NEW VENUE SCHEME (FETCH & SEND SERVICE)

APPLICATION FORM
I agree to accept the following rules and regulations in addition to item 6 and 9 in the Terms and
Conditions of Learners Course Agreement as well as any additions/amendments from time to
time.

1. I am aware that a Venue surcharge of $2.14 and Fix Group Surcharge of $3.21 per session
(inclusive of GST) shall be deducted upfront upon booking confirmation. Surcharges are non-
refundable or non-transferable if absent for any training session due to whatsoever reasons.

2. I am aware that the Centre has the right to replace or appoint another instructor(s) to
Learners Fixed Group instructors (Group of 6 to 8 instructors, including reserves). The Fixed
Groups surcharge will still be payable.

3. I am aware that I should block book the practical lessons in advance so as to avoid the waiting
period of 2 weeks should I make last-minute bookings.

4. I will report and end all my practical lessons at my selected venue/pick-up point (an
approved designated pointed determined by me) stated below, except for AT 1.01, theory
training, driving simulator, theory test, practical test and any other lessons determined by
the Centre in due course.

5. The instructor will drive to the pick-up point (as stated below) to start the lesson and will end
the lesson at the same point. The travelling time from the venue to my pick-up point and
vice versa shall be within my training hours. The travelling time would depend on the traffic,
weather conditions and other factors such as accident and vehicle breakdown.

6. I am aware that in the event whereby I am unable to locate the training vehicle at the
selected venue/pick-up point on the booked date and time, I have to contact the Centre
immediately at 1800 666 8888 and 6594 3526 (before and after the Centres Customer
Service Operating hours) for assistance.

7. An administrative fee of $10.70 (inclusive of GST) shall be imposed for:
Cancellation/termination of this scheme
Changing of pick-up point

8. There may be changes to the above surcharge/administrative fee from time to time without
prior notice.
