Step 1

Overview
The Step 1 examination is the first component of the
USMLE and is taken by medical students and graduates
seeking to practice medicine in the United States. The
overall purpose of the Step 1 exam is to assess an
individuals basic science knowledge. The content that
comprises this multiple choice exam is organized into
general science principles and individual organ systems.
While some of the exam questions may involve testing
an examinees range of knowledge, most of the
questions place a strong emphasis on the application of
basic science principles in the practice of clinical
medicine.
Examination Content
Step 1 includes test items in the following content
areas:
o Anatomy
o Behavioral sciences
o Biochemistry
o Microbiology
o Pathology
o Pharmacology
o Physiology
o Interdisciplinary topics, such as nutrition, genetics, and
aging
Step 1 is a broadly based, integrated examination. Test
items commonly require you to perform one or more of
the following tasks:
o Interpret graphic and tabular material
o Identify gross and microscopic pathologic and normal
specimens
o Apply basic science knowledge to clinical problems
Step 1 classifies test items along two dimensions,
system and process, as shown below:

Test Length and Format
Step 1 has approximately 322 multiple-choice test items.
This is divided into seven sixty minute blocks and
administered in one eight hour testing session. For Step
1, during the defined time to complete the items in each
block, you may answer the items in any order, review
your responses, and change answers. After you exit the
block, or when time expires, you can no longer review
test items or change answers.
Eligibility Criteria
To be eligible to take the USMLE Step 1, you must be in
one of the following categories at the time of application
and on the test day:
o a medical student officially enrolled in, or a
graduate of, a US or Canadian medical school
program leading to the MD degree that is
accredited by the Liaison Committee on Medical
Education (LCME),
o a medical student officially enrolled in, or a
graduate of, a US medical school leading to the DO
degree that is accredited by the American
Osteopathic Association (AOA), or
o a medical student officially enrolled in, or a
graduate of, a medical school outside the United
States and Canada and eligible for examination by
the ECFMG.






Step 2 CK
Overview
The Step 2 Clinical Knowledge (Step 2 CK) portion of the
Step 2 exam consists of multiple-choice questions that
reflect the broad representation of academics, clinical
practice, and licensing communities across the United
States and Canada. The clinical science material of the
Step 2 CK is organized along two dimensions:
o Normal conditions and diseases (normal growth and
development, basic concepts, and general principles)
and
o Physician tasks (preventive medicine and health
maintenance, understanding mechanisms of disease,
establishing a diagnosis, and patient management).
Examination Content
Step 2 CK includes test items in the following content
areas:
o Internal Medicine
o Obstetrics and Gynecology
o Pediatrics
o Preventive Medicine
o Psychiatry
o Surgery
o Other areas relevant to provision of care under
supervision
Most Step 2 CK test items describe clinical situations and
require that you provide one or more of the following:
o a diagnosis,
o a prognosis,
o an indication of underlying mechanisms of disease,
o the next step in medical care, including preventive
measures.

Test Length and Format
Step 2 CK has maximum of 355 multiple-choice test
items. It is divided into eight sixty minute blocks and
administered in one nine hour testing session. For Step
2 CK, during the defined time to complete the items in
each block, you may answer the items in any order,
review your responses, and change answers. After you
exit the block or when time expires, you can no longer
review test items or change answers.
Eligibility Criteria
To be eligible for Step 2 CK, you must be in one of the
following categories at the time of application and on
the test day:
o a medical student officially enrolled in, or a graduate of,
a US or Canadian medical school;
o program leading to the MD degree that is accredited by
the Liaison Committee on Medical Education (LCME);
o a medical student officially enrolled in, or a graduate of,
a US medical school leading to the DO degree that is
accredited by the American Osteopathic Association
(AOA); or
o a medical student officially enrolled in, or a graduate of,
a medical school outside the United States and Canada
and eligible for examination by the ECFMG.


Step 2 CS
Overview
The Step 2 Clinical Skills (Step 2 CS) portion of the Step
2 exam assesses whether you can demonstrate the
fundamental clinical skills essential for safe and effective
patient care under supervision. There are three
subcomponents of Step 2 CS:
Integrated Clinical Encounter (ICE)
o Data gathering - patient information collected by history
taking and physical examination
o Documentation - completion of a patient note
summarizing the findings of the patient encounter,
diagnostic impression, and initial patient work-up
Communication and Interpersonal Skills (CIS)
o Questioning skills
o Information-sharing skills
o Professional manner and rapport
Spoken English Proficiency (SEP)
o Clarity of spoken English communication within the
context of the doctor-patient encounter
Step 2 CS uses standardized patients, i.e., people
trained to portray real patients in the examination. You
are expected to establish rapport with the patients, elicit
pertinent historical information from them, perform
focused physical examinations, answer patient
questions, and provide counseling when appropriate.
After each patient interaction, you will record pertinent
history and physical examination findings, list diagnostic
impressions, and outline plans for further evaluation, if
necessary. The cases cover common and important
situations that a physician is likely to encounter in
common medical practice in clinics, doctors' offices,
emergency departments, and hospital settings in the
United States.
Test Length and Format
Step 2 CS has twelve patient cases. You will have fifteen
minutes for each patient encounter and ten minutes to
record each patient note. If you do not use the entire
fifteen minutes for the patient encounter, the remaining
time will be added to the time you have to record the
patient note. The testing session is approximately eight
hours. Step 2 CS cases must be taken in the order
presented. After you exit the case or session, or when
time expires, you can no longer review test items or
cases, change answers, or collect additional information.
Eligibility Criteria
To be eligible for Step 2 CS, you must be in one of the
following categories at the time of application and on
the test day:
o a medical student officially enrolled in, or a graduate of,
a US or Canadian medical school;
o program leading to the MD degree that is accredited by
the Liaison Committee on Medical Education (LCME);
o a medical student officially enrolled in, or a graduate of,
a US medical school leading to the DO degree that is
accredited by the American Osteopathic Association

Step 3
Overview
Step 3 consists of multiple choice and computer based
simulations to assess a medical student or graduates
application of their medical knowledge and
understanding of biomedical and clinical science in
managing patients in an unsupervised setting. The
results of this exam demonstrate their ability to
independently provide a standard of care to their
patients. Step 3 is the final component of the USMLE
exam series.
Examination Content
The Step 3 examination is organized in three organizing
dimensions:
o Normal conditions and disease categories (normal
growth and development, basic concepts, and general
principles)
o Clinical encounter frame (initial work up, continuing
care, urgent intervention)
o Physician task (applying scientific concepts, formulating
a diagnosis based on history, physical exam, and lab
findings, and managing the patient).
Clinical encounter frames are common clinical scenarios
physicians may encounter. They range from
nonemergency problems, to the continuity of care, to
life-threatening emergency situations encountered in
emergency departments, clinics, offices, care facilities,
inpatient settings, and on the telephone. Each test item,
in an encounter frame, represents one of the six
physician tasks. For example, initial care encounters
emphasize taking a history and performing a physical
examination. In contrast, continued care encounters
emphasize decisions regarding prognosis and
management.
Test Length and Format
Step 3 examinees test using two formats: multiple
choice questions and Primum computer-based case
simulations (CCS), a testing format that allows you to
provide care for a simulated patient.
Step 3 has approximately 480 multiple-choice test items,
divided into blocks of thirty-six to forty-eight items. You
will have sixty to forty-five minutes to complete each of
these blocks. There are approximately twelve computer
based case simulations, with one case in each block.
You will have a maximum of twenty or ten minutes
depending on the case to complete each of these
blocks. Step 3 is administered in two eight hour testing
sessions.
Step 3 case simulations must be taken in the order
presented. After you exit the case or session or when
time expires, you can no longer review test items or
cases, change answers, or collect additional information.
Eligibility Criteria
To be eligible for Step 3, prior to submitting your
application, you must:
o obtain the MD degree (or its equivalent) or the DO
degree,
o pass Step 1, Step 2 CK, and, if required based upon the
rules referenced below, Step 2 CS,
o obtain certification by the ECFMG or successfully
complete a "Fifth Pathway" program if you are a
graduate of a medical school outside the United States
and Canada (information on Fifth Pathway programs is
available in ECFMGs Information Booklet, and
o meet the Step 3 requirements set by the medical
licensing authority to which you are applying.
