operator. Letter of authorization for representation is a mandatory document to be attached to the
application.

not mandatory and that rejection of publication on the internet will in no way affect the processing of my
application or any customs formalities in which I am involved.

I authorize / do not authorize (tick the appropriate box) the Customs Administration of the Republic of
Slovenia to supply the European Commission with the data referred to in Annex 38d, points 1, 2 and 3 of
Regulation 2454/1993 (EORI number, name and address) for their publication on the European
Commissions' website.
