Dear Dr Phommasack,
As discussed in our letter of 16 April 2004 and in follow up discussions with your
portfolio manager, the Board of the Global Fund at its March Board meeting enabled
Country Coordinating Mechanism (CCM) to request an amendment to their Grant
Agreement to reflect the date Grant funds were first received or to otherwise account for
unique program circumstances. This implementation letter serves as an amendment to
the Grant Agreement to put that date change into effect.
By signing this implementation letter the Global Fund and the Principal Recipient agree
to amend the following blocks of the face sheet as follows:
Block 4: 01 May 2003
Block 5: 30 April 2005
Block 6: 30 April 2008
The start date for the first quarterly period set in Annex A of the Grant Agreement shall
be deemed to have begun from the Program Starting Date, as amended by this
implementation letter, and the terms for all sUbsequent quarterly periods are amended
respectively.
The face sheet of the Grant Agreement, as amended, is attached.
Please confirm your agreement with the foregoing by signing the enclosed copies of this
letter and return one to us, retaining one copy for your records.
