General information
You must complete this form and provide the documents that apply to your situation, in the required formats, or else your
application will be rejected.
The form called Documents submitted in support of the Application for Selection Certificate (A-1520-AA) is available in PDF format only.
This form must be attached to your application. Only you, as principal applicant, must complete the form, even if your spouse or a
dependent person aged 22 or older is accompanying you.
Steps to follow
1. Check the box corresponding to each document that you are submitting. The entry N/A (Not Applicable) indicates that you do not need
to submit this document.
2. Gather all the requested documents in the required format (original, certified true copy or photocopy). Refer to the List of authorities
recognized for certifying documents at www.immigration-quebec.gouv.qc.ca/dcs-workers.
3. Place all your documents and supporting items in the order indicated in the form.
4. Include payment of the fee.
5. Send your application to the Ministre office responsible for your country of residence. The postal addresses of our offices are given on
our website at www.immigration-quebec.gouv.qc.ca.

Make sure to send your documents in the required format. A non-conforming format may result in the rejection of the application or
its return to the sender.
Translation
If you are submitting documents or parts of documents in a language other than French or English, you must provide:
The original of the translation
The translation must be done by an approved translator (see the Definitions section) and include the translators seal.
The text, seals and signatures of the original document must be translated.
The document in the language of origin, in the format requested in this form (original, certified true copy or photocopy)
Verification and investigation
Be advised that the Ministre may:
verify the accuracy of your information with third parties, and that you are committing an offence and are liable to a fine if you give the
Ministre, an investigator or inspector any information that you know or should have known to be false or misleading in relation to your
application for a selection certificate;
reject any application that contains false or misleading information or documents;
refuse to examine an application for a selection certificate from a person who, in the past five years, has provided false or misleading
information or documents in relation to an application under the Act respecting immigration to Qubec;
cancel a selection certificate if the application contains false or misleading information or documents, the certificate was issued in error
or the conditions required for its issuance cease to exist.

Definitions
Spouses
Two persons of the same or opposite sex who are married or in a de facto union.
Your spouse is the person who fits the definition of married or de facto spouse given in the Application for Selection Certificate.
Certified true copy
Copy or photocopy of a document that is certified as true to the original by:
the document issuer, or
the authority of the country that issued the document, recognized for certifying documents as true. Consult the List of authorities
recognized for certifying documents at www.immigration-quebec.gouv.qc.ca/dcs-workers.
Example 1 Copies of your diplomas or transcripts must be certified true by:
the administration or registrar of the educational institution (school, college or university) that issued your diploma or transcripts;
or
the authority of the country that issued the document, recognized for certifying documents as true. Consult the List of authorities
recognized for certifying documents at www.immigration-quebec.gouv.qc.ca /dcs-workers.
Example 2 A copy of your act of birth must be certified true by the national, regional or municipal authority in your country of birth
responsible for keeping civil status registers.

The authorized person must sign the copy and append the seal of the document issuer or recognized authority, where applicable.
Approved translator
A translator who is a member of a professional order or authorized to act professionally as such.

Translations done by any other person who is not approved will be refused.

PAYMENT OF FEES
You cannot pay by credit card if
you submit your application to
the Qubec immigration office
in Hong Kong.
You must pay in Canadian currency. Cash and personal cheques are not accepted. For
information on fees charged and acceptable payment methods, consult our website at
www.immigration-quebec.gouv.qc.ca/en/informations/fees.
To pay by credit card (Visa, MasterCard, American Express), you must complete
and enclose with your application:
the Payment by Credit Card form (A-0591-A0).
The processing of your application will not start until we receive payment of the
required fees. If the payment is missing or insufficient, we will return your
application.
