Social Action Project Template:

The following chart should appear on the first/cover page of the social action project report.
Name of Organisation
Contact Person
Phone Number
Cell Number
Email Address
Date of Submission

Issue: Briefly describe the issue making sure to mention where (precise demographic location) it is
spread, and the underlying causes of it.
Initiative description: Describe the parameters of the project. Include all logistical details such as
what cluster forms the target community (beneficiaries), which areas of the city/province the project
will span over, and what strategies it will make use of to identify beneficiaries.
Research/Rationale: This section must consolidate all data that justifies investing time and money in
a project. It must state all issues that put the project into perspective, show the urgency which the
concerned work/project needs to be carried out with, the number of people currently adversely
affected by the problem, how many will be benefitted. Alternatively, how many people will get rid of
the problem after the successful execution of the project.
Goals/Milestones: Sub divide the project into smaller compartments and obligatorily, set deadlines
for each phase/facet/part.
Timetable: Correspond the section above with this one and comprehensively divide the project into
subtasks, or phases, and match these with dates by which they must be completed for the project as
a whole to be completed within assigned timeframe.
Personnel: If you have not covered division of labour in the previous two sections, set out the roles
and responsibilities of each participant in this section, and/or assign them parts. Each participant
would have an area of expertise and it becomes easier if charges of areas such as communication
and rapport or transport are divided accordingly.
Resources/Budget: This is a very important section in organisational terms and is paramount for
garnering funding. Make a table of project expenses and one of funding sources that corresponds.
Both tables must show the same amount unless surplus funding is set to be sent back to
donors/contributors.
Challenges: In correspondence with the research and rationale section, set out prominent hurdles
that may hinder the smooth execution of the project. This list of challenges must be comprehensive
in the sense that it must mention social, logistical, financial roots and context of impediments.
Documentation: Organisations supporting the project in question will almost definitely ask to see a
documented and inclusive report which covers all areas of the multiple aspects the project
encountered at different stages. At each phase, progression, or after an even smaller stage of the
project is complete, someone from the participants must document what was done, what difficulties
presented themselves during what was to be executed, which one of these had not been
anticipated, whether the phase was completed on time, and any noteworthy/praiseworthy progress.
Publicity: During or after the completion of a successful project, it is paramount that success is
shared and positive awareness spread. Devote this section towards mentioning how you will
publicise your concept and share successful project execution with the community in question.
