INTRODUCTION
Discharge planning is conducted to plan for when a patient or resident
leaves a care setting. Health care professional(s) and the patient or
resident participate in discharge planning activities.

HOSPICES
If certain conditions are met, the Medicare Hospice benefit provides
Hospice services for the palliation and management of a patients
terminal illness and related conditions.

SWING BEDS
Swing beds are hospitals, as defined in Section 1861(e) of the Social
Security Act, or Critical Access Hospitals (CAH) with a Medicare provider
agreement that includes Centers for Medicare & Medicaid Services
approval to provide swing bed services, that may use their beds as
needed to provide the patient with either acute or Skilled Nursing
Facility-level care.
