DISCLOSURE REGARDING BACKGROUND INVESTIGATION

As an applicant for a temporary placement assignment at Great-West Life & Annuity Insurance Company or one of its affiliates
(collectively "Great-West"), through Corestaff Services, you are a consumer with rights under the Fair Credit Reporting Act.
Corestaff Services may choose to consider information about you contained in either a consumer report or an investigative
consumer report obtained from a consumer reporting agency when: 1) considering your application for employment, 2) making a
decision whether to offer you a temporary assignment at Great-West, 3) assuming you are hired, deciding whether to continue your
employment, 4) reviewing any applications for FINRA securities registration with one or more of Great-Wests affiliate broker
dealers or investment advisors that you may make, or 5) making other employment-related decisions affecting you.
A "consumer reporting agency" is a person or business which, for monetary fees, dues, or on a cooperative non-profit basis,
regularly assembles or evaluates consumer credit information or other information on consumers for the purpose of furnishing
consumer reports to others, such as Corestaff Services.
Please be advised that the nature and scope of the most common form of investigative consumer report obtained with regard to
application for employment is an investigation into your education and/or employment history conducted by Employment
Background Investigations, Inc (EBI), P.O. Box 629, Owings Mills, MD 21117, 1-800-324-7700. These reports may be obtained at any
time after receipt of your authorization and, if you are hired, throughout your employment. You have the right, upon written
request made within a reasonable time after receipt of this notice, to request disclosure of the nature and scope of the investigative
consumer report and a Summary of Your Rights Under the Fair Credit Reporting Act. The scope of this notice and authorization is all-
encompassing, however, allowing Great-West to obtain from EBI all manner of consumer reports and investigative consumer reports
now and, if you are hired, throughout the course of your employment to the extent permitted by law. As a result, you should
carefully consider whether to exercise your right to request disclosure of the nature and scope of any investigative consumer report.

ACKNOWLEDGMENT AND AUTHORIZATION

I acknowledge receipt of the NOTICE REGARDING BACKGROUND INVESTIGATION and A SUMMARY OF YOUR RIGHTS UNDER THE
FAIR CREDIT REPORTING ACT. I hereby authorize the obtaining of consumer reports and/or investigative consumer reports at
any time after receipt of this authorization and, if I am hired, throughout my employment. To this end, I hereby authorize, without
reservation, any law enforcement agency, administrator, local, state or federal agency, institution, school or university (public or
private), information service bureau, employer, or insurance company to furnish any and all background information (including, but
not limited to, driving and/or motor vehicle records, transcripts, grades and attendance records, employment history, salary
information and references, workers compensation reports in Pennsylvania, Arizona and in all other states, drug and alcohol testing
results) requested by EBI acting on behalf of Employer, and/or Employer itself. I agree that a facsimile (fax) or photographic copy
of this Authorization shall be as valid as the original.

California, Minnesota and Oklahoma applicants or employees only: Please check the box if you would like to receive a copy of a
consumer report if one is obtained by the Company.

New York applicants or employees only: By signing below, you also acknowledge receipt of Article 23-A of the New York Corrections
Law.
