PARENT CONSENT FORM/INJURY WAIVER
FOR THOSE UNDER 18 YEARS OF AGE WHO WANT TO PLAY PAINTBALL WITH or WITHOUT THEIR
PARENT BEING PRESENT
As a patron of Paintball Mayhem 6501 E 57

St Sioux Falls, SD 57108 desiring to participate in the sport
of PAINTBALL and acknowledging my understanding that the sport of PAINTBALL subjects the
participants to a substantial risk of personal injury and harm and could result in serious physical injury;
my participation in such activities and/or use of such equipment may result in participants injury or
illness including but not limited to bodily injury, strains, fractures, eye injury, blindness, heat stroke, or
other ailments that could cause serious injury, and these risks and dangers may be caused by
participants own behavior and/or negligence of others, accidents, breaches of contract, the forces of
mature or other causes. These risks and dangers may arise from foreseeable or unforeseeable causes;
and by the participants participation in these activities and/or use of equipment with full knowledge of
such danger, do hereby assume the risk for any injuries that our child may sustain in the activity of
PAINTBALL while on the premises of PAINTBALL MAYHEM and do hereby hold harmless and remise,
release and forever discharge PAINTBALL MAYHEM, its owners, lessors, agents and employees from any
actions, suits, damages, claims, or judgments that may result from any personal injury our child may
sustain while on the premises of PAINTBALL MAYHEM. While engaged in the activity of PAINTBALL
upon the premises of PAINTBALL MAYHEM.
As a parent of the child I/we assume full responsibility for the child participating in paintball activities. I
have read and understood this release. I have explained the effect of this to my child. I have read and
understood the rules of Paintball Mayhem. I have carefully explained all the rules and the conditions
which apply to my child participating in paintball activities. I have read all of the terms set out above and
accept that in consideration of my child being permitted to participate in Paintball Mayhem activities
that I assume full responsibility for my child as set out in the waiver release and indemnity above.
I HAVE READ, UNDERSTOOD, AND SIGNED THE PARENT CONSENT FORM AND DO UNDERSTAND THE
RISKS INVOLVED FOR MY CHILD.
I HEREBY GIVE CONSENT FOR TO PARTICIPATE IN PAINTBALL AT
PAINTBALL MAYHEM WITH OUT MYSELF OR ANOTHER ADULT PRESENT.
We allow our child to participate in paintball activities.
