DECLARATION CUM UNDERTAKING LETTER

I, ______(RSE Name)_________________, S/o. ____(RSE father’s name)____________________,

aged about _____ yrs, residing at __________________________ and am working as the Key
Accounts Manager with you on deputation from Adecco __(full name of
Adecco)___________________.

I have been entrusted by you with the motorcycle bearing registration No.___________
(“Vehicle”) as authorized distributor of Unitech Wireless (Tamilnadu) Private Limited. I
am fully aware that Unitech Wireless (Tamilnadu) Private Limited is the registered owner
of the Vehicle entrusted to me and I undertake to fully comply with the following
guidelines in respect to the said Vehicle:

1) I shall always carry with me the valid driving license along with the copy of RC book
& Insurance policy and shall produce the same whenever the same is sought by the
authorities.
2) I undertake to strictly adhere to the following guidelines:-
a) Not to drive the Vehicle without wearing helmet.
b) Not to drive the Vehicle after consumption of alcohol or in any intoxicating
state.
c) Not to carry pillion driver on the Vehicle without helmet.
d) Not to use the Vehicle for personal business or usage.
e) Not to permit others to use the Vehicle entrusted to me.
f) Not to entrust the Vehicle to friends & family.
g) Not to drive the Vehicle in a rash manner.
h) Not to violate the traffic rules & guidelines.
i) Not to use the Vehicle for unlawful or illegal activities.
j) Not to cause disrespect to the brand “UNINOR” on the Vehicle.

I am fully aware of the risk/exposure associated in using the Vehicle and am taking the
Vehicle on my own volition only for use by me.

I shall be fully responsible and accountable for any breach or violation of the aforesaid
guidelines and traffic rules.

I am fully aware that any breach committed by me will affect the brand image of
“UNINOR” and I shall strictly adhere to the guidelines issued to me and as well as city
traffic rules.

Neither me nor my family or any other person claiming through me will have any claims
against you or UNINOR in the event of an accident caused by me or to me while using
the Vehicle and I have clearly explained to my legal heirs/family on the risk associated in
using the Vehicle and about this declaration.

I undertake to forthwith return the Vehicle, in the original condition (except normal wear
and tear) in the event of my resignation or termination of my employment without any
condition or protest or whenever I am asked for by you or UNINOR or am intimated by
you or UNINOR to return the Vehicle.

I also understand and declare that nothing contained herein creates any right or title in
my favour in respect to the said Vehicle and that you and/or UNINOR either through itself
or through any person claiming under them shall reserve the right to take back the said
Vehicle at point of time without any prior notice.

I request you to entrust the Vehicle to me on this undertaking to strictly adhere to the
guidelines and traffic rules.
