I certify that my dog has been vaccinated against Canine Distemper and Hepatitis, Canine Parvovirus and also
complies with the Dog Registration and Hydatids Act.

I acknowledge that should my dog suffer from or be in contact with any contagious ailment, or should be in
season, that I will not bring it to classes until cleared.

I accept that the Hawkes Bay Dog Training Club Inc. has a NO refund policy.

I accept that my dog may not be allowed to continue the course should it be deemed by my Hawkes Bay Dog
Training Club Incs Instructor/s to pose a significant safety risk to other dogs, handlers and/or the
Instructor/s of the class.

I also accept that the Hawkes Bay Dog Training Club Inc. has an all care, no responsibility policy and I will not
hold the Hawkes Bay Dog Training Club Inc. and/or any of its Instructors responsible for any injury, either to
myself, my dog or to any other person or animal I choose to bring to the club grounds.

Race Course, Prospect Road. You will be rung with a day, time and other details. Please bring poo bags,
water, treats (cheese, cherios etc. not bickies) and a toy (non squeaky) for your dog. Also please bring
current vaccination certificate for inspection.

Handlers must be over 10 years old. If handler is under 15 years old please state age.

Fee for the 8 week course - $80.00 (non refundable).
