schedule for the student so he can get caught up on his missing work.

5. Multicultural Factors: The client is multi-ethnic in a predominately Caucasian school. This is not something I have explored yet with the client but feel it is important to discuss in our future sessions. 6. Theory: I am using solution focused brief therapy with this client. I would like the client to focus on what he would like for his future and what steps he needs to take to get there. I would

like to see the client tap into his emotions a little more, and I would like to explore more of his feelings around not being able to play football and how that is affecting him now. 7. Three areas I did well: I think I did a good job asking clarifying questions, active listening and beginning to come up with a plan for his future. I also like how I started the session with a positive not from his teachers. I think this lightened the mood and took his mind off of the camera. 8. Three things to improve: I talked A LOT during this session, which is something I need to work on. I would like to see myself give the client more space in the future to explore any other areas that he feels are necessary. I also need to work on not asking leading questions or putting words in the clients mouth. Example of questions/responses from teachers about Student #1: 1. What do you see as the biggest obstacle in Student #1s academic success? He needs to manage his time more effectively and be more exacting with his work, has to develop a work ethic that matches his work load 2. What is his level of engagement in class? He tends to be very engaged loves to participate (though at times he can be off task) 3. Does he come to tutorial to ask clarifying questions or make up missing work? no 4. Does the student appear to be distracted in class or during test taking time? no 5. What is a strength you see in this student? His enthusiasm 6. What other observations have you made that might be helpful in discussing next Steps, and making a plan, for this student? I think helping with short term, medium term, and long term goals would be helpful, and help him get organized if he is organized it will be easier for him to keep on top of assignments and help him utilize the resources that he has at his disposal. He needs a lot of help with writing. I am available in tutorial, but he can also have you help him revise his last essay (he will get his scored rubric on Monday, but he already has an electronic copy of the paper (he got 54% on it). 1- Student #1 had a difficult time catching up when he got behind due to absences at the beginning of the year 2- Student #1 is very engaged in class. Hes excited to contribute to class discussions, and celebrates when he gets the correct answer on the classwork. He needs redirection at times during class, but is pretty motivated 34- He is distracted during class. I think the primary reason for that is the fact that math is 6th period, and the class is 2/3 boys 5- Student #1 is great at participating in class, but has a difficult time advocating for himself when he needs help or when he gets behind
