SAFETY AND J OB RULE BOOK

HTG ENGINEERING AND TRADING COMPANY has made s serious commitment to
Safety. In addition to the humanitarian reasons for preventing personal injury, increased
awareness and attention to safety by everyone is essential to the economic health of the
construction industry.
This booklet is issued to identify certain safety and work rules that are mandatory for all
individuals on this project. Following these rules will contribute significantly to your well
being and a successful completion of the project. Violation of some of these rules may
result in immediate dismissal.
Accidents can be prevented through use of good judgement and common sense. By
following safe practices along with all of our safety regulations, those of our customer and
applicable governmental regulations, you can help us in making this project a safe place
of employment for yourself and your fellow employees.
Remember that everyone loses from an accident. We are concerned with your safety.
Help us keep you safe!
If you dont know the safe way well teach you. If you wont work safely, you wont work
here.
A. SAFE PROGRAM RULES
Your safety and accident prevention are of the utmost importance to us, more importantly
to you, and your family who need the protection. To help achieve an accident-free job,
the Safety Rules listed in this booklet will be observed by all employees. Each of us is
required to comply with all regulations. As each project progresses, additional safety
regulations may be required to meet the Owners safety and operational requirements.
Our Safety Program begins with the Construction Manager, who in turn holds each
Supervisor, Engineer and employee accountable for the safety involved in that portion of
work for which each is responsible.
B. GENERAL SAFE PRACTICES
1. All employees shall follow safe procedures, render every aid to safe operation, and
report all unsafe conditions or practices immediately to his/her supervisor.
2. Supervisors are instructed to insist on employees obeying rules and regulations in
order to promote good safety practices for the protection of the employees as is
necessary to the safe conduct of the work. They will take such action as is necessary
to enforce these instructions.
3. Always ask your supervisor if you do not know how or are in doubt as to the safe way
of doing your job.
4. Ignorance of safety rules and practices is no excuse for their violation. If you dont
know the safe way to do a job, well teach you. If you wont know the safe way, you
wont be working with us.
5. Help the new or inexperience employee by pointing out potential hazards.
6. Employees are required to attend the weekly tool box safety meeting.

7. Employees are to be alert and report any deficiency in the way of defective tools,
equipment, guards, or protective devices immediately to the Supervisor or
superintendent.
8. Careful investigation as to possible danger to other persons is necessary before
beginning any operation.
9. Operate only the equipment you are trained and authorized to use.
10. Warning, prohibition, and command signs shall not be removed or changed.
11. Before doing any climbing, inspect the soles of your shoes and clean them of any
slippery substance such as mud or oil.
12. Running is prohibited on this project.
13. HTG ENGI NEERING AND TRADING COMPANY has a chemical hazard recognition
program and maintains files of material safety data sheets for your information. You
will be trained about the program and provided specific training about any hazardous
substances with which you may work. We require your cooperation with the hazard
communication program.
14. HTG ENGINEERING AND TRADING COMPANY also has a procedure for reviewing
high hazard work. If you have a safety concern or suggestion about your work,
contact your Supervisor to initiate a review of the procedures.
C. EMERGENCY PROCEDURES
Emergency procedures will vary with different projects. As you assigned to each project,
emergency procedures governing that location will be published for your information and
use).
D. HOUSEKEEPING
1. Working areas are to be kept clean and free of all debris and waste material.
2. Pieces of lumber with protruding nails are to be promptly piled out of the way and the
nails with drawn or bent over.
3. All scrap is to be removed and deposited in proper containers. Keep all materials piled
neatly.
4. Bolts, nuts, etc. should be kept in containers or removed form the work area when no
longer needed.
5. All tools and other equipment are to be returned to their proper place after use.
6. Special attention is to be given to the removal and disposal of oily rags, paint cans,
and any container that may have held flammable liquids.
7. Eliminate the slipping and falling hazards caused by oil and liquid spillage by
immediately wiping or spreading an absorbing material on the area.
8. Walkways, scaffolds, and stairways shall be kept clear of welding cords, power cords,
hoses and small tools.
9. Welding rods and rod stubs are to be removed from the floor, ground, or gratings.
10. Change rooms and toilets must be kept clean. Place papers and other refuse in the
container provided for their disposal.
E. FOR YOUR PERSONAL PROTECTI ON
1. Employees shall wear suitable work clothing. Shirt tails must be tucked in at all times.

2. Loose or frayed clothing, dangling ties, etc. shall not be worn around moving
machinery or other sources of entanglement. Long hair shall be secured under the
hard hat enclosed in a hair net and/or other similar device.
3. Proper safety work shoes or boots in good repair shall be worn on the job site.
4. All employees shall wear approved hard hats and authorized visitors at all field
locations outside the office area. Your Employers Company Name shall be on your
hard hat.
5. Approved ear protection shall be worn through the work-site. Additional job related
eye protection may be needed such as: suitable goggles and/or face shields shall be
worn when welding, burning, grinding, chipping, handling caustics, or using hot tar or
hot lead. This is a mandatory eye protection project. EYE PROTECTION MUST BE
WORN AT ALL TIMES.
6. Approved ear protection shall be worn when high noise in excess of 85 dba is
encountered.
7. Suitable respirators shall be worn if the concentration of dust, toxic fumes, or other air
contaminants exceeds safe exposure levels.
8. Hair styles and facial hair, which interferes with an employees ability to use
respiratory protection effectively, will not be permitted. You must be fitted and trained
in the use of the respirator.
9. Gloves shall be worn when engaged in activities where danger of hand injury exists.
10. Compressed air is not to be used for blowing dust from body parts or clothing. Never
point an open hose at other persons. Air hose connectors are to have a safety wire or
clamp attached while in use.
11. Use safety belts and life lines when working from high places (over 2 meters).
12. Floor openings shall be guarded by a standard railing and toe-boards or secured
covers.
13. Light Hydrocarbons are not to be used for cleaning purposes.
14. Glass bottles are forbidden on the job-site; only thermos, paper, and metal, or plastic
containers may be used.
15. Unsafe short cuts, such as walking or sliding down pipelines, shall be prohibited.
16. Pedestrians shall face oncoming traffic.
F. MATERIAL HANDLING
1. Remember in lifting, the safe way is the best and easiest way. The back and
stomach muscles are easily injured and they bear most of the strain if lifting is done
incorrectly. When lifting, use the large muscles of your legs, rather than the small
muscles of your back. Take a firm grip, secure a good footing, place the feet a
comfortable distance apart, keep the load closed, set your back in its normal S curve
(like a weight lifter), bend your knees, and lift with your legs. If the load is too heavy,
get help. Keep one elbow tucked into your side to prevent twisting and straining your
back.
2. Never use ropes, chains, or slings that are defective. Make sure all loads are centered
in slings before lighting and use extreme care in removing slings from piled stock. Use
tag lines to control the load.

3. Dont stand or walk under loads. Areas under suspended loads must be barricaded
and danger signs posted.
4. Dont ride loads. Lifting of personnel by crane is prohibited, unless done by means of
a crane basket and personnel safety belt or harness tied above the ball.
5. Attach a guideline or tag line to all equipment and materials begin lifted to higher
levels. All loads are to be secured properly and tied off when being transported by
vehicles. Cherry pickers, crawlers, and truck cranes are to use tag lines while moving
loads and tie the load off to the vehicle.
6. Only one signalman is allowed to signal a load at a time.
7. Chokers, when not in use, shall be kept on a rack or a suitable assigned storage place.
Damaged chokers shall be removed from service and destroyed. The sling inspection
program will be enforced and recorded.
G. TRANSPORTATION
1. All employees authorized to operate vehicles on Client property must obey traffic sign
and signals. In all cases, the low-posted speed limit shall govern.
2. All loads are to be secured properly, and hauled material that over-hangs the sides or
end of a truck shall be marked with red flags. Loads shall not be suspended from a
cable that allows free swinging beyond the sides of a truck or crane. Dragging or
skidding material and objects along roads is prohibited.
3. Employees are not allowed to ride on running boards, fenders, side rails, tailgates,
tops of vehicles, extend legs over the side of any truck bed or stand up in the bed of a
truck or vehicles while the machine is in motion. Passengers may only be transported
when there are adequate seats or other safety provisions made.
4. All vehicles parked within the job site must be left uncloaked and with the ignition key
in place.
5. Never park in front of a hydrant or switchboard.
H. ELECTRI CAL
1. Electrical work shall only be undertaken by qualified persons familiar with the electrical
code requirement and qualified for the class of work being performed.
2. All machinery and electrical equipment; regardless of the type of power shall be
tagged and tested for proper safeguarding before any employee begins maintenance
or repair work.
3. When working in or on energized substations, transmission lines, power plants, etc.
accident prevention rules, procedures, and regulations of our client and FDM are to be
followed by all employees.
4. Metal ladders shall NOT be used.
5. Turn in extension cords or electric hand tools with exposed wires or other defects.
Splices must have soldered wire connections with insulation equal to the cable. Worn
or frayed cords shall not be used.
6. Place extension cords or electric cables so that others will not trip over them. Feeder
cables must be hung with insulated supports or laid clear of workmen or moving
equipment, not fastened with staples, hung from nails or wires.
7. Always respect electricity! It takes very little electric current to kill. There have been
many cases where amperage as low as 500 milliamps has caused death.

I. EXCAVATI ON AND TRENCHES
1. All excavation work or similar work, like digging, setting out pegs or any work carried
out beyond a depth of 100mm, shall be covered by an Excavation Permit.
2. Banks shall be shored, laid back to a stable slope or some other equivalent means of
protection shall be provided where employees may be exposed to moving ground or
cave-in.
3. Excavated material shall be effectively stored and retained. The minimum distance
between the toe of the pile and the edge of the trench shall be 1.5 times the depth of
the trench. A strip of 1 meter wide on both sides of the trench shall be leveled and
cleared of all loose objects or material.
4. Access ladders every 7 meters of lateral travel shall be provided in trenches where
employees are working.
5. All excavations, trenches, and openings must be flagged, barricaded and identified,
when necessary, with approved amber lights.
6. Extreme case shall be taken when excavating in the vicinity of underground piping or
electrical cables.
7. Electrical cables or pipeline exposed through excavating shall be sufficiently supported
and protected to prevent any damage.
J . WELDING AND CUTTING
1. Only experienced persons are allowed to do any electric or acetylene welding or
cutting.
2. No one shall do any welding or cuttings in hazardous are without instructions and
written burning and welding permits from the proper authority.
3. Warning signs shall be posted around and at each level below the area of each
overhead welding or cutting operation. No hydrocarbons shall be allowed below hot
work areas.
4. Welding or cutting should not be done on drums, barrier, tanks, or other containers. If
such welding is necessary, care shall be taken to assure the container is purged and
gas free.
5. Your supply of breathing oxygen burns up when you weld in closed or confined
places. Make sure there is plenty of fresh air-lines, blowers, or other means. When
welding stainless steel in enclosed or confined spaces, local ventilation or air-supplied
respirators are required unless industrial hygiene sampling indicates that the general
ventilation is adequate.
6. Wear proper head, eye and face protection when welding.
7. Welders should be responsible to see that the helpers eyes are properly protected at all
times.
8. Never allow oil or grease to come in contact with oxygen equipment.
9. Always consider a cylinder as full and handle it accordingly. Never drop or permit
cylinders to strike together.
10. All cylinders are to be placed in carts or tied off in a vertical position. Cylinders when
not in use must have safety s on. Empty cylinders are to be removed from
construction areas and returned to rack. Oxygen cylinders in storage shall be

separated from fuel gas cylinders by a minimum distance of 6 meters or by a
noncombustible barrier at least 2 meters in height.
K. LADDERS AND SCAFFOLDS
1. Access ladders will be tied off. No one is permitted to stand on or work from any step
ladder beyond the third step from the top.
2. A ladder should not be placed at a blind corner or a doorway where if could be struck.
If it is necessary to use a ladder in such places, the area is to be blocked off. Ladders
are not to be placed upon unstable objects; such as, boxes, loose lumber, ect
3. Portable ladders are to be securely fastened, anchored, or tied off with ropes when
being used. Leather shoes shall be used on all ladders.
4. Metal ladders are NOT to be used on this project.
5. Face the ladder when ascending or descending and use both hands. Raise or lower
tools or equipment with ropes or tool bags.
6. For any work that is carried out at a height of 2 meters or move above ground level,
scaffolding is required. Scaffolding can only be used when it is approved and a green
signboard marked SCAFFOLD COMPLETE, SAFE FOR USE is displayed near the
access ladder. A red tag indicates that a scaffold is not approved or not ready for use.
7. Inspect the scaffold and ladders on which you are to work to make certain they are
safe. Broken or deficient scaffolds or ladders are to be withdrawn from service and
tagged DO NOT USE until they have been repaired and the tag removed.
8. Any damage to scaffolds or other supporting structures must be reported promptly to
your supervisor.
9. Suitable handrails for personnel shall be installed on all scaffolds. Guardrails and toe-
boards shall be installed on all open sides and ends of scaffolds and platforms.
L. HAND TOOLS
1. All electrical equipment shall be properly grounded subject to periodic inspection.
2. All hand-held power tools, such as drills, grinders, etc. shall be equipped with a trigger
dead man switch and lockout switches shall be removed. If greater than 110 volts the
power tool must be doubly insulated.
3. Inspect cords of electric tools regularly. Dont use electrical cords for lifting tools and
materials.
4. Dont use defective tools. Report them to your supervisor.
5. Tools are not to be left lying in elevated places, in walkways, aisles, or stairways.
6. No tools, materials, etc. will be thrown down from high places. Use rope or tool bags
to raise and lower these items.
M. EQUI PMENT AND MACHI NERY
1. Only trained, experienced people are allowed to operate power equipment.
2. Do not operate equipment in an unsafe condition. Repairs or defects ae to be reported
immediately to your supervisor.
3. Cleaning, oiling, fuelling or repairing is not to be done on equipment while it is in
operation.
4. Equipment cannot be operated without an approved Equipment Safety Checklist.

N. FIRE PREVENTION
1. No matches or lighters are allowed on the job-site. All employees must take every
precaution to prevent fire. Do not tamper with fire fighting equipment. Observe NO
SMOKING rules.
2. In case of fire: KEEP CALM. Report the fire immediately, giving the location, your
name, and telephone extension if applicable. Evacuate non-essential people from the
area. Consider first the safety of all personnel, then direct attention to the saving of
property.
3. Know where the fire extinguishers and hydrants are located and how to operate them.
After a fire extinguisher has been used, notify your supervisor whose duty it is to see
that proper replacement is made promptly.
O. RADIATION SAFETY
1. Obey all barricades and warning signs that are installed around all areas where an X-
ray radiation or other source is in use or stored.
2. Radiological workers must observe safe working practices and wear radiation-
monitoring devices. They must have regular medical checkups.
P. OFFI CE SAFETY
1. Good housekeeping in the office is a must. Keep your desk and cabinets clean and
orderly. An open drawer of a desk or a cabinet is a hazard which can cause yourself
and others to trip or collide.
2. Dont keep defective chairs and desk in use. Report them to your supervisor for
replacement or repair.
3. Carry pencils, fountain pens, scissors, etc. in such a way that sharp points cannot
cause puncture wounds to yourself or others.
4. Cords on electrically operated office machines and telephones can cause tripping
hazards when they are left on the floor or across walkways.
5. The standard four drawer filling cabinet can cause serious injury if it upsets as a
result of opening a heavily loaded top drawer if the lower drawers are empty or filled
with light weight material.
6. Never adjust or clean a piece of office machinery while it is in operation.
7. Dont attempt any electrical repairs. Your supervisor will call a qualified electrician.
8. Do your reading at your desk not while walking.
9. When using stairway, take your time. Use the handrails.
10. Dont stand and talk in front of closed doors that may be opened suddenly.
11. If you are permitted to smoke, remember the fire hazard. Use ashtrays, not
wastebaskets.
12. If you must reach high or do any climbing, use a safe ladder and not make shift
boxes, cabinets, etc.
13. Switch off electrical equipment when not in use over a long period of time and at the
end of each working day.

Q. PARKING FACILITIES AND ACCESS TO THE PLANT
Limited parking facilities have been provided for your employers use at your own risk.
Please park only in designated areas. Respect your fellow employees and their vehicles in
the parking lot. Enter the plant at all times through your designated gate only and
proceed to your work site by the directed route with no deviations or shortcuts.
R. I DENTI FI CATI ON
Badge numbers are needed for gate clearance and identification. Employees shall wear
their badge in full view at all times.
S. CONDUCT ON THE J OB
Employees violating the rules of conduct listed below will be subject to termination from
the project.
1. Willful or grossly negligent violation of any safety, fire, security, sanitary, or medical
rules and practices; or engaging in any conduct which creates a safety hazard.
2. Failure to use or wear safety equipment after one warning.
3. Blatant violation of safety or security rules.
4. Theft of company, client, or employee property. Employee will be subject to
prosecution.
5. Willful or grossly negligent damage or mutilation of company property.
6. Possession, consumption, or being under the apparent influence of alcohol or illegal or
non-prescribed drugs on the project.
7. Acts of sabotage or defacing buildings, facilities, and/or equipment.
8. Possession of firearms or other lethal weapons on the project.
9. Picking up or leaving another employees badge, employee number and identification.
10. Making or stating false claims or falsifying any reports or record.
11. Insubordination.
12. Assault on co-worker or supervisory personnel.
13. Fighting on the project.
14. Bringing cameras on client property and taking pictures without the express
permission of proper client authorities.
15. Engaging in horseplay.
16. Sleeping in the job-site.
17. Eating, drinking and/or smoking in any area where prohibited.
18. Use of personal transistor radios, etc. during working hours.
19. Cooking or heating food on project.
20. Refusing to submit to lunch box and/or vehicle inspection.
21. Misuse of tools, equipment, or facilities.
T. J OB I NJ URI ES AND SI CKNESS
1. First-aid facilities are available at the job-site and must be used for all injuries. All
injuries, no matter how minor, must be immediately reported to the employees
supervisor. Use the first-aid facilities; they are for your protection.

2. Do not move an injured person unless it is necessary to do so in order to prevent
further injury. Contract first aid and your supervisor.
3. Haste in giving first aid is sometimes harmful. There are two instances when speed is
necessary severe bleeding and artificial respiration.
4. Do not attempt to remove foreign particles from your eye or body or from the eye or
body of another. Get qualified treatment from first aid.
U. SANITARY FACILITIES
Adequate sanitary facilities are provided on the job-site and are to be used by all
employees. We request your operation in maintaining these facilities in a clean and
orderly condition.
V. REMAINING IN WORK AREAS
Each employees must remain on the job-site at his/her work location at all times during
regular working hours, unless authorized to leave by his supervisor. J obsite boundaries
include those bound by the perimeter fence. Existing/operating portions of the plant shall
not be entered without proper authorities.
