in the Lisle Area Chamber 5K Run/Walk,10K Run, Youth Run, Tot Run.

I am physically fit and sufficiently trained to participate in this running
event. I hereby grant my consent to any of the above named parties
and their authorized agents to use my name and any photographs,
video tapes or recordings of my participation in this event for any
lawful purposes without compensation to me or my behalf. Entry will

winding through slightly hilly paths of the
Lisle Community Park.

The race will be held at Lisle Park Districts Community
Park, 1825 Short Street. Located one mile South of
I-88, 2 miles West of I-355. Just West of the intersection
of Route 53 & Short Street (South of Ogden Avenue).
FREE Parking is available at Lisle Community Park.
