reliable persons form my belief that J erome Allen, ,:<eldingthe defendant committed the crime charged.

On Tuesday, September 9, 2014, 1515 hours I was dispatched to the Dollar General Store in Sac City for a report of a subject who
attempted to start a fight with another in the business. On the scene, I spoke with both parties involved and the manager of the store.
The rnanaqer advised that the defendant walked into the store and got into face of the victim and said, "You want a piece of me
asshole?" J erome was placed under arrest and transported to the Sac County Law Enforcement Center where he was charged with
Disorderly Conduct and jailed.

simple misdemeanor and conviction carries a maximum penalty of $625.00 Fine plus 35% Surcharge
