Bring ONE ORIGINAL of the following to prove you live in
Washington, DC Document Must Include Pay Stub Name and current DC home address DC tax withholding for current tax year Dated within the past 45 days Financial Assistance from the DC Government (Ex: TANF, Medicaid, DC Alliance) Name and current DC home address Dated within the past 12 months Supplemental Security Income Annual Benefits Name and current DC home address Dated within the past 12 months Tax Information Authorization Waiver Form (Form D40) Name and current DC home address Certified by DC Office of Tax and Revenue Evidence of payment of DC taxes for prior tax year Embassy Letter Name and embassy address Dated after April 1 of current school year Proof of living on embassy property Official embassy seal Military Housing Orders Name and current DC home address Proof the child is a ward of DC Name and current DC home address Court order or official documentation from DC Child and Family Services Agency OR bring TWO ORIGINALS of the following items to prove you live in Washington, DC Unexpired DC Motor Vehicle Registration Name and current DC home address Unexpired Lease or Rental Agreement Name and current DC home address Receipts for payment or cancelled check within 2 months Unexpired DC Motor Vehicle Operators Permit Name and current DC home address One Utility Bill (Only gas, electric, and water bills are acceptable) Name and current DC home address Receipt of payment or cancelled check from within 2 months Open Enrollment for New Applicants Fall 2014 The Carlos Rosario School will be accepting student applications for the Fall semester lottery during open enrollment.
