added or removed from our network after this directory was printed. We do not guarantee

Kaiser Permanente is an HMO plan with a Medicare contract. Enrollment in Kaiser

Permanente depends on contract renewal.

brochure, but the continued participation of any one doctor, hospital or other provider cannot
be guaranteed.

SECTION 1 INTRODUCTION
This directory provides a list of Kaiser
Permanente Senior Advantages network
providers. To get detailed information about your
health care coverage, please see your Evidence
of Coverage.
You will have to choose one of our network
providers who are listed in this directory to be
your Primary Care Physician (PCP). The term
PCP will be used throughout this directory.
Generally, you must get your health care
coverage from your PCP.
PCPs include:
Doctors in Family Medicine provide
continuing care for individuals and
familiesfrom newborns to the elderly
including sports medicine.
Doctors in Internal Medicine specialize in
adult medicine, with a focus on preventing
and treating complex adult conditions and
caring for the elderly.
Doctors in Pediatrics care for children from
birth to 18 years old.
Please note: Female members may elect to see
an OB/Gyn for their routine health maintenance
exams, while seeing their designated PCP if nonfemale specific problems arise.
The network providers listed in this directory
have agreed to provide you with your health care
coverage. You may go to any of our network
providers listed in this directory; however some
services may require a referral. If you have been
going to one network provider, you are not
required to continue going to that same provider.
In some cases, you may get covered services
from non-network providers. Your PCP may also
coordinate the rest of your care and may refer
you to a specialist when needed. You can also
consult with most of our specialists
without a referral. You may also self refer
(schedule an appointment without a physician

referral) for consultation or routine visits to most
network and Kaiser Permanente specialists.
A self-referral covers routine office visits and
office-based services. You will still need a
physicians referral and written authorization
from Kaiser Permanente if you need additional
services not provided in the doctors office,
such as surgery, MRI, CAT scan, colonoscopy,
or hospital admissions. Consult this Directory
for department listings and telephone numbers
at the various medical offices (pages 9-29) or to
get the most-up-to date information on which
specialty physicians are eligible for self referrals,
call Member Services toll free at
1-800-476-2167, seven days a week, 8 a.m. to
8 p.m. TTY users should call 711.
The acupuncture, chiropractic, and hearing aid
providers listed in Section 2 are covered by
the Senior Advantage Plus Choice (HMO-POS)
plan and Advantage Plus optional supplemental
benefits packages and may not be a part of
the Senior Advantage Core, Silver, and Gold
plan provider network. The listing information
is subject to change. For the most updated
information, please call Member Services toll
free at 1-800-476-2167 / TTY: 711, 8 a.m. to
8 p.m., seven days a week. When calling to
schedule services, please inform the provider if
you have Kaiser Permanente Senior Advantage
Plus Choice (HMO-POS) or Advantage Plus.
If you receive care from an out-of-network
provider and they ask you to pay for services
at the time of service, its best to ask an out-ofnetwork provider to bill us first. If you receive a
bill from an out-of-network provider, you should
not pay the bill and instead send it to our Claims
Department for processing and determination of
your cost-sharing amount, if any.

Our payment for covered services is based upon
charges that we determine to be reasonable in
accord with Medicare payment guidelines.
Emergency care can always be obtained in or
out of the service area from the nearest available
provider. When in the service area, you must use
plan providers for urgent care. When out of the
service area, urgent care may be obtained from
the first available provider. In addition, when
out of the service area, you can obtain dialysis
treatment from any qualified dialysis provider.

Permanente Senior Advantage?
The map is a geographic overview of our Denver
Metropolitan service area and is for illustrative
purposes only. Since some counties extend
outside the actual limit of our service area, please
refer to the list of ZIP codes below to confirm that
your residence is within our Denver Metropolitan
service area.
The counties and parts of counties in our service

You must use plan providers except in emergency

or urgent care situations, for out-of-area renal
dialysis, for care that has been approved by
us in advance, or for services covered under
the Kaiser Permanente Senior Advantage Plus

benefit. If you obtain routine care from out-ofnetwork providers, neither Medicare nor Kaiser
Permanente Senior Advantage will be responsible
for the costs.
