This document is Exeter City Councils Internal Health & Safety Report for the financial year 2011- 2012.
The Councils management of health & safety at work during this period is examined via the components of successful health & safety management as determined by the Health & Safety Executive (HSE) 1 .
KEY FACTS

KEY TASKS
The Corporate Health & Safety Action Plan, included at Appendix I, identifies key tasks for the Council during this period and shows progress made as appropriate.
Further key tasks of a proactive nature were identified throughout this period. These are shown on the Health & Safety Monitoring Matrix at Appendix II.
Tasks of a reactive nature that have arisen during the period covered by this report are also shown on the Health & Safety Monitoring Matrix at Appendix II.
Key tasks are discussed in detail in Section 3, Measuring Performance.
Key Tasks for the forthcoming year, 2012 / 2013, are summarised as follows:
maintain commitment to the Corporate Health & Safety Action Plan; update the Councils Health & Safety Policy to reflect the new structure; maintain commitment to the Exeter City Council Health & Safety Management System; reduce the incidence of accidents & injuries through ongoing risk assessment & safety management commitment.

1.1 STATEMENT OF GENERAL POLICY
1.1.1 The Councils Statement of General Policy and the organisation & arrangements necessary to carry it out are available for viewing on the Councils websites external internet page http://www.exeter.gov.uk/index.aspx?articleid=4833 and intranet link on http://intranet/index.aspx?articleid=2629.
1.1.2 The Statement of General Policy, plus the organisation and arrangements to carry out the policy were reviewed in July 2011. A further review of the policy itself is required to reflect the new organisation structure. The policy statement however remains valid. The Principal Health & Safety Officer (PHSO) has submitted a draft revised policy to Assitant Director Environment .
1.2 PROCEDURES
1.2.1 Procedures are identified through various Safe Working Practices that are in place throughout the Council. Safe Working Practices (SWP) are arrived at through risk assessment.
1.2.2 Within Exeter City Council SWPs have facilitated specific risk management across a diverse range of services. This arrangement has proved more flexible for day to day safety risk management than blanket procedures to cover everything. This approach to health & safety management of medium and high risks will continue.
SECTION 2 - PLANNING
2.1 PLANNING FOR ACTIVE MONITORING
2.1.1 Team Health & Safety reviews take place as part of the active monitoring process and are carried out by the PHSO.
2.1.2 Health & Safety audits are carried out by Internal Audit and planned into their work schedule accordingly.
2.2 PLANNING FOR EMPLOYEE HEALTH
2.2.1 A programme of low key health surveillance for the Materials Reclamation Facility (MRF) employees has commenced. A schedule of health monitoring activities has been implemented: lung function (lower level); general health questionnaire; hand skin health inspection. This is led by the Environment Directorate.
2.2.2 The Council has employed the services of a Radiation Protection Adviser (Radon) with regard to monitoring and mitigation of radon gas in the Underground Passages. Advice from the Adviser has led to increased training and information sessions for staff, increased monitoring of mitigation trials, nomination and training of two employees - one as a Radon Protection Supervisor and the other as Deputy Radon Protection Supervisor. This is led by the Economy Directorate.
2.2.3 Parks and Open Spaces have now fully implemented monitoring of exposure employees have to vibration. Hand arm vibration (HAV) data on tools has been incorporated into a database controlled by the Fleet Office and Parks and Open Spaces. Operatives are issued with machine time log books and machinery is risk rated via a tag system. Detailed training sessions have taken place and health records are retained by the unit. A recommendation was made, via the Key Partner Group for Health and Safety, to extend Exeter City Council Internal Health and Safety Report 2011 - 2012
S:\CE\DSS\EHS\Internal H&S Reviews\Internal Health and Safety Report 2011 -2012 this level of monitoring to other areas of the Council where risk assessment identifies relevant control measures. This was adopted by a number of teams including Bereavement Services, Building and Electrical Services and River and Canal. This is jointly led by the Public Realm Directorate (Parks) and the Environment Directorate (Fleet).
2.2.4 In the coming year a recommendation has been made to assess Council employed tradesmen in relation to health surveillance under the revised regulations for the control of asbestos: The Control of Asbestos Regulations 2012. This is led by action 8d of the Corporate Health and Safety Action Plan.
SECTION 3 - MEASURING PERFORMANCE
3.1 CORPORATE H&S ACTION PLAN (KEY TASKS)
3.1.1 The Corporate H&S Action Plan at Appendix I contains four updates that plot progress throughout the year: August 2011, November 2011, December 2011 and March 2012.
3.1.2. Actions completed during the year include base line audits across all Council teams, training of current managers and supervisors in stress risk assessment and managing change, a new electronic induction training package for all employees, WISH Forum (HSE) sickness absence data submission, full establishment and support of the work of the First Aid Group and the health surveillance items mentioned in 2.2.
3.1.3. Actions ongoing into next year include management of asbestos in council houses, control of contractor policy and procedures, reporting on proactive (and statutory) maintenance of safety critical equipment and systems, review of refuse collection vehicle working practices on the rounds, ongoing completion of C actions asbestos removals, formulation of a corporate risk based approach policy on workplace inspection regimes.
3.1.4. Actions not completed during this year include replacement of the COSHH assessment system on Cubs/Maple and promotion of accident / incident investigation outcomes at SMT. With regard to the former, the action will be revisited during the forthcoming year. With regard to the latter, SMT will advise further in due course.
3.2 CORPORATE H&S PERFORMANCE MONITORING MATRIX
3.2.1 The Corporate H&S Matrix is sent to the Strategic Directors, Assistant Directors, Corporate Managers and Unison every quarter.
3.2.2 Entries onto the matrix are primarily arrived at through the health and safety audit and review process; this is termed active monitoring. Other types of active monitoring reach the matrix via routes such as Key Partner Group for Health and Safety, Joint Consultative Group or Employee Liaison Forum. Similarly, an Assistant Director or a Corporate / Team Manager may identify (usually through risk assessment) a safety control measure that requires action to eliminate or reduce risk. If the PHSO concurs with the risk control measure then this too may appear on the matrix under active monitoring.
3.2.3 Conversely, some entries arrive onto the matrix through accidents, incidents and near misses. These are termed reactive monitoring. Reactive monitoring will usually seek to implement risk management measures to prevent recurrence of an accident, or provide a target date for investigation into a particular trend in accident and injury patterns.
3.2.4 A copy of the matrix at the final quarter 2011/2012 is attached at Appendix II. The matrix shows two-hundred and forty-two entries, seventeen of which were new entries during the quarter. S:\CE\DSS\EHS\Internal H&S Reviews\Internal Health and Safety Report 2011 -2012 hundred and forty-two entries sixteen were reactive monitoring. This demonstrates the predominantly proactive (active monitoring) nature of the matrix.
3.2.5 Managers and Assistant Directors are encouraged to use the matrix as a support mechanism in their health and safety management. Timely confirmation to PHSO of actions that have been completed, in part or whole, help to demonstrate at a glance the current corporate status of health and safety management within the Council.
3.2.6 Comparison with last years matrix data can be made to gauge progress in areas such as, for instance, completion time in relation to target dates on high risk issues. Officers or Members wishing to do this i.e. for audit or performance purposes should approach the PHSO for past data.
3.2.7 The matrix may also be used by Assistant Directors and Corporate Managers to inform and assist compilation of the required end of year Checklist (formerly heads of Service checklist), which forms part of the appraisal process, if this procedure is to be continued under the forthcoming revised health and safety policy .
3.3 ACCIDENT & INCIDENT MONITORING AND INVESTIGATION
3.3.1 The accident and incident spreadsheets at Appendix III (notifications by type) and Appendix IV (notifications by detail) show that there were one-hundred and five accidents (29% less than last year), with forty-one incidents (30% less than last year). Of this sum total sixteen (16% less than last year) were reportable to HSE under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR).
3.3.2 The RIDDOR sixteen comprised of ten injuries to employees that caused them to be absent from work for over-three-day absences, four members of the public conveyed to hospital from the scene of an accident on our premises and two releases of a substance hazardous to health. Table 1 provides details.

3.3.3 Manual handling injuries were higher than last year at twenty-three, compared to nineteen previously, but slips-trips-fall injuries were slightly lower at twenty-five, compared to twenty- seven last year. The number of incidents involving contractors and asbestos rose.
3.3.4 The rate for all sixteen RIDDOR reports equate to an incidence of 2,210 injuries per 100,000 employees. Excluding injuries to members of the public i.e. strictly speaking employees the twelve RIDDOR reports equate to an incidence of 1,657 injuries per 100,000. Further excluding the two dangerous occurrences the ten RIDDOR reports equated to an incidence of 1,381injuries per 100,000. These figures are calculated on a full-time equivalent establishment of 724 employees.
3.3.5 The incidence rate is higher than the South West average of 910 injuries per 100,000 employees for 2009/2010 (latest figures available), but it should be borne in mind that we incorporate the Refuse, Cleansing and Recycling employment group, which is a high risk sector second only to the construction industry nationally.
3.3.6 Indeed, 50 % of RIDDORs were in Refuse, Cleansing and Recycling and this is an increase on last years 43%. Furthermore, fifty-one of the one-hundred and five general accidents were in Refuse, Cleansing and Recycling, which equates to around 49% of all accidents in the Council.
3.3.7 The continued close monitoring of Refuse, Cleansing and Recycling aims to reduce accidents and incidents. Rounds monitoring for safety compliance were undertaken during 2011/2012 by the Supervisors, PHSO and Assistant Director Environment. Ongoing membership of the WISH forum also ensures we learn of any new techniques or equipment available to assist as soon as possible. HSE have commenced a waste intervention programme with local authorities. The first meeting between Exeter City Council and HSE took place 21 st March 2012 and further meetings will take place during the forthcoming year.
3.3.8 There were again many incidents of personal threat towards Civil Enforcement Officers (CEOs) this year. There was one injury assault on a CEO and another non-injury assault on a CEO. Elsewhere in the Council, a Street Sweeper was assaulted by a youth and required hospital treatment afterwards for a perforated eardrum and other injuries. An Elected Member was assaulted with a glass bottle during duties assisting licensing. All of these incidents are of great concern and highlight the need to keep safety monitoring and risk control methods under continual review in order to reduce risk further where reasonably practicable.
3.3.9 Where risk assessments and safe working practices are reviewed, all employees involved must be consulted on any changes proposed to reduce risk. Indeed, employees are often the most valuable source of information on how tasks are progressing and are best placed to contribute ideas for improvements in safety. Team meetings must always include health and safety.

4.1 INTERNAL AUDIT
4.1.1 Internal Audit carried out a personal protective equipment audit of teams during this period. This was part of a process audit and followed on from the inspection regimes (safety) audit by Zurich Ltd. Actions from the personal protective equipment audit have been incorporated into the Corporate Health and Safety Action Plan 2012 / 2013.
4.1.2 It is anticipated that the Council will continue to resource the Internal Audit team to enable internal health and safety audits to continue.
4.2 INSURERS AUDIT
4.2.1 Following an audit on inspection (safety) regimes, the Councils insurers, Zurich Municipal, delivered training sessions to a number of key managers and supervisors. This took place on 27 th and 28 th March 2012. A policy to capture and formalise the essence and recommendations of this exercise will be formatted during the coming year under the Corporate Health and Safety Action Plan.
4.3 A SELECTION OF OTHER SALIENT POINTS FROM THE PAST YEAR
4.3.1 Fire safety training was streamlined into a central training policy. 4.3.2 Training in Event Risk Assessment was organised during September 2011. 4.3.3 First Aid practical refreshers were undertaken in October 2011 and March 2012. 4.3.4 Housing concentrated on their lone working policy and procedures. 4.3.5 Parks and Open Spaces had extensive risk reviews of the Citys paddling pools undertaken. 4.3.6 Car Parks have made very favourable progress with ongoing completion of their fire risk assessment reviews. They commenced City incident drill practices in March 2012. 4.3.7 A corporate approach to order and purchase of personal attack alarms was established. 4.3.8 A detailed review of the control of contractors policy has begun and is due for completion in the forthcoming year. 4.3.9 A detailed review of the asbestos management policy has begun and is due for completion in the forthcoming year. 4.3.10 A trial to provide improved quality gloves to Refuse and Cleansing crews is in progress. A survey was conducted by PHSO and Environment. 4.3.11 An electronic induction training package for all employees called Learning Pool was completed by PHSO and HR.
4.4 TOWARDS OHSAS 18001
4.4.1 OHSAS 18001 has been developed to be compatible with the ISO9001 (Quality) and ISO 14001 (Environmental) management systems standards. This was developed in order to facilitate the integration of quality, environmental and occupational health and safety management systems into organisations, should they wish to attain this accreditation.
4.4.2 The specification gives requirements for an occupational health and safety management system that enables an organisation to control its health and safety risks and improve its performance. It does not state specific performance criteria, nor does it give detailed specifications for the design of a management system.
4.4.3. Within the Corporate Risk Register the Councils stated intention is to aim for attaining the standard for OHSAS 18001 by the end of 2017. Should the Council wish to gauge its Exeter City Council Internal Health and Safety Report 2011 - 2012
S:\CE\DSS\EHS\Internal H&S Reviews\Internal Health and Safety Report 2011 -2012 current health and safety culture, the Council may choose to seek an external audit in order to attain RoSPA gold/silver/bronze or the British Safety Council Five Star rating; by the end of 2014, the Council would be in a position to decide whether to proceed with external audit for OHSAS 18001.
4.5 CONCLUSION
4.5.1 One aim of the Health and Safety Management System is to ensure a more consolidated centralised approach to protect the Council and its employees alike. This is the first year that comparative data has been available since the system was revised.
4.5.2 Moderate improvements are noted in areas like accident and incident trends. However, this should be kept in perspective when considering the reduction in the employee full-time equivalent establishment from that of last year and may signify no real change. Indeed, the incidence rate for injuries amongst employees is still higher than average for the South West.
4.5.3 Similarly, assaults on staff and incidents where staff have been threatened have risen slightly.
4.5.4 Notwithstanding the above, the Council over the last tow years has achieved a very good reporting culture in health and safety and this is reflected in the minutes of the key health and safety committees: Key Partners Group, Employee Liaison Forum and Joint Consultative Group.
