1. Health Perception/ Health Management Pattern


Patient spo4 views health as a very important factor in his life. He thinks that being
healthy means that he will be able to attend to his familys needs, demands from his work, and
do his activities of daily living without interventions. He views sickness as a limiting factor of his
self that keeps him from doing all his normal activities. Whenever he is sick he usually take otc
drugs and goes to the doctor if he cannot tolerate it already

Prior to admission, patient spo4 usually rides his bicycle around Catarman then heads to his
work as his exercise. He usually helps in doing chores around the house such as fetching water
and scrubbing the floor. During his stay at the hospital, he usually walk around his room, do
some stretching exercises and talk to other patients as his activity.

5. Sleep-rest pattern
Prior to admission, patient usually sleeps for 6 hours from 9:00 pm to 3:00 am without any
disturbances. During admission, client find it difficult to sleep without any interruptions. He
usually slept for 3 hours from 11:00 pm to 2:00 am. He takes naps during noontimes for 2 hours
from 1:00 pm to 2:00 pm
6. Cognitive pattern
Patient has a decreased hearing ability that was observed during assessment . Patient also has
difficulty reading small letters from 1 foot apart. He is oriented to time, place, and person and
responds appropriately to verbal physical stimuli. Patient was able to count recite the numbers
spoken to him and was able to subtract 3 from 100. Patient is still able to remember his past
experiences.
7. Self -perception/ self- pattern

Before patient experienced his illness he thought that he is very healthy and never thought that
someday he would have any serious illnesses. During his illness patient expressed worry about
his self and condition. He refuses the care of the health care team because he does not feel any
better. He wants to consult higher centers in Manila. He wants to be free from illness as soon as
posiible in order for him to return to his normal daily functions.

8. Role/ relationship pattern


Patient had a wife and 2 children but eventually separated. He married another woman and had
5 children. All of his children already have their own families but they still keep visiting the
patient and his wife once in a while. Patient is friendly and loves to talk to other people, he
usually hang out with his co-workers and drink alcohol.
9. Sexuality-Reproductive pattern
Patient stated that he goes to yo! Biz with his boss and other co-workers whenever their
needs arises. He and his wifecannot fully attend to their sexual needs because his wife thinks
that they are too old to do so. He has 7 children in total, 2 from his first wife and 5 from his
current spouse.
10. Coping-tolerance pattern
Patient stated that whenever he has a problem, he would sometimes drink alcohol with his
friends or co-workers. He is positive about life and expresses willingness in facing his problems,
11. Value-Belief pattern
Patient is a Roman Catholic while his wife is an Eglesia ni Cristo. He does not go to church often
but attends the mass during special occasions. Client used to be an Eglesia ni Cristo but
converted to Roman Catholic because he thinks that Eglesia ni Cristo is too uptight while Roman
Catholic has freedoms. He goes to cockfights right after attending the mass
