Required Documents
1. Photocopy of any two (2) valid IDs with photo and signature:
SSS Photocard
TIN ID Card
Passport
Driver's License
Company ID
2. Proof of income
If Employed, you are required to submit any two (2) of the following:
Latest Income tax return (ITR) duly stamped as received by the BIR
W2 (BIR Form 2316)
Latest payslip (within last 3 months)
Original Certificate of Employment
Photocopy of your most recent credit card billing statement from other banks (if any)
If self-employed with assets less than P15 million, you are required to submit one (1) of the following:
Latest Audited Financial Statement (AFS) duly stamped as received by the BIR
Latest Income Tax Return (ITR) duly stamped as received by the BIR
Photocopy of your most recent credit card billing statement from other banks (if any)
If self-employed with assets greater than P15 million, you are required to submit all of the following:
Latest Audited Financial Statement (AFS) with BIR or bank stamp
Latest Income Tax Return (ITR) with BIR or bank stamp (2 pages)
If you are a foreign national, you required to submit any one (1) of the following:
VISA and Working Permit (with validity of 1 year and not expired at time of application)
Immigration Certificate of Residence (ICR)
Alien Certificate of Registration (ACR - iCard type with permanent status)

1. Philippine and Foreign Government-issued passport (for passports without signature, the second ID should bear a signature)
2. Drivers License
3. Alien Certificate of Registration or Immigration Certificate of Registration or any similar visa certificate issued by a recognized
foreign country
4. Social Security System (SSS) photo card type
5. Tax Identification Number (TIN) photo card type
6. Government Security Insurance System (GSIS) e-Card
7. Senior citizen photo-ID
8. Photo ID's issued by a government owned or controlled corporation, department, agency or instrumentality of the national
government including Armed Forces, Philippine National Police, Department of Foreign Affairs, Executive, Legislative,
Judiciary, Senate, Philippine Overseas and Employment Agency, Home Development Mutual Fund, Civil Service Commission,
Professional Regulations Commission, Philippine Postal Service, Philippine Veterans Affairs Office.
9. National Bureau of Investigation or Philippine National Police clearance with photo
10. Company ID of any entity regulated by the Bangko Sentral ng Pilipinas (BSP) or Insurance Commission (IC) or company ID of
a publicly-listed company or belonging to top 2000 companies operating in the Philippines that are regulated by the Securities
& Exchange Commission (SEC)
11. Unified Multi-Purpose Identification (UMID)
12. Special Resident Investors Visa (SRIV)
13. Special Resident Retirement Visa (SRRV)
