STEPS TO FILL ONLINE ADMISSION APPLICATION FORM :

First of all candidate will have to go to the University website i.e.
www.bfuhs.ac.in. Read the prospectus carefully.
(a). Take print out of the challan form after filling the requisite information
such as Name, Fathers Name, email ID, Phone no. Course, category,
etc. carefully. A unique Candidate ID will be generated on the challan
form.
(b) Depositing Fee:- Candidate should have to deposit requisite fee in any
branch of the Oriental Bank of Commerce by using printout copy of Bank
Challan Form.
(c) After depositing requisite fee in the bank account of the University, a
unique Transaction ID will be issued by the bank, which is required to be
entered in the application form.
(c) Again go to the university website and fill online admission application
form including candidate ID and Bank Transaction ID.
(d) After completion of filling online admission application form, an unique
password and registration no. will be sent to the candidate through email /
SMS. Please keep the password secret which will secure your application
and mandatory for further process.
(e) Take printout of the online filled admission application form.
(f) Candidates are required to submit Printout of admission application
form alongwith required self attested copies of certificates and
university copy of bank challan form in the University by due date
and time.

General Instructions.
1. Merely deposition of fee will not entitle the candidate for claim of admission.
2. Fee once deposited in University Account will not be refunded/adjusted in any
case.
3. Candidates are advised to fill online admission application form before
the due date. BFUHS will not be responsible for Postal / Courier delays etc. or
loss of documents in transit. Applications received after the due date/time for
any reason will be rejected. No request for accepting the forms after cut off
date will be entertained. Fee once deposited shall not be refunded/adjusted in
any case.
4.Applicant are advised to download the prospectus from website or candidates
can get hard copy (printed booklet) of prospectus only at University counter
only on payment of Rs. 200/-.
5. Fee shall not be accepted in any other mode i.e. Cash, Cheque, Demand Draft
etc.

concerning admissions/Date of counselling from time to time.

(a) Only online depositing of fee in any OBC Branch through the Challan Form
available on university website will be accepted. Last date for depositing
online fee in the OBC Bank is 24.03.2014 upto 4.00 PM.
(b) Fee will not be accepted in any form after due date i.e. 24.03.2014 in any
case.
(c) Admission application Form is available on University website.
(d) Candidates will fill only online admission application form.
(e) Only those Admission Application Forms will be considered for counselling,
fee of which has been deposited online upto 24.03.2014 and thereafter
printout copy of admission application form alongwith University copy of Bank
Challan Form and self-attested copies of certificates/documents submitted in
the University upto 28.03.2014 (5.00 p.m.).
(f) Printout copy of Admission Application Form can also be submitted alongwith
late fee of Rs. 4000/- in the shape of DD favouring Registrar, BFUHS payable
at Faridkot upto 02.04.2014.
(g) No request for accepting the Admission Application Forms after 02.04.2014
will be entertained.
(h) Fee once deposited in University Account shall not be refunded/adjusted in
any case.
(i) Deposit of only online fee, will not make a candidate eligible. His/her form
alongwith self attested copies of certificates must be received in the
University by last date as mentioned above.
3. Merely filling up online admission application form will not entitle the candidate
for admission. The Print out copy of Admission Application form should also be
submitted in the university by the stipulated date and time with requisite fee in
the shape of challan form and self attested copies of certificates, failing which no
claim of the candidate will be considered.
4. Merely depositing fee will not entitle the candidate for admission. The print out

5. Fee should be deposited in the University account through any branch of the

available on the university website). Fee shall not be accepted in any other mode
other than online system.
6. Notwithstanding the participation of candidates in counselling, only eligible
candidates, as per Government notification/s, shall be considered for admission.
7. Candidates are advised to ensure deposition of print out of admission application
forms by due date & time in the university.
8. Candidates applying under any of the reserve category are directed to attach all
relevant documents/proof of respective category with admission application form
and also directed to bring along originals at the time of counseling.
9. Instructions only for NRI Candidates:- Candidates who will apply under NRI
quota, will have to obtain Eligibility certificate from the University and apply on
prescribed proforma for NRI quota seats available at University website
alongwith prescribed fee for NRI candidates.

1. INTRODUCTION

BRIEF HISTORY:
The University was established at Faridkot under an Act of Punjab State
Legislature (Punjab Act No. 18 of 1998) in the name of great Sufi Saint Sheikh
Farid Ganj-E-Shakar, (1173 1265 A.D.) in 1998 for purposes of affiliating,
teaching and ensuring proper and systematic instruction, training and research in
Modern Systems of Medicine and Indian Systems of Medicine. It is not only
affiliating and examining body but also a teaching and research centre in health
sciences. It is expected to play a vital role in improving the standards of medical
and health education in the state and in conducting relevant research in all
aspects of health sciences, basic as well as applied, which ultimately will
ameliorate the health standards of the people of Punjab. II is believed that the
university will be a trend setter in developing appropriate modes and models of
health care to ensure quality health care to the people.

The University has been recognized by MCI vide letter no. V/11015/3/2000-ME
(UG) dated 9

June, 2000. The UGC has also included the University in the list
of recognized universities maintained by the University Grants Commission,
under section 2(1) of the UGC Act. 1956 vide letter no. F.9-3/97 (CPP-I) dated 4
July. 2002. The Regional Director. World Health Organization, Regional Office
for South-East Asia, World Health House, New Delhi has also forwarded the
name of the University for inclusion in the World Directory of Medical Schools
vide letter No. M12/6212 dated 6 May 2002. The University is also included in
the list of Commonwealth Universities and their yearbook 2001 at page No. 692
(Vol. l).

Established with a mission to create an intellectual, academic and physical
environment conducive to free flow of ideas and exchange of information
between various faculties of the university, between this university and other
universities of health sciences in the country and abroad, thereby opening a
window to the world for the health professionals, health planners, health
managers. biomedical and social scientists and educationalists in health
sciences of the country. Efforts are on to establish contact with various
institutions towards achieving this goal.

UNIVERSITY'S PRESENT STATUS:
All the new buildings of the University and its constituent college, GGS Medical
College & Hospital were completed in the year 2011. The university was shifted
to its new building situated at Sadiq Road, Faridkot in July 2011. Hon'ble Chief
Minister, Punjab. inaugurated two new University buildings (Senate Block and
Academic Block) on 22-09-2011. Hon'ble Deputy Chief Minister inaugurated the
Radiotherapy Unit, and other buildings of GGS Medical College & Hospital, on
23-09-2011. The GGS Medical College & Hospital, a constituent college of the
University is also shifted to its new building.

The University Campus comprises of GGS Medical College along with
Auditorium of 800 capacity, Senate Block, Academic Block, University College of
Physiotherapy, University College of Nursing, Drug De-addiction Centre,
Telemedicine, Edusat and a well equipped state-of-the-art 500 bedded hospital.
Coveted projects to set up Trauma Centre, Regional Cancer Centre all in
process. The University has added to its academic convoy University Library &
Informatics Division, Department of Health Sciences Library & Information
System, University Centre of Excellence in Research, Rose Garden and Herbal
Garden.

UNIVERSITY'S NEW PROJECTS:
University is on the way to establish
1) Advanced Cancer Diagnostic, Treatment & Research Centre at Bathinda.
2) University Regional Centre for Public Health & Paramedical Sciences at
Goindwal Sahib.
University has acquired 20 Acres of land for its Regional Center at Goindwal
Sahib. Hon'ble Chief Minister, Punjab has already inaugurated both the projects
in October 2011. The University has planned to run a number of employment-
oriented diploma courses and certificate programmes in health sciences.

The University is also in process of developing the following five study centres:
1. Centre for Distance Learning (Medical) and Educational Technology
miscellaneous.
2. Centre for Health Systems and Health Services Management.
3. Centre for Bio-information Technology.
4. Centre for Human Resources Development and Health Management.
5. Centre for Population Sciences.

UNIVERSITY'S CONSTITUENT COLLEGES:
The Government of Punjab incorporated the three institutions as constituent
colleges of the Baba Farid University of Health Sciences, Faridkot viz. School of
Nursing (upgraded to University College of Nursing) vide notification no.
11/299/01-5SS3/4770 dated 14.10.2002,
Guru Gobind Singh Medical College & 500 bedded Hospital. Faridkot vide
notification no. 8/02/2005-1HB-I11/6308 dated 31.10.2006 and State Institute of
Nursing & Paramedical Sciences, Badal vide notification no. 1/46/07-1HBIV/1163
dated 28.11.2007. In addition to above, the Board of Management of University
also accepted the proposal for starting of Physiotherapy College in the University
campus in its meeting held on dated 03.04.2007 and fourth constituent college
was established viz. University College of Physiotherapy. University has also
established its own Institute of Pharmacy. Recently, Govt. of Punjab has
transferred 100 beded Civil Hospital, Badal to University for strengthening and
expanding the connection between rural medical services and the education and
research carried on by the Baba Farid University of Health Sciences. The GGS
Medical College & Hospital, a constituent college of the University has 25
departments and 7 post graduate departments and is recognized by MCI.

COLLEGES AFFILIATED WITH UNIVERSITY:
At present the University is having 143 affiliated colleges comprising 137
affiliated and 6 constituent Colleges (with an approximately 35,000 students).

University will soon endeavor to achieve excellence in health care delivery and
medical education and research in the State. Admissions to various
undergraduate and post graduate courses in the affiliated institutions of this
University in the State of Punjab are made on merit basis through the conduct of
Entrance Tests. The AIPGMEE & AIPGDEE - 2014 is being accordingly
conducted by the NBE/AIIMS to enroll suitable candidates to the various PG
Courses in the Medical/Dental Colleges of the State for the session 2014.

The university is now primarily focusing on strengthening postgraduate medical
education and research and evolving as a centre of excellence in a rural
backdrop.

I M P O R T A N T
PLEASE READ IN DETAIL
1. This prospectus contains two parts :

Part - A Pertains to the general procedure for applying to admission
application form, deposition of online fee, important instructions,
etc.
Part -B Contains Punjab Govt. Notifications for admission to post graduate
Medical and Dental Courses, proformas for different categories,
resident proformas, self undertaking, etc.

4. This prospectus is subject to alteration(s) / modification(s) at any time without
notice. For updates please visit University website from time to time.

5. Changes, if any, in the Schedule / Counselling etc. shall be notified only through
website of the University i.e. www.bfuhs.ac.in

6. Candidates must retain the prospectus with them till admissions. They should
read carefully the instructions and the rules and regulations of the Punjab Govt.
Notification/s and comply with all instructions therein strictly.

7. The allotment of specialty and college to the applicants for admission, shall be
made by the Admission Committee as per merit of the candidate determined on
the basis of AIPGMEE-2014 / AIPGDEE-2014 and the choice of the speciality
and station by the candidate at the time of the counselling. Personal appearance
of candidate in the counselling/interview shall be compulsory. Exemptions from
personal appearance should not be sought as it shall not be granted in any case.

8. For Admission to PG Course : Admissions including Eligibility to various PG
Courses shall be governed and regulated as per the rules and regulations etc. as
per Punjab Govt. notifications. After allocation of provisional Rank/Marks by NBE
/ AIIMS on the basis of AIPGMEE-2014 / AIPGDEE-2014, each applicant
seeking admission to PG Course is required to apply online on the ADMISSION
APPLICATION FORM available on university website and thereafter submit
print out copy of admission application form in the University by stipulated
date alongwith UNIVERSITY COPY OF BANK CHALLAN FORM and
requisite self attested copies of certificates as prescribed to substantiate
claim for admission.

9. Baba Farid University of Health Sciences, Faridkot has provided access to e-
resources [HSLIBNET] to the students of all the affiliated colleges of the
University for which Rs. 2,000/- per annum will be charged for HSLIBNET from
the student.

10. Undertaking (ANNEXURE-B) is compulsory for all the candidates applying
for admission, except 60% quota candidates.

11. Undertaking (ANNEXURE-C) is compulsory for all candidates applying for
seats under 60% quota.

12. The tentative list of the institutions / specialty wise distribution of seats will be
displayed on the website of the university. The admission for PG courses shall
be made only to those institutions and to those specialties for which the
permission from all of the concerned authorities including the affiliation of the
University are received by the concerned institute.

13. Final Category/Institution wise distribution of seats shall be displayed at the
time of counselling.
14. The fee structure for the Govt./Privately managed colleges is as per Punjab Govt.
notification(s) for Post Graduate Degree/Diploma courses. If lateron Punjab Govt.
issues any amendment/corrigendum for change in fee structure, that will be
applicable in letter and spirit.
15. Please visit our website www.bfuhs.ac.in from time to time for updated
information.
16. In case of any discrepancy or contradiction between the Government
notification/s and the University instructions contained in this Prospectus, the
letter and spirit of the notification/s shall be prevailing, however in academic
matters the University shall be the final Authority.
17. The candidate should read the eligibility condition carefully and just appearing in
counselling does not entitle him/her for admission. Eligibility conditions for
admissions are given in the Notification. The eligibility shall be determined by the
Selection Committee at the time of counseling. Those who are not eligible as per
Punjab Govt. notification will not be considered for admission. The candidates
will appear in counselling at their own risk & responsibility.
18. Any attempt by a candidate to influence directly or indirectly any person
concerned with the admission will disqualify him/her.
19. The College wise/Speciality wise/category wise tentative Distribution of seats will
be displayed on the university website in due course of time.
20. This is for the information of all candidates that as per Honble Supreme
Court of India, if any incident of ragging comes to the notice of the
authority, the concerned student shall be given liberty to explain and if his
explanation is not found satisfactory, the authority would expel him from
the institution.

2. GENERAL ELIGIBILITY CONDITIONS
2.1 The admission to PG courses (MD/MS/PG Diploma/MDS) shall be open to
candidates who have qualified AIPGMEE-2014 / AIPGDEE-2014 and eligible as
per Punjab Govt. notification (incorporated in this prospectus in Part B) and
possessing basic qualifications of MBBS for various Medical PG Courses and
BDS for various Dental PG Courses. The merit/eligibility will be determined as
per Punjab Govt. notification(s).
2.2 Whereas the AIPGMEE-2014 / AIPGDEE-2014 is open to all eligible candidates,
no claim to a seat/admission shall be made on the basis of the AIPGMEE-2014 /
AIPGDEE-2014 rank alone. Eligibility and Merit of a candidate for admission
against any PG seat will be determined by the Selection Committee, strictly as
per rules and regulations laid down in the Punjab Govt. Notification(s) as
published in this prospectus and all other such documents as are required with
the ADMISSION APPLICATION FORM.
2.3 Counseling / Admissions shall be conducted by the Admission / Selection
Committee constituted by the Government for this purpose.

3. INSTRUCTIONS FOR FILLING UP ONLINE ADMISSION APPLICATION FORM
3.1 Before filling online admission application form, candidates are required to
deposit requisite fee in the any Oriental Bank of Commerce Branch through the
challan form available on university website.
3.2 ADMISSION APPLICATION FORM is available on university website. A
candidate who qualifies in AIPGMEE-2014 / AIPGDEE-2014 and is eligible to
apply ONLINE for admission to Post Graduate Courses including Post Graduate
Diploma Courses and after taking a printout of online Admission Application
Form, candidates will submit it alongwith self attested photocopies of the
required documents and UNIVERSITY COPY OF BANK CHALLAN FORM in
the University. Incomplete ADMISSION APPLICATION FORM not
accompanied by requisite fee and necessary documents, shall be rejected
summarily.
3.3 Candidates will attach the downloaded copy of result card of AIPGMEE-2014 /
AIPGDEE-2014. The candidate is also required to attach self attested photocopy
of the Admit Card of AIPGMEE-2014 / AIPGDEE-2014 with ADMISSION
APPLICATION FORM.
3.4 Candidate shall fill only once online ADMISSION APPLICATION FORM.
3.5 Printout copy of admission application form alongwith self attested copies of
required certificates/documents and University Copy of Bank Challan Form as a
proof of deposition of requisite fee should be submitted in the University.
3.6 In case any candidate is found to have furnished false information or certificate,
etc. or is found to have withheld or concealed information in his/her application
form he/she shall be debarred from admission.
3.7 For claims on reserved seats/categories/quotas wherever applicable, the
candidates must support their claim by attaching self attested photocopies of
appropriate certificate(s)/documents issued by competent authority as per
specimens given in Annexures to this prospectus with the Admission Application
Form. Original certificates/documents shall have to be produced by the
candidate at the time of admission/counselling.
3.8 INCOMPLETE ADMISSION APPLICATION FORM OR WITHOUT REQUISITE
FEE WILL NOT BE ENTERTAINED AND WILL BE REJECTED WITHOUT ANY
INTIMATION TO THE CANDIDATE(S).
3.9 Fill category(ies) carefully in the online admission application form. Category
once claimed will not be changed in any case after submitting the form in the
university. Those candidates who will not fill/claim any category code as per
prospectus or wrongly filled category code other than mentioned in prospectus,
will be treated as General Category candidate and no claim will be entertained in
this regard. The candidate may select a category code from the codes given
below and fill the category accordingly. More than one category may be filled by
a candidate if it is considered applicable in the candidate's particular case.
Category codes are exclusively for the purpose of facilitating the
candidates to know and understand his/her respective merit placement at
the time of counselling in various reserved categories as might have been
filled by him/her. Reserve category selection on the part of the candidate
shall not in any case be construed to give him/her any legitimacy or right or
claim on the benefits for reserved seats during admission. Claims to
admissions against reserved seats in MD/MS/PG Diploma and MDS courses will
be on the basis of appropriate claims made in the Admission Application Form
subject to rules and regulations contained in the Punjab Government
Notifications printed in the Part-B of this prospectus and as per the decisions of
the Selection Committee constituted for this purpose by the Punjab
Government. No benefit of reservation whatsoever shall be given if the claim is
not made in Admission Application Form.

4. INSTRUCTIONS FOR COUNSELLING.

4.1 Personal appearance of candidates is essential at the time of
interview/counseling. Exemptions from personal appearance may not be sought
as it shall not be granted.
4.2 The candidate should read the eligibility condition carefully and just appearing in
counselling does not entitle him/her for admission. Eligibility conditions for
admissions are as per Punjab Govt. Notification(s). The eligibility shall be
determined by the Selection Committee at the time of counseling. Those who are
not eligible as per Punjab Govt. notification will not be considered for admission.
The candidates will appear in counselling at their own risk & responsibility.
4.3 The allotment of specialty and college shall be made as per the choice of
speciality and station by the candidate at the time of the counselling by the
Admission Committee on the basis of the merit determined as per marks
obtained by the candidate in AIPGMEE-2014 / AIPGDEE-2014 and other terms
and conditions contained in the Punjab Government Notification for admission to
Post Graduate Courses.
4.4 The candidates shall have to produce all the certificates/degrees/diplomas/
documents in original at the time of admission/counseling.
4.5 The choice(s) of courses/ institutions shall be exercised by the candidate at the
time of Counseling/ interview. No waiting list shall be prepared.
4.6 The seats left vacant from All India Quota and surrendered by the D.G.H.S., New
Delhi, on or before 2nd and final counselling, shall be filled strictly as per terms
and conditions contained in the Punjab Govt. Notification.
4.7 All the candidates selected under 40% quota for various PG courses will have to
submit their bond (complete in all respect) as per Punjab Govt. notification at the
time of reporting with the institution concerned for submission of fee at the time
of joining, failing which their fee/ joining will not be accepted. The specimen
copy of bond to be submitted is attached as Annexure N. The candidates will
attach the attested copy of proofs of the ownership of property which is being
pledged by the sureties. The bond should be attested by the Executive
Magistrate. The incomplete bond will not be accepted at all.

2. The Governor of Punjab is pleased to notify admissions to Post Graduate
Degree/Diploma courses in the Health Sciences Educational Institutions

3. The Governor of Punjab is further pleased to authorize Baba Farid
University of Health Sciences, Faridkot to conduct centralized counselling
for admissions on the basis of merit obtained in All India Post Graduate
Medical Entrance Examination (AIPGMEE) - 2014 as per procedure laid
down by MCI and notified under The Punjab Private Health Sciences
Educational Institutions (Regulation of Admission, Fixation of Fee and
Making of Reservation) Act 2006.
4. The admissions to all the PG courses including Diploma courses but
excluding Super speciality courses in the State shall be based on
AIPGMEE-2014 merit.
5. AIPGMEE-2014 is Eligibility-cum-ranking examination which is a single
entrance examination to various MD/MS & PG Diploma courses with the
approval of the Ministry of Health & Family Welfare, Govt. of India.
AIPGMEE-2014 is to be conducted by the National Board of Examinations
for Medical Courses.
6. In order to be eligible for admission to a Post Graduate course it shall be
necessary for a candidate to obtain minimum of 50% marks in AIPGMEE-
2014. However, in respect of candidates belonging to scheduled castes,
scheduled tribes, other backward classes, the minimum marks shall be at
40% or as notified by MCI. In respect of candidates with locomotor
disability of lower limbs, the minimum marks shall be at 45%.
Provided when sufficient number of candidates in the respective
categories fail to secure minimum marks as prescribed in AIPGMEE,
Government in consultation with MCI, may at its discretion, lower the
minimum marks required for candidates belonging to respective
categories.
7. One year compulsory rotatory internship is an essential prequalification for
acquiring eligibility for admission. The cut off date for completion of
Internship shall be 31
st
March 2014.
8. The admission to NRI seats shall also be on the basis of merit obtained in
AIPGMEE-2014.

9. Admission to minority institutions shall be made as per provision of the
Punjab Private Health Sciences Educational Institutions (Regulation of
Admission, Fixation of Fee & Making of Reservations) Act, 2006.
10. All the seats in Govt. as well as Private Institutions will be filled by
Centralized counselling.
11. The schedule of counselling will be decided by Baba Farid University of
Health Sciences, Faridkot. The details will be available in the prospectus
to be issued by BFUHS.
12. The tentative distribution of seats will be displayed by the Director
Research & Medical Education on the official site i.e
www.punjabmedicaleducation.org. The final distribution of seats will be
displayed prior to date of counselling on the official website of the
department.
13. A candidate who is already admitted/completed any Post-Graduate
degree course shall not be eligible for admission.
14. Candidates shall give a declaration that he/she has not abandoned the
Post Graduate seat after opting for admission or after being admitted in
Post Graduate Course in previous year.
15. Candidates will have to give an undertaking that he/she has not been
admitted to any Post Graduate Course for the corresponding session and
if admitted he/she has already left the same before counselling.
16. Even if the Candidate is having P.G. diploma, he/she is allowed the choice
of subject in postgraduate course as per merit, irrespective of the subject
of diploma.
17. The allotment in the specialty and college shall be made by the Admission
Committee on the basis of the merit obtained by the candidate on the
basis of AIPGMEE. Choice of the speciality and station by the candidate
at the time of the counselling shall be final. Personal appearance in the
counselling / interview of the candidate shall be mandatory. No waiting list
shall be prepared.
18. In case a candidate once admitted, drops out or leaves the Post
Graduation Degree course in between, he/she shall be debarred for a
period of three years for admission to any PG course in the State of
Punjab. The University shall display the list of the names and addresses of
such debarred persons.
19. In case there is a vacancy after the first counselling, such seats shall be
filled through the same process in the second counselling.
20. The schedule of admission process i.e. dates of counselling and
commencement of classes, cut off date, etc. shall be as declared by the
Medical Council of India and any court orders by the Hon'ble Supreme
Court or Hon'ble High Court as the case may be.
21. Baba Farid University of Health Sciences may fix and charge fee for
Prospectus, Application Fee, etc.

22. The application for Admissions, complete in all respects shall be submitted
to the Baba Farid University of Health Sciences, Faridkot, Punjab.
23. The merit list for general and reserve candidates shall be combined.
Reserve candidates will be eligible for open seats i.e. in general as per
their merit, eligibility and option as the case may be except for SC & BC
candidates having 40 percent or more but less than 50 percent marks.
24. In case of any dispute in distribution of seats for all the institutions situated
in the State of Punjab, Director Research & Medical Education, Punjab will
be the appellate authority.

26. Seat distribution in Govt. Institutions (Govt. Medical College, Amritsar
& Patiala, GGS Medical College, Faridkot.)
I. In the Govt. Institutions, 50% of the total seats shall be filled by the
Government of India at all India level through AIPGMEE -2014. The
remaining seats shall be filled through AIPGMEE-2014 at the state
level from amongst the candidates having Punjab Resident status.
Out of State quota seats 60% seats shall be filled up from amongst
the eligible PCMS in service doctors & 40% seats shall be open to
all eligible medical graduates. There shall be separate merit list for
in-service 60% quota candidates for Medical Colleges and a
separate merit list for those who are not covered under the
category of in-service candidates.
II. For 60% quota candidates (PCMS in service doctors).
a) The Eligibility requirements are as under:

i. Regular PCMS employee
ii. Has completed 4 years service in very difficult (Category-
D) area or 6 years service in difficult (Category C) area or
an appropriate combination of both. In case of candidates
who have completed 5 years of service as on 01-01-
2012, they should have completed 2 years of service in
most difficult areas or 3 years of service in difficult areas.

iii. RMOs once they are selected in PCMS, will be given
benefits of rural service rendered by them as RMOs
under Zila Parishad.
iv. Has cleared the probation period
v. Has Good service record
vi. Has no vigilance/departmental/disciplinary inquiry
pending against him/her.
vii. Will have 10 years of service left after completion of the
course
b) The period of rural service shall be computed as on March
31
st
2014.
c) Adhoc service rendered in respective category will be
counted for the purpose of computing the stipulated period.
d) Weightage of 1 mark for each year of service in difficult area
& 1.5 marks for most difficult area over & above the eligibility
of rural service shall be given, subject to a maximum of 5
marks.
e) PCMS in service candidates will submit along with the
application a certificate regarding length of service, length of
rural service, number of years of service left after completion
of PG course & that no department/vigilance inquiry is
pending against the candidate.
f) All PCMS doctors who are selected for admission to post
graduate courses shall have to produce a No Objection
Certificate from the Govt. of Punjab, Department of Health &
Family Welfare.
g) All in service doctors shall have to submit a bond of Rs 40
lakhs to serve the Punjab Government for a period of 10
years after completion of post graduate course. If the
candidate fails to do so he/she shall have to deposit the
bond money with the government.


III. For 40% quota candidates
40% quota seats shall be open to all eligible medical graduates.
Any candidate in state government employment shall produce a No
Objection Certificate from his/her employer.

IV. For 40% & All India quota candidates
Candidates selected in All India quota will be considered at par with
40% state quota candidates. They will get fixed
emoluments/stipends as determined by the Government from time
to time for the course period of 3 years subject to the following
conditions:

i. The candidate is to submit a bond of Rs15 lakhs to
serve the Government of Punjab for a period of three
years after completion of PG. This clause will not be
applicable in case the offer is not given by the
Government of Punjab within a period of one year of
passing of the postgraduate examination.
ii. The candidate will inform the Government of Punjab
that he has passed the postgraduate examination.
iii. Failure to serve the Government of Punjab for a period
of three years will lead to deposition/ recovery of bond
money to the Government of Punjab i.e. Rs 15.00 lakhs.

V Reservation in Government Colleges
i. Scheduled caste 25%
ii. Backward class 5%
iii. Physically handicapped 3%

Only orthopedically handicap is entitled to reservation in
physically handicapped category & shall have locomotor
disability of lower limbs between 50%-70%. With this
disability but otherwise found medically fit to pursue the
course in the speciality concerned by the Medical Board duly
constituted by the government & consisting of Heads of
Departments of Orthopaedics of 3 Medical Colleges. In case
the candidates are not available then the candidates with
disability of 40% to 50% may be considered.
iv. Sports Persons 2%
Credit will be given for the sports achievement made during
the MBBS academic course only & shall be graded by the
Director Sports, Punjab with a minimum benchmark of B
grading
v. Wards of terrorism/riot affected persons(in order of
preference to the exclusion of the next category) 2%
i. Persons killed in terrorist activities in Punjab or in
1984 riots outside Punjab
ii. Terrorism or riot affected or displaced person of
Punjab origin.
Preference will be given to a candidate whose parent
or guardian is killed in such situation (Guardian to be
considered only in case neither parent was alive at
the relevant time)
vi. Wards of Defence personnel ( in order of preference to the
exclusion of the next category) 2%
a. Killed in action
b. Disabled in action to the extent of 50% or
above & boarded out of service

c. Died while in service & Death attributed to
Military service
d. Disabled in service & boarded out with
disability attributed to Military service
e. Gallantry Award winners both serving /retired
f. Serving/ retired Defence personnel /ex-

Punjab Home guards & Para military forces

A certificate issued by the Inspector General of Police (HQ)
Punjab shall have to be produced. In case of Paramilitary
forces, this certificate shall be counter signed by the IG
Police (HQ) Punjab.

viii. Children/Grandchildren of Freedom fighters of Punjab 1%
Note 1 The availability of seats for reservation shall be as per hundred
point roster being maintained category wise/ subject wise/
institution wise.
Note 2 All the certificates shall be as per latest instructions issued
by the Government of Punjab.
VI. Fee structure for the Government Institutions for the year
2014 will be as notified by the government.
26. I. Distribution of seats in Private Institutions would be :
Govt. Quota Seats - 50%
Management/Minority Quota Seats - 50% (including
15% NRI Quota)

Candidates seeking admission in Private Institutions should
be residents of Punjab. However, in case adequate number of candidates having
Punjab resident status arent available even after lowering of minimum marks as
above, only then the seats will be offered to other candidates.

II. Reservation in Private Colleges:
Reservations in Private Unaided Health Sciences Educational
Institutions shall be as notified by the Punjab Govt..
The reservation will be as follows:
(i) Schedule Caste 25%
(ii) Backward Classes 5%
(iii) Physically handicapped as per MCI guidelines 3%

(iv) Two seats will be reserved in DMC Ludhiana for candidates
working in rural training centre under Dayanand Medical
College.
(Note: The Scheduled Caste & Backward Class Certificate must be
as per the latest instructions issued by the Government of
Punjab)

III NRI Seats

In private Colleges NRI Seats shall be 15% of the total seats. The
seats will be filled on the basis of merit as per AIPGMEE 2014, in
order of preference as under:-
Category I: NRIs who originally belonged to the State of Punjab.
Category II: NRIs who originally belonged to an Indian state other
than Punjab.
(a) If sufficient number of candidates under first category is
eligible and available then they will be admitted first even
if students under subsequent category are higher in merit.
(b) The left over seats in the NRI quota in private institutions
will go to management quota and shall be filled on merit
based upon the AIPGMEE 2014
(c) Candidates applying under NRI category must get
eligibility certificate from BFUHS, Faridkot.
(d) The NRI students will have to give a bank guarantee /
surety bond for the balance of fee, if they opt to pay the
fee in instalments on annual basis proportionately.
(e) Any misrepresentation, false information, fake
documentation and impersonation for securing admission
shall be punishable under the relevant provisions of Indian
Penal Code along-with cancellation of admission.


IV Fee Structure in Private Colleges:

(a) The fee for the Private Institutes including Medical
Institutes at Adesh University and Desh Bhagat or any such
University coming up in future will be as notified by the
Govt. under the The Punjab Private Health Sciences
Educational Institutions (Regulation of Admission, Fixation
of Fee and Making of Reservation) Act 2006.
Note : The fee will be collected on the spot by the Baba Farid
University of Health Sciences, Faridkot on behalf of the
respective institutions at the time of counselling and the
same will be transferred to the respective institutions after
2nd counselling.
(b) Hostel fee and security advance (Annual and provisional)
for private institutes will be as notified by the government.

Note 1 The fee will be payable on annual basis and the institutes
shall not insist upon the candidate to deposit fee of the
entire course in advance. At the most, candidates can be
asked to furnish a bond/bank guarantee that they will be
liable to pay the balance fee for the whole course even if
they leave the institute/course midway course.
Note 2 The fee shall be refunded / adjusted if candidate is shifted
from one college to another in the subsequent
counselling.
Note 3 The availability of seats for reservation shall be as per
hundred point roster being maintained category wise/
subject wise/ institution wise.

27. Conversion of seats under All India/60%/ 40% Quota Unfilled
seats:
All India Quota - The vacant seats transferred from All India quota,
will be transferred to General Category candidates of 40% state
quota.
After exhausting all the eligible reserve category candidates in 60%
quota, the vacant seats will be transferred to General Category
candidates of 60% quota. In case of non-availability of candidates
in 60% quota the vacant seat (s) will be offered to the eligible
general category candidates in 40% quota.
After exhausting all reserve category candidates in 40% quota
vacant seats will be transferred to General Category candidates of
40% quota.
28. Procedure for all seats remaining vacant after 2
nd
counselling
including NRI seats.
a. The candidate can apply or fill the form at the university
BFUHS or institution level. The fee for the forms shall be
same as that of the University.
b. Applications received at institution level or at the University
level will be considered for counselling together.
c. The counselling will be held by the respective institutions in
the presence of Punjab Govt. & University representatives
after adopting due procedure and vide publicity. The date of
counselling will be fixed in consultation with Baba Farid
University of Health Sciences, Faridkot.
d. The admission will be made as per AIPGMEE-2014 merit in
transparent manner.

29. For any dispute arising out of this notification or of the admissions
under this notification, the jurisdiction shall be Punjab and Chandigarh
only.
30. This notification supersedes all the prior notifications for admission to
the medical postgraduate degree/ diploma courses in the State of
Punjab.

1. In super session of all prior notifications for admission to the dental
postgraduate degree courses in the State of Punjab the Governor of
Punjab is pleased to notify admissions to Post Graduate Degree courses
in the Health Sciences Educational Institutions (Dental) in the State of
Punjab for the year 2014.
2. The Governor of Punjab is further pleased to authorize Baba Farid
University of Health Sciences, Faridkot to conduct centralized counselling
for admissions on the basis of merit obtained in All India Post Graduate
Dental Exam (AIPGDEE) - 2014 as per procedure laid down by DCI and
notified under The Punjab Private Health Sciences Educational
Institutions (Regulation of Admission, Fixation of Fee and Making of
Reservation) Act 2006.
3. The admissions to all the PG courses in the State shall be based on
AIPGDEE-2014 merit .
4. AIPGDEE-2014 is Eligibility-cum-ranking examination which is a single

8. No condonation of Internship of 12 months period shall be accepted
except (i) the students of the batch admitted during the academic
session 2007-2008 and have completed 4 year BDS Course
alongwith 06 months compulsory rotating internship

9. However, a candidate who, after passing the final qualifying examination
(BDS or the foreign dental degree recognized by GOI/DCI), is undergoing
12-month compulsory rotating internship/practical training and is likely to
complete the same by 31st March 2014 can take up this examination, but
he/she shall not be eligible for admission unless he/she has completed
the compulsory rotating internship and obtained provisional or permanent
registration on or before 31st March 2014 or the candidate produces a
certificate from the head of Institution that he/she will be completing
internship by 31st March 2014.

10. The admission to NRI seats shall also be on the basis of merit obtained in
AIPGDEE-2014.

Credit will be given for the sports achievement made during

A certificate issued by the Inspector General of Police
(HQ) Punjab shall have to be produced. In case of
Paramilitary forces, this certificate shall be counter signed
by the IG Police (HQ) Punjab.

viii. Children/Grandchildren of Freedom fighters of Punjab 1%
Note 1 The availability of seats for reservation shall be as per hundred
point roster being maintained categorywise/ subjectwise/
institutionwise.
Note 2 All the certificates shall be as per latest instructions issued
by the Government of Punjab.
VI. Fee structure for the Government Institutions for the year
2014 will be as notified by the government.

28. I. Distribution of seats in Private Institutions would be :
Govt. Quota Seats - 50%
Management/Minority Quota Seats - 50% (including 15%
NRI Quota)
Candidates seeking admission in Private Institutions should be
residents of Punjab. However, in case adequate number of
candidates having Punjab resident status arent available even after
lowering of minimum marks as above, only then the seats will be
offered to other candidates.

II. Reservation in Private Colleges:
Reservations in Private Unaided Health Sciences Educational
Institutions shall be as notified by the Punjab Govt..
The reservation will be as follows:
(i) Schedule Caste 25%
(ii) Backward Classes 5%
(iii) Physically handicapped as per DCI guidelines 3%

(Note: The Scheduled Caste & Backward Class Certificate must
be as per the latest instructions issued by the Government of
Punjab)

III NRI Seats
In private Colleges NRI Seats shall be 15% of the total seats. The
seats will be filled on the basis of merit as per AIPGDEE- 2014, in
order of preference as under:-
Category I: NRIs who originally belonged to the State of Punjab.
Category II: NRIs who originally belonged to an Indian state other
than Punjab.
a) If sufficient number of candidates under first category is
eligible and available then they will be admitted first even if
students under subsequent category are higher in merit.
b) The left over seats in the NRI quota in private institutions will go
to management quota and shall be filled on merit based upon
the AIPGDEE 2014
c) Candidates applying under NRI category must get eligibility
certificate from BFUHS, Faridkot.
d) The NRI students will have to give a bank guarantee / surety
bond for the balance of fee, if they opt to pay the fee in
instalments on annual basis proportionately.
e) Any misrepresentation, false information, fake documentation
and impersonation for securing admission shall be punishable
under the relevant provisions of Indian Penal Code along-with
cancellation of admission.


IV Fee Structure in Private Colleges:
(a) The fee for the Private Institutes including Dental
Institutes at Adesh University or any such University
coming up in future will be as notified by the Govt. under
the Act 2006.
Note : The fee will be collected on the spot by the Baba Farid
University of Health Sciences, Faridkot on behalf of the respective
institutions at the time of counselling and the same will be
transferred to the respective institutions after 2nd counselling.
(b) Hostel fee and security advance (Annual and provisional) for
private institutes will be as notified by the government.

Note 1 The fee will be payable on annual basis and the institutes
shall not insist upon the candidate to deposit fee of the entire
course in advance. At the most, candidates can be asked to furnish
a bond/bank guarantee that they will be liable to pay the balance
fee for the whole course even if they leave the institute/course
midway course.
Note 2 The fee shall be refunded / adjusted if candidate is shifted
from one college to another in the subsequent counselling.
Note 3 The availability of seats for reservation shall be as per
hundred point roster being maintained category wise/ subject wise/
institution wise.
29. Conversion of seats under All India/60%/ 40% Quota Unfilled
seats:

All India Quota - The vacant seats transferred from All India quota,
will be transferred to General Category candidates of 40% state
quota.
After exhausting all the eligible reserve category candidates in 60%
quota, the vacant seats will be transferred to General Category
candidates of 60% quota. In case of non-availability of candidates
in 60% quota the vacant seat (s) will be offered to the eligible
general category candidates in 40% quota.
After exhausting all reserve category candidates in 40% quota will
be transferred to General Category candidates of 40% quota.
30. Procedure for all seats remaining vacant after 2
nd
counselling
including NRI seats.
a. The candidate can apply or fill the form at the university
BFUHS or institution level. The fee for the forms shall be
same as that of the University.
b. Applications received at institution level or at the University
level will be considered for counselling together.
c. The counselling will be held by the respective institutions in
the presence of Punjab Govt. & University representatives
after adopting due procedure and vide publicity. The date of
counselling will be fixed in consultation with Baba Farid
University of Health Sciences, Faridkot.
d. The admission will be made as per AIPGDEE-2014 merit in
transparent manner.
31. For any dispute arising out of this notification or of the admissions
under this notification, the jurisdiction shall be Punjab and Chandigarh
only.

Letter No.1/3/95-3PP II/2043, dated 29/1/1996 vide which the instructions were

judgement delivered by the Hon'ble Supreme Court of India in the case of Dr.
Pradeep Jain Vs. Union of India and others reported as AIR 1984-SC-4121
wherein it was held that instead of the word 'Domicile' the word 'Residence' be
used in the instructions issued by the State Government on the subject.
Accordingly, it has been decided by the Government to revise the Government
instructions referred to above as under:

From: Chief Secretary to Government of Punjab to all the Heads of the department etc.
Subject: Issuance of certificate for the purpose of seeking admission to various
educational/technical/professional institutions in the state and also for employment.
Sir,
It has been brought to the notice of Government that educational authorities and institutions
prescribe various certificates regarding residence, SC/BC category and backward area etc. to be furnished by
the students/candidates with the applications forms for entrance examinations or employment in the formats and
by the authorities different from those prescribed by the Government departments. Cases have also been
reported where the authorities concerned insist on retaining original certificates and refuse to accept the attested
photo copies of the certificates. All this results in harassment and panic among the students/candidates and
their parents/guardian, since they have to apply with such certificates to more than one authorities
simultaneously and within a limited period. The lists of certificates required at the time of applying for
CET/PPMET/PMT and PAU are enclosed by way of illustration.
2. In order to overcome the difficulty mentioned above, it has been decided by the Government that
following procedure has been decided by the Government that the following procedure shall be followed
meticulously by all the educational and other authorities concerned:-
i) Certificates issued in the format and by the authorities prescribed by the Government Departments shall
be accepted as valid by all educational institutions and other authorities.
ii) Where a number of authorities have been authorized to issue the certificates there shall be no
insistence on issue of a certificate by any particular authority or by the highest authority indicated in the
format.
iii) (a) In case of entrance examinations, no certificate should be called for in the first instance alongwith the
application form. It will be sufficient for the candidate seeking admission or other facility to indicate in
the application from whether he/she belongs to any particular category entitling him/her to certain
concessions or facility. Requisite certificates may be obtained only from the selected/wait listed
candidates.
iii) (b) To further simplify the procedure attested copies of the certificates only shall be retained and original
certificates if required to be produced at the time of interview shall be returned immediately, thereafter.
iv) Parents/guardian or candidates can also obtain SC certificates from the Head of the institution where
the candidates has studied if the proof is available in the school records.
v) The persons who are employed in Government of India or any other State Government are to be
treated at par with the employees of the Government of Punjab in the matter of issue of Scheduled
Caste certificate provided relevant Scheduled Caste certificate exists in their service record. The
Scheduled Caste certificate in such will be issued by the respective Heads of Departments in the
Proforma below:-

is yet to be hold or deadline for submission of forms is not yet over.

Notwithstanding that anything that may be contained in the offer of appointment
under the government the Principal shall on his appointment be deemed to be in terms of
this bond under the government and he would not resign from the service of the government

without the previous sanction in writing of the competent authority, before the completion of
the minimum period of service as stipulated under this bond unless he/she is discharged
from such service by the government for any reason and no such resignation accepted or
purported to have been accepted by any authority shall for the purpose of this bond only,
become effective unless the pervious permission in writing of the competent authority has
been obtained by the Principal.

The decision of the competent authority as to the commission of breach or as such
indiscipline, misconduct on the part of the Principal and as to whether the Principal has or
has not performed and observed the conditions and obligations under these presents shall
be final and binding on the principal and the sureties.

And upon making such payment the above written bond shall be void and of no
effect but otherwise it shall be and remain in full force and virtue.

It is further agreed that in the event of the Principal being unsuccessful in any of the
qualifying examination conducted in the said institute/college/hospital, the government may
at their discretion, with hold the payment of scholarship for the prosecution of further studies
and the decision of the competent authority in this behalf shall be final and binding.

It is agreed that the liability of the sureties hereunder shall not be impaired or
discharged by reason of time being granted or any fore-bearance, act or omission of the
government (whether with or without the knowledge or consent of the sureties) in respect or
in relation to the several obligations and conditions to be performed or discharged by a
Principal or by any other matter or thing what so ever, the law relating to sureties shall but
for this provision have the effect or releasing the sureties from liability nor shall it be
necessary for the government to sue the Principal before suing either of the sureties for
amounts due hereunder .

Instructions for filling/submitting Print out copy of Online Admission Application Form


1. Before submitting the properly filled and signed print out copy of ADMISSION
APPLICATION FORM please ensure that one attested photo is pasted on the
prescribed space provided on the 2
nd
page of form.
2. Candidates must ensure that University copy of Bank Challan Form is
attached with admission application form after depositing prescribed fee on
Bank Challan Form in the OBC account (mentioned on challan form).
3. Attach attested copies of certificates, undertaking, category certificate (if any),
resident certificate etc. with the admission application form.
4. Candidates applying under 60% quota must attach required certificates
issued by the proper authorities.
5. Fill category(ies) carefully in the online admission application form. Category
once claimed will not be changed in any case after submitting the form in the
university. Those candidates who will not fill/claim any category code as per
prospectus or wrongly filled category code other than mentioned in
prospectus, will be treated as General Category candidate and no claim will
be entertained in this regard.
