The 2014 Resolution Health and Zurreal benets are here!
All Resolution Health members have access to the basic
Zurreal programme at no additional charge.

The Millennium option combines the exibility of a medical
savings plan with an Above Threshold Benet (ATB) when
day-to-day expenses are particularly high. This option allows
for unused savings to be carried over annually to the next
year and includes comprehensive in-hospital and chronic
cover.

Option Ideal for Key Benets
The Classic option is traditional in design, and provides

premiums. This option encompasses exceptional benets for
the little ones, making it ideal for young families who place a

Contracted specialist fees paid up to 150% of Scheme rate
in and out of hospital.
Cover for 31 chronic conditions.
Extended maternity benets including 4 2D scans and
postnatal maternity visits.
2 Additional paediatrician visits per beneciary less than 2
years of age and 2 additional GP visits per beneciary
between the ages of 2 and 12 years.

Progressive Flex is a balanced option which is ideally suited to

All Foundation members are reminded to send Resolution Health a copy of their latest proof of income before
07 December 2013 to conrm their income bracket for contributions in the coming year. Should we not receive
proof of income from you before 07 December 2013, we will automatically select the 2014 income bracket
above that of your current contribution bracket.

the basic Zurreal programme at no additional charge with the
option to upgrade to the phenomenal intermediate and ultimate
lifestyle solutions of Zurreal Gold and Platinum.
2014 will bring with it a number of enhancements to the Zurreal Gold
and Platinum programmes which will oer you even more lifestyle
benets and cash rebates, making these sensational lifestyle and wellbeing

As a Zurreal Platinum member you can choose whether to utilise the fantastic enhanced 2014 gym rebate or the
unique education rebate benet and earn substantial cash back into your Zurreal Healthcard on an annual basis.
