Tips and Talk from the Inside

A recent industry survey of in-house hospital recruiters showed that more than 10% of
physician candidates are hired through medical recruiting firms. With this in mind I
thought it might be worth exploring the things a recruiting agent looks for in a star
candidate.

There are literally hundreds of mainstream recruiting firms that specialize in healthcare. And within this group are
two typescontingency and retained. The term head hunter was originally coined to describe an employers agent
working on retainer on a specific project. They are paid a fee regardless of the outcome of the search. Their fees are
usually considerably higher than for their contingency brethren and they often focus on one project at a time, giving
them greater focus and client access.

In most cases a retained firm does not accept unsolicited resumes. It is the job of the recruiter to reach out to highly
qualified candidates who meet all of the clients criteria. Most practicing physicians have received calls from head
hunters seeking a referral for an active search in which they are engaged. Both parties are aware that the true
motivation is to recruit the physician at the other end of the line.

As a general rule, the retained recruiter has already researched potential targets and the recipient of this type of call
is probably well qualified for the opportunity. But there will still be criteria that the recruiter will be evaluating as
the process moves forwardif at all. Here is a partial list of criteria in no particular order:
Personal appearance these days, your photo is probably easily available online through social media. We have
talked in the past about the importance of maintaining a professional image at all times online.
Communication skills a sharp recruiter will do far more listening than talking. They will be evaluating your
ability to express yourself in a sophisticated manner. Delivery at this early stage is almost as important as content.
Ability to handle difficult, open ended questions If the recruiting dance goes beyond the initial phone call
the recruiter will want to see how well you can handle touch interview questions.
Flexibility A star candidate will be flexible on salary, relocation and a host of other issues. A recruiter always is
encouraged when they receive a reasonable response to a trial question that keeps doors open. A response such as,
Under the right conditions I might be willing to consider an offer with some of those elements. But I would need to
hear more to make a definite commitment.
Seriousness No one wants to waste their time. A recruiter wants to be sure that a potential candidate is serious
about a potential opportunity before investing time and money in the pursuit.
Sophistication A recruiter wants to work with a candidate that understands how the game is played and
understands the role that each party in the drama plays. A mature approach and respect for the others in the
process can go a long way to moving the process forward.

These are but a few of the things a veteran recruiter will look for. At the contingency level, these factors can be even
more important because the recruiter may be working with multiple employers and candidates. They will be looking
at each candidate from the perspective of the likely hood of earning a commission.

Tips & Tools for Todays Physician

Today Id like to speak directly to young physicians just out of residency or
fellowship. Congratulations, you have just moved out of slavery and into the
working poor. Working poor? you say. You must be nuts. I just signed a
six figure three year contract and my income just tripled. Ok. Let me ask
you a question and you can answer as honestly as you wish. Are you
presently saving, consistently, at least 25% of every pay check? Im talking 25% of your after tax earnings.

If you are able to answer yes, than you can pat yourself on the back because you are in the top 5% of young
physicians as far as savings go. What many young physicians forget is that you have two big disadvantages to
overcome. One, it took you at least seven years longer than your college contemporaries to begin earning. Im not
counting any stipends you may have had. They were meant to keep your cupboard filled with soup, a tenement roof
over your head and a half tank of gas in your car.

The second problem is medical education debt. And that isnt going away overnight. So you dont have as much to
work with as you think and you dont have as much time to save for college education for the kids and a comfortable
retirement as you would like. You need to set aside 25% or more of that pay check to have any shot at the future that
you dreamed of in med school. Today Im just going to touch on some of the major mistakes that you may be
making that are keeping you from meeting your savings goals.

1. If you havent already overspent on your first house you still have time to get some sage advice. Check some
of my precious columns on this topic. Buy what you can affordnot what you would like.
2. If you havent already employed a top notch financial advisor and put together a water-tight budget and
financial plan dont make another single financial decision until you do. Think of your pay check as a sieve
and your budget as the cement that you use to fill the holes.
3. Your car is falling apart. You need a new wardrobe. You havent had a vacation in ten years. Your spouse
has supported you throughout your training and deserves some rewards. And you are working hard on
starting your family. Each of these represents a spending trap that can keep you from meeting your savings
goals. Reward yourself and your spouse but do it after you bank your 25%.
4. If you believe that your new social status means that it is beneath you to pay anything but full sticker price
for all that you purchaseyou are wrong. Think back to those days when you negotiated for EVERYTHING.
Never pay full price and put the savings in the cookie jar.
5. This may be a tough one to accept, but as the famous comedian once said, The wise man supports his
spouse in the manner to which she has become accustomedlet her keep her job. You can reverse the
gender in all parts of that statement but the message is the same. Until you are well established in your
career and the money is truly rolling in, a two income family has a brighter future than one.
6. Credit cards are the enemy. Nuff said.
7. If you think you can beat the market than you should be working on Wall Street not in the operating room.
Let a professional handle your investments and stick to what you are trained to do. A fool and his money
8. If you think you know how much in savings you will need to live at your current level after you retire, you are
probably off by a country mile. Have your financial advisor run the numbers and you will be shocked.
9. Dont count on social security. It wont keep the pets fed.
10. As a physician you are truly blessed. Dont forget to give back some of what you receive.

Thats the sermon for the day. Remember, 25% of what you earn is yours to keep. Pay yourself first.

Heal Physician Burnout Overnight with the Treasure Hunt Technique

In this article you will learn to recognize when stress and overwhelm have driven you into Survival
Mode and the Treasure Hunt Technique to turn that around overnight.

There is a tipping point when doctors are under stress ... it happens silently, slowly and when you
pass through this threshold your downward spiral will frequently accelerate.

I am talking about when you drop into Survival Mode. When you unconsciously begin to focus
your energy 100% on simply getting through the day and dragging your exhausted butt home.

How do you know you are in Survival Mode?

It is when you start to see the majority of your job duties as a hassle. You begin to feel like the patients, your nurse,
the lab reports, the phone calls ... they are all out to get you. You feel you have no control over what happens in your
day ... you are swinging in the breeze of everyone else's needs and urgency.

Your entire focus is to get through your patients and out of there as quickly as possible. You are focused on surviving
your practice day, dreaming of the moment when you can put your key in the ignition, your foot on the gas and get
out of there.

In survival mode you are blind to the positive things in your day because even a positive patient encounter gets
between you and getting home on time.

This is also a time when Compassion Fatigue kicks in and you may find yourself being increasingly sarcastic and
cynical - especially under your breath or in your inner dialog. I have even seen doctors in survival mode post internet
chat comments about how they would like to use a nail gun on patients or light them on fire - its true.

What has happened in psychological terms is you have lost your Inner locus of control. You have assumed the role
of the victim ... helplessly at the mercy of forces that you cannot control. This change is usually gradual and takes
place when your energetic bank accounts drop into a negative balance. You simply dont have the energy to be
proactive and fight the good fight any more.

How can you turn Survival Mode around?

Here is a simple, powerful process to take back your locus of control and put yourself back in charge of your daily
experience. This is one of the core tools I teach every one of my one on one coaching clients because it keeps your
awareness on why you became a doctor in the first place ... in just a couple minutes a day. It can radically alter your
experience of your practice overnight and works every time you use it.

The Treasure Hunt allows you to move from the unconscious intention to simply survive the day ... to a conscious
intention to have at least one meaningful encounter at work every time you are in the office or hospital. But it is
much more powerful than that last sentence can convey.

Before I show you the process, just imagine for a moment ... in your next day in the office or the hospital, you are
consciously looking for a satisfying, enjoyable, fun encounter with a patient or staff member ... just one. You are
open to having a meaningful encounter and on the lookout for its appearance.

When one of my coaching clients decided to switch from Survival Mode to this conscious intention to look for a
spark in his day ... he said that would be like turning the day into "a Treasure Hunt."

This change in stance from unconscious victim, hassled on all sides, to actively seeking out meaning and connection
makes a night and day difference in your attitude. You get your locus of control back. The control over your
awareness (on the lookout for good things) and your emotions - curious and engaged rather than defensive and
hassled.

2 steps to the treasure hunt

1) Define your treasure

Take a piece of paper or open a new document and write down the last meaningful patient encounter you remember.
That interaction where afterwards you said, "Oh yeah, THAT right there is why I became a doctor" with a smile on
your face.

When you are done writing it down ... use as much detail as you can remember ... read it back to yourself and focus
on how it felt. Where did you notice that feeling of satisfaction in your body? Invite that feeling to become even more
clear in your awareness so you recognize it easily.

This is your Treasure. This feeling is what you are looking for in your days when you take step two.

2) Start the hunt

Before you go in for your next shift, open your journal and write down your intention to be on the lookout for this
kind of a meaningful encounter and this feeling of satisfaction today. Writing it down takes it from a desire in your
head into physical reality. The pen strokes on the paper reinforce your intention. They bring you back into control of
your experience and take you out of the unconscious grip of Survival Mode.

Your intention might be as simple as this, "I am open to and on the lookout for a satisfying interaction with a patient
or staff member today."

Write it, say it out loud, feel what it will feel like when you find that treasure, close your journal and head in to work.
Now watch what a difference this makes in your experience of the day.

Power Tips:

1) Release attachment

It is very important to release attachment. By that I mean don't get attached to actually having that encounter today.
Be on the lookout for it and don't get attached to whether it happens or not.

If you don't have that interaction today, set your intention again tomorrow. Here is why.

Just writing down your intention changes your focus and dramatically increases the chance of having that fun
interaction ... dramatically. You will never find treasure unless you are looking for it. The treasure is there when your
intention is on finding it.

2) Close the loop

Journal about your experience at the end of the day. Sit down with the same journal for just five minutes at the end
of your day and write down what happened around your intention today. How did it go? What treasure did you find
and how did that feel? What treasure will you look for tomorrow and how will it feel when you find it?

3) Make being intentional an everyday habit

A large part of recovering from physician burnout is taking back control over your life and being more intentional
about your actions and awareness. The Treasure Hunt process is just one example where you get clear on what you
want and then go get it.

I encourage you to set an intention every day, even your days off.

On your next day off, decide what your one intention is for today. Perhaps it's to be really present with your
spouse/significant other or children or really enjoy yoga class or take a nap in the sun and completely let your cares
fall away. When you set that intention, it is very likely to come true. All it takes is deciding what you want and
writing it down in the early part of your day and then being open to your wish coming true.

4) Enroll your team

Ask your team members what their favorite patient encounter in the last month was. You may not know what they
enjoy most about their job at this point. Tell them about the Treasure Hunt and encourage them to journal their
intention. Share your stories of treasure in your days and encourage them to share theirs. This simple change can
make a huge difference in your work environment for everyone on the team.

The Treasure Hunt triggers your turn around

As you read the description of Survival Mode above, did that feel familiar? The Treasure Hunt is one of the ways to
stop the downward spiral and begin to find peace, joy and fulfillment in your practice again. It does not make the
hassles go away - that is another topic for another day and there are hundreds of ways to improve your practice
efficiency. Today we are talking about your intention, your awareness and your experience of your day. This article is
about who you are being in your practice and your life.

Take the 5 day challenge

I know intention journaling will fundamentally change your experience of your days - on and off work - when you
take it on as a habit. I challenge you to take it on for your next five days at work and see what a difference it makes
for you. The steps are outlined above. I encourage you to release attachment and close the loop by journaling in the
evening too. Do this for just five days. Then you decide whether you want to make it a habit. Enjoy.


Real Estate

Investing in real estate has always been an attractive wealth building strategy for
physicians. And although the real estate market from the investors perspective has
moderated since 2007 when the housing market crash began. The combination of
historic low interest rates and property values that have dropped by 30 to 50% is a
marvelous combination. And although we may be at or even past the bottom, there is
value to be had if one keeps some basic principles in mind.

1. Buy the cheapest house on the block. This basic truism still applies. And for the sophisticated investor that
isnt afraid to consider distressed properties this strategy has even greater relevance. Purchasing a short
sale, foreclosure or distressed property for $75,000 in a neighborhood where most homes are priced at
$200,000 leaves a good deal of leverage to rehab the property and still retain a nice piece of equity. And
with strong credit it wont take much of a cash down-stroke to walk away with positive cash flow.
2. Consider a multi-unit property if you are just starting your medical career. Many young physicians coming
out of residency or fellowship with a mountain of medical education debt assume that they cant qualify for a
mortgage on a single family home let alone a multi-unit property. But the math can be both compelling and
manageable if you are interested in living in one of the units and renting out the others. This may not
represent your dream home near the golf course but it is not impossible to generate substantial equity and
live nearly for free if the deal is well struck. There will be more than enough time to move to the ninth
fairway.
3. If you are a principle in your medical practice and do not presently own your office or for that matter the
entire building, than you should sit down with your financial advisor and learn how to properly structure an
investment in your own space. There are few investments that make more sense or are easier to do. But
there are also some very real legal and tax issues that can jump up and bite you if you dont get the proper
advice.

This may well be the greatest investment environment of our time. Get good advice. Get in the game. Get set for a
secure retirement from the returns of a broad real estate investment portfolio.


3 Steps to Physician Retirement by Design

How do you truly thrive in your retirement years?

Lets define your working years as a time in which you build wealth; you work for your
money.

Retirement is the time in which your money works for you. You enjoy the financial
freedom to spend your time any way you wish.

While a life on the golf course is a nice fantasy, most physicians find it gets old very
quickly. Before long they miss the many rewards that our medical careers offer.

You can continue to engage in activities that give your life meaning in retirement. Here are three steps to retirement
by design.

Step #1: Identify whats important in your working years.

Score the importance of each part of yourself expressed during your working years.

(1 = not important; 5 = very important)



To thrive in retirement, you can create opportunities to express these parts of yourself.

Step #2: Identify whats missing in your working years.

Here are some parts of yourself that may not have been expressed as much as you would want. How important will
they be in retirement?



Let your guilt guide your retirement plans. If you regret missing special moments with your kids, set aside time to
invest in your relationship with your adult children and your grandchildren during retirement.

Step #3: Get creative.

Poet Mary Oliver asks, What is it you plan to do with your one wild and precious life? This includes the 20+ years
after you hang up your white coat.

You know physicians who have the financial freedom to retire, but continue to see patients because its what they
love to do.

Here are 10 activities that you could incorporate into your retirement plan:

1. Treat patients.

Volunteer at a local free clinic
o
Go on a medical mission
o
Get involved in telemedicine
o
Serve as a physicians on a cruise ship. You can get paid to cruise around the world!

2. Take on a leadership role in hospitals and clinics. Physicians executives are in demand.
3. Run for public office. Our country can use your experience as we make the transition to the Affordable Care Act.
4. Teach at medical schools, dental schools or nursing schools.
5. Mentor medical students and physicians in early stages of their careers.
6. Serve as an expert in medical malpractice lawsuits
7. Enroll in courses that youve always wanted to learn about, whether its learning to tango, grow a tomato or play
tennis.
8. Write a book.
9. Launch a speaking career.
10. Form a foundation and solve a problem that calls to you.

You have dedicated your life to serving others. In retirement you have a chance to serve on your own terms. You
can express your passions, control your calendar achieve life balance.
