Dear Sioux Valley Parents and Students:
The 2014-2015 school term begins with a normal day of classes on Wednesday, August 20, 2014. The
staff and administration are looking forward to the return of the students. There will be a Back-to-School
event on the evening of August 18, 2014. You are encouraged to attend.
Registration for Elementary Students will be on August 5 and Middle School and High School
registration will be August 7.
On the Sioux Valley website you will find information that we hope is helpful to you as the school term
begins. We frequently place informational items on our home page and provide links to the other school
related informational sites.
Applications for free or reduced meals are also online and additional copies will be available at
registration. If you qualify for these programs, we hope you will participate. All information is kept
confidential. Participation does benefit the school district financially because of reimbursement we
receive for serving those meals. The percentage of students on free or reduced meals is used to determine
funding levels for many other programs as well. If you are eligible, we strongly encourage you to take
advantage of the program.
We will use the School Reach phone system during the school year to contact you with important
announcements, weather related closing or postponements, and emergency notices. It is important that
you provide the school with a current phone number so we can reach you via the School Reach automated
dialing system.
Please review all the information made available on the website.
If there is anything we can do to help your child, please do not hesitate to contact us. We trust that your
experience with our school will be a positive one.

Sioux Valley Elementary
Welcome Back to all students who will be attending our excellent elementary school! I can assure you that our
students and staff are top notch! We are looking forward to starting our new school year and I know you will
all have a very positive school experience. I hope everyone is as excited for school as I am!
The teachers have been in and out of the school a lot this summer. They have spent countless hours preparing
their classrooms, lesson plans and themselves for the 2014-15 school year.
There have been some changes in staffing at Sioux Valley Elementary. We have Alanna Bezdichek teaching

grade, Dawn Lucas in kindergarten, and Sarah Jensen will be our new Junior Kindergarten teacher. I am
looking forward to working with all of the staff at Sioux Valley Elementary!

On behalf of the entire Sioux Valley Staff, we want to welcome you back and hope that your family had an
enjoyable summer. We are looking forward to another excellent school year. The following schedule will
help your family prepare for classes this fall.
Thursday, August 7 Registration
Hours are from 8:00 am - 12:00 pm and 1:00 pm - 4:00 pm.
Parents will need to update their contact/emergency information for the 2014-15 school year.
Middle school and high school students may pick up their class schedule and locker assignment.
School pictures will be taken. Please pick up picture packets at the school business office, Holiday,
Caseys or Jagers.
Students may purchase an activity ticket. The cost is $20 and provides admittance to all activities.
Students may deposit money into their lunch accounts.
Yearbooks will be available for pick up.

Open House hours are from 5:00 pm to 8:30 pm.
Students and parents/guardians are invited to stop by to visit classrooms. Teachers will be available

Welcome to the 2014-2015 school year. We are looking forward to a successful year at Sioux Valley School.
Activity tickets and season tickets may be purchased and meal account deposits may be made at the business office
between 8:00 am to 12:00 noon and 1:00 pm to 4:00 pm. We encourage you to take care of these items prior to the beginning of
school.

FOOD SERVICE INFORMATION
We look forward to the coming school year at the Sioux Valley School District as Lunchtime Solutions starts their eleventh year in
providing meals to the District. The purpose of this communication is to provide you with some idea of what to expect from the
school food service program this year. If you have any questions or comments about our program, please feel free to call Heather

School Breakfast
Breakfast is served in the dining area before school starts. All food should be eaten in the dining area and not taken to classrooms.
Please encourage your child to participate in school breakfast. Research studies show that eating breakfast can help students
perform better in the classroom. Breakfast prices are:

School Lunch
School lunch offers a wide variety of choices for students. All students are offered three entre choices daily. Entrees may
include Home-style Chicken & Noodles, Italian foods, Mexican foods, Chicken Strips, Entre Salads, Wraps and Sub
Sandwiches. All meals include an entre choice, a hot vegetable, a milk choice, plus the all you can eat Fruit & Veggie Bar.
Bread and peanut butter will no longer be available on the Fruit and Veggie Bar according to USDA guidelines.

NEWLY RELEASED NUTRITION STANDARDS FROM USDA
Your ProgramAhead of the Curve on the New Nutritional Requirements
The new school meal guidelines announced by the USDA in 2012 continue to be phased in per the required
implementation timeline. Beginning school year 2014-2015:
Graduated sodium limits
Whole grains all grains offered must be 100% whole grain rich
Fruits minimum of 1 cup of fruit must be offered at breakfast
Breakfast meals must contain cup fruit or vegetable on the tray
A la carte competitive food rules first year of implementation is 2014-2015
Menus and a la carte offerings will continue to be constructed so they appeal to students and exceed the
requirements so we remain in total compliance well ahead of the required schedules.
Diet Modifications
Families are responsible for notifying the School Food Service if their child requires a diet modification to be addressed and
monitored by the School Food Service. A licensed physician must provide specific written medical documentation. Please
contact the School Food Service office at 627-5657 ext. 276 for more information or Heather Wahl at 712-266-6469 for more
information.
Please note: Students may NOT substitute juice for milk. Federal guidelines require that milk must be served with the meal.
The only exception to the requirement is if a physician provides written documentation that the child will have a life
threatening reaction if milk is consumed. Juice may be purchased separately.
Fruit & Veggie Bar (All you can Eat!)
Fruit & Veggie Bar All students purchasing the regular school lunch may have extra servings from the fruit & veggie bar:
fresh fruit, canned fruit, and fresh vegetables at no charge. All extra servings of these items are free. With entre choices and
the all you can eat fruit & veggie bar, no student should be hungry unless they choose to be hungry!
Extra Milk Half-Pint Cartons
One 8 oz carton will be included in the cost of a K-12 student lunch. One carton will be included in a student breakfast, all
grades. Extra milk beyond the carton included with the meals will be $0.50.
Extra Entre
All Students may purchase an additional entre offered as part of the school lunch program for $1.60 and Extra Branded (such
as Pizza Ranch) Pizza (MS/HS) for $1.80. For example, if a student wants a second entre, such as a slice of pizza, that entre
may be selected in the lunch line and charged to the students account. Extra entrees are only sold to students who first
purchase a school lunch. Again, all students may have extra servings of canned fruit, fresh fruit, vegetables, and salads at no
charge. All extra servings of these items are free. If a student is free or reduced they may receive one entre with their meal
but they must have money in their account to purchase extra entrees.
School Lunch & Breakfast Account
Parents are notified when money is due on a lunch account. The school board has a policy that no more than $8.00 per
student, maximum of $16.00 per family, may be charged for school lunches. If the account is overdrawn beyond that amount,
the school may refuse to serve the student(s) until the account is brought current. If the student is a free student, they still will
be able to purchase a school lunch, but no extra entre will be allowed.
A Point of Sale (POS) software system is used for the school lunch program. Please note the following important information:
Each student will have his/her own individual Student Account.
Deposits must be turned in by 9:00 a.m.
Please make checks to Sioux Valley School.
If sending cash with a student, place in a sealed envelope, along with the students name and lunch account number. We
are not responsible for lost, unidentified, or stolen cash.
Students must have money deposited in their lunch account in order to purchase extra entrees, regardless of eligibility
status (Paid, Free, and Reduced).
Lunch Account Deposits must be made before school starts. Parents are encouraged to make a minimum deposit at the
business office of $40.00 per student for meals before school begins Aug. 20

. Because we receive an overwhelming amount
of deposits the morning of the first day of school, we cannot guarantee that we will be able to input all deposits into accounts
before the lunch period begins. To insure that your deposit is credited to your childs lunch account on the first day of school,
please plan to make a deposit during the registrations dates in August or send your first deposit, for each child in your family,
to the address below:

Availability of on line credit/debit card meal payments and online account balance viewing. Visit svs.k12.sd.us
parent/student resources, lunch prepay to set up an online meal account. Meal deposits may then be made with a
credit or debit card at your convenience. Student lunch balances can also be viewed after creating an account.
Free & Reduced Meal Applications: Please submit application prior to August 15, 2014, to insure approval before the
first day of school. If applications are brought in on the first day of school or later, students will be required to pay regular
prices for meals until the application is processed. Students who are eligible for free or reduced price meals may get only
one meal at that rate, extra Lunch Entrees are $1.60 and Extra Branded Pizza (MS/HS) is $1.80.

Outside Food and Beverage
The District does not allow the delivery of food or beverages from outside vendors or fast food establishments for a students
breakfast or lunch meal. All meals are to be provided by the Districts School Food Service or a prepared meal from home

Dear Parent/Guardian:
Children need healthy meals to learn. The Sioux Valley Schools offers healthy meals every day that it is open USDA
provides reimbursement for healthy meals and snacks served to children. Breakfast costs $1.50; lunch costs $2.40
(JK 5) or $2.50 (6-12). Children may qualify for free meals or for reduced price meals. Reduced price is $.30 for
breakfast, $.40 for lunch. If the children are eligible for free or reduced price lunch, they are also eligible for free or
reduced price breakfast.
If you have received a Notice of Direct Certification for free meals, do not complete the application. Do let the
school know if any children in your household are not listed on the Notice of Direct Certification letter you
received.

1. Who can get free meals without providing income information?
Schoolchildren from households getting Supplemental Nutrition Assistance Program (SNAP) and Temporary
Assistance for Needy Families (TANF) can get free meals without applying. Contact the school if you are on
SNAP/TANF but do not get a letter from the school... Direct certification does not apply to the Child & Adult Care
Food Program.
Children in households getting assistance through the Food Distribution Program on Indian Reservations (FDPIR) can
get free meals. Contact the school if you are on FDPIR but did not get a letter from the school about free meals.
Foster children (see #9 below.) and children enrolled in Head Start are eligible for free meals.
Homeless, runaway, and migrant and Head Start children usually are eligible for free meals. Please call the school if you have
not been told already that they will get free meals.
2. Who needs to fill out an application to get free or reduced price meals?
If you receive benefits but do not have your notice from the school or FDPIR, fill out an application and write your
FDPIR, SNAP, or TANF case number on it. Turn that into the school/center.
If your household income is within the limits on the Income Guidelines Chart with this application, fill out an
application.
Children in households who get WIC or Medicaid may be eligible for free or reduced price meals. Please fill out
an application and list family members and income.
3. Do I need to fill out an application for each of my children? No. Use one application for all children from your
household. We cannot approve a form that is not complete, so be sure to read the instructions carefully and fill out all
required information.
4. Should I fill out an application if I received a letter this school year saying my children are approved for free meals? No, but
please read the letter you got carefully and follow the instructions. Call the school at [phone number] if you have questions.
5. My childs application was approved last year. Do I need to fill out a new one? Yes. Your childs eligibility from last year is only
good for that school year and for the first few days of this school year. You must send in a new application unless the school told you that
your child is eligible for the new school year.
6. Who can get reduced price meals? Your children can get reduced price meals if your household income is within the
reduced price limits on the Income Guidelines Chart, shown on this application.
7. May I fill out a form if someone in my household is not a U.S. citizen? Yes. You or your children do not have to be
U.S. citizens to qualify for meal benefits.
8. Who should I include as members of my household? You must include everyone in your household who shares
income and expenses. This includes grandparents, other relatives, or friends who live with you. You must include yourself
and all children who live with you. You also may include foster children who live with you.
9. What should I report as income? The income you report must be the total gross income listed by source for each
household member received last month. If last months income does not accurately reflect your circumstances, you may
provide an estimate of your monthly income. If no significant change has occurred, you may use last months income as a
basis to make this projection. List the amount that is usually listed on your paystubs and how often you get the paycheck.
For example, if you normally get $1000 each month, but you missed some work last month and only got $900, put down
that you get $1000 per month. If you normally get overtime, include it. If you only get overtime sometimes, do not include
it. If a household member, including children, does not have any income, be sure to check that box.
10. What if my income changes during the year or my SNAP, TANF, or FDPIR benefits change? If your application
for free or reduced price benefits was properly approved, you will remain eligible for those benefits for an allotted time
period. You may visit with a school/center official to get the exact date the meal benefits will expire.
11. What if I have foster children? Foster children that are under the legal responsibility of a foster care agency or court
are eligible for free meals. Any foster child in the household is eligible for free meals regardless of income. Households
may include foster children on the application, but are not required to include payments received for care of the foster
child as income. Households wishing to apply for meal benefits for foster children should contact (insert name, address,
and phone number).
12. We are in the military.
Do we include our housing, food, or clothing allowances and supplemental allowances as income? If your
housing is part of the Military Housing Privatization Initiative and you receive the Family Subsistence
Supplemental Allowance, do not include these allowances as income. Also, concerning deployed service
members, only that portion of a deployed service members income made available by them or on their behalf to
the household will be counted as income to the household. Combat Pay, including Deployment Extension
Incentive Pay (DEIP) is also excluded and will not be counted as income to the household. All other allowances
must be included in your gross income.
My spouse is deployed to a combat zone. Is the combat pay counted as income? No, if the combat pay is
received in addition to the basic pay because of deployment and it was not received before deployment, combat
pay is not counted as income. Contact your school for more information.
13. Will you tell anyone else about the information on my form? We will use the information on your form to decide if
your children should get free or reduced price meals. We may inform officials associated with other child nutrition, health,
and education programs of the information on your form to determine benefits for those programs or for funding and/or
evaluation purposes.
14. Will the information I give be checked? Maybe. We may ask you to send written proof to verify the information you
submitted on the form.
15. What if I do not agree with the school's or center's decision about my application? You should talk to
school/center officials by calling Business Manager Lori Nelson at 627-5657. You may also ask for a hearing by calling or

16. If I do not qualify now, may I apply again later? Yes. You may apply at any time during the year if your household
size goes up, income goes down, if you start getting SNAP, FDPIR, or TANF, if you are temporarily laid off, or temporarily
disabled.
17. What if my child needs special foods? The school/center will make substitutions to the regular meal pattern for
children whose disability restricts their diet when a physician certifies that disability. The staff may choose to make
substitutions for individual children who do not have a disability, but who cannot eat a food item due to medical or other
special dietary needs that are supported by a certified medical authority. These cases will be handled on a case-by-case
basis. Please call the school/center food service department for further information to request the special diet.
18. My family needs more help. Are there other programs we might apply for? Contact the local assistance office to
find out how to apply for SNAP or other assistance benefits.

Find your household size. HOUSEHOLD is: All persons, including parents, guardians, children (including foster children and
exchange students), college students, grandparents, and all people related or unrelated who live in your home and share living
expenses.
Find your household income frequency. TOTAL HOUSEHOLD INCOME is the income each household member got last month
before taxes. This includes wages, social security, pension, unemployment, welfare child support, alimony, and any other cash

SIOUX VALLEY ELEMENTARY PTO (Parent Teacher Organization)

Welcome to the 2014-2015 School Year!
The Sioux Valley Elementary PTOs mission is to ensure a successful learning experience for all children by providing a
supportive environment for students, teachers, and families.
We had a busy school year and were able to sponsor numerous activities such as Hot Chocolate Day, Root Beer Floats on the
last day of school, Library Summer Reading Program, staff appreciation events and various improvements around the school. Recently
we purchased additional benches on the playground, painted the shelter of the Outdoor Classroom and planning is underway to
sponsor an educational assembly this year. A few of our bigger accomplishments over the past few years was updating the entry way
to include new carpet, paint and a Sioux Valley rug to welcome students and visitors to our great school! These projects would not
have been possible without the overwhelming support of parents and community members.
Our fundraising efforts include The Cossack Store and Cossack apparel sales. Another way you can help is by saving Hy-Vee
receipts and Box Tops for Education and depositing them in the black containers outside of the Elementary office. The PTO receives
$1,000 for every $150,000 in receipts! As we all know, every little bit helps and we appreciate your efforts and generosity helping
raise funds for the PTO. Our goal is to continue providing Sioux Valley students, teachers and families with a supportive environment!
We welcome new members at any time. Our monthly meetings are held in the library on the first Tuesday of the month at
7:00 pm. Come and join us!

The Sioux Valley Music Masters are looking forward to an exciting year! The Music Masters mission is to support the
Music programs of Sioux Valley High School and Middle School. If your child participates in these Music programs, you are
officially a member of the Music Masters! Projects supported by the Music Masters include: Feast & Fest, SV Marching Band
Festival, All- State Choir & Band participants, fundraising to provide needed equipment for the SV Music Program, coordinate
planning and fundraising for a music student performance trip scheduled every four years, scholarships for high school senior music
students, and MORE!

BASE (Before and After School Enrichment)

The Sioux Valley BASE program serves children residing in the Sioux Valley School District in grades K-5 by providing
quality care during out of school time hours. While at BASE, children will have the opportunity to participate in activities in a safe
and supervised environment.
The BASE program opens at 7:15 in the morning and takes the children to the lunch room for breakfast before school
begins. We then open after school at 3:30 for after school care. The BASE program closes every night at 5:45. Full day
programming will be available by sign-up for teacher in-service days and school holidays, with the exception of a few holidays noted
in the parent policy manual.
Program fees are either a full-time (AM and PM) rate of $32.50 per week, a part-time (PM only) rate of $22.50 per week, or
a drop-in rate which is a minimum of $12.50 per week per child. (Rates are subject to change)

Sioux Valley Alumni & Friends Foundation
The mission of the SV Alumni & Friends Foundation is to promote increased academic and co-curricular opportunities at Sioux
Valley Schools through an organized network of friends and alumni representing the Bruce, Sinai and Volga communities. We invite
new members and encourage anyone who is interested in helping with the many projects we assist with to attend a meeting. A few of
SV Alumni & Friends most recent projects include collaborating with the SV PTO to successfully raise funds for new playground
equipment; donating funds to various SV groups over the past few years; sponsoring the annual SV Distinguished Alumni award and
recognizing past alumni royalty at SV Coronation; providing scholarships to SV Seniors and planning the All-School Reunion which
was last held in June 2012. This year our big project is a fundraiser for the new Sioux Valley Performing Arts Center. Please refer to

The Family Educational Rights and Privacy Act (FERPA) affords parents and students over 18 years of age certain rights with respect to the student's
education records. These rights are:
1. The right to inspect and review the student's education records within 45 days of the day the School receives a request for access. Parents
or eligible students should submit to the School principal [or appropriate school official] a written request that identifies the record(s) they
wish to inspect. The School official will make arrangements for access and notify the parent or eligible student of the time and place where
the records may be inspected.
2. The right to request the amendment of the student's education records that the parent or eligible student believes are inaccurate, misleading,
or otherwise in violation of the student's privacy rights under FERPA. Parents or eligible students who wish to ask the School to amend a
record should write the School principal [or appropriate school official], clearly identify the part of the record they want changed, and
specify why it should be changed. If the School decides not to amend the record as requested by the parent or eligible student, the School
will notify the parent or eligible student of the decision and advise them of their right to a hearing regarding the request for amendment.
Additional information regarding the hearing procedures will be provided to the parent or eligible student when notified of the right to a
hearing.
3. The right to provide written consent before the school discloses personally identifiable information (PII) from the students education
records, except to the extent that FERPA authorizes disclosure without consent. One exception, which permits disclosure without consent,
is disclosure to school officials with legitimate educational interests. A school official is a person employed by the school as an
administrator, supervisor, instructor, or support staff member (including health or medical staff and law enforcement unit personnel) or a
person serving on the school board. A school official also may include a volunteer or contractor outside of the school who performs an
institutional service of function for which the school would otherwise use its own employees and who is under the direct control of the
school with respect to the use and maintenance of PII from education records, such as an attorney, auditor, medical consultant, or therapist;
a parent or student volunteering to serve on an official committee, such as a disciplinary or grievance committee; or a parent, student, or
other volunteer assisting another school official in performing his or her tasks. A school official has a legitimate education interest if the
official needs to review an education record in order to fulfill his or her professional responsibility.
4. The right to file a complaint with the U.S. Department of Education concerning alleged failures by the School to comply with the
requirements of FERPA. The name and address of the Office that administers FERPA are: Family Policy Compliance Office U.S.
Department of Education 400 Maryland Avenue, SW, Washington, DC 20202.
5. The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that Sioux Valley Schools, with certain exceptions,
obtain your written consent prior to the disclosure of personally identifiable information from your child's education records. However
Sioux Valley Schools may disclose appropriately designated "directory information" without written consent, unless you have advised the
District to the contrary in accordance with District procedures. The primary purpose of directory information is to allow the Sioux Valley
Schools to include this type of information from your child's education records in certain school publications. Examples include:
A playbill, showing your student's role in a drama production;
The annual yearbook;
Honor roll or other recognition lists;
Graduation programs; and
Sports activity sheets, such as for wrestling, showing weight and height of team members.
Directory information, which is information that is generally not considered harmful or an invasion of privacy if released, can also be disclosed
to outside organizations without a parent's prior written consent. Outside organizations include, but are not limited to, companies that
manufacture class rings or publish yearbooks. In addition, two federal laws require local educational agencies (LEAs) receiving assistance under
the Elementary and Secondary Education Act of 1965 (ESEA) to provide military recruiters, upon request, with the following information
names, addresses and telephone listings unless parents have advised the LEA that they do not want their student's information disclosed
without their prior written consent.
If you do not want Sioux Valley Schools to disclose directory information from your child's education records without your prior written consent, you must notify
the District in writing by November 1, 2014. Sioux Valley Schools has designated the following information as directory information:
Student's name
Address
Telephone listing
Electronic mail address
Photograph
Date and place of birth
Grade level
Participation in officially recognized activities and sports
Weight and height of members of athletic teams
Degrees, honors, and awards received
Student ID number, user ID, or other unique personal identifier used to communicate in electronic systems that cannot be used to access education
records without a PIN, password, etc. (A student's SSN, in whole or in part, cannot be used for this purpose.)
