ThisarticleaboutspecialcaredentistryinthemiddleyearsconsiderspeoplewhohaveDownssyndromeandcerebral
palsyandthosewhohavecardiacandrespiratorydisease.TheincreasedlifeexpectancyofpeoplewithDownssyndrome,
currently50-60years,isreectedinthechangingpopulationproleandneedsoftheseindividuals.Thepreventiveand
dentaltreatmentofmostpeoplewithDownssyndromeandcerebralpalsycanbemetingeneraldentalpractice.However,
thosepeoplewithprofounddisability,anxietyorlearningdisabilitymayrequireeitherasharedapproachtocareorreferral
forspecialistcare.Cardiacandrespiratorydiseaseoccurcommonlyinthegeneralpopulationbothinmiddleandolderage
groupsandthedentalteamwillmeetincreasingnumbersofpeoplewiththeseconditions.Theproceduresanddrugsused
indentistrycanaggravateheartdiseaseanditisimportantthatthedentalteamareawareofthecommoncardiaccondi-
tionsandtheirmanagement,aswellashowtobestmanagetheoralcareofthisgroup.Also,theyhavearoletoplayin
theprovisionoforalhealthadvice,smokingcessationanddietaryadvice.Thisisparticularlyimportantaspoororalhygiene
hasbeenlinkedtorespiratorypathogencolonisationanddentalplaquemayactasareservoirforaspirationpneumoniain
susceptibleindividuals.
