The immune system has developed to respond to and neutralize pathogenic micro-organisms
as well as coordinate tissue repair. The inflammatory response to injury or infection involves
cell signaling, cell migration, and mediator release. Minor host insults instigate a local
inflammatory response that is transient and in most cases beneficial. Major host insults may
propagate reactions that can become amplified, resulting in systemic inflammation and
potentially detrimental responses. This topic is highly relevant because systemic
inflammation is a central feature1 of both sepsis and severe trauma. Understanding the
complex pathways that regulate local and systemic inflammation is necessary to develop
therapies to intervene during overwhelming sepsis or after severe injury. Sepsis, defined by a
systemic inflammatory response to infection, is a disease process with an increasing
incidence of over 900,000 cases per year. Trauma is the leading cause of mortality and
morbidity for individuals under 50 years of age.
This chapter reviews the autonomic, cellular, and hormonal responses to injury. These facets
of the inflammatory response to injury and infection are discussed in reference to the specific
response being considered.
Systemic Inflammatory Response Syndrome
The systemic inflammatory response syndrome (SIRS) is characterized by a sequence of host
phenotypic and metabolic responses to systemic inflammation that includes changes in heart
rate, respiratory rate, blood pressure, temperature regulation, and immune cell activation
(Table 2-1). The systemic inflammatory response includes two general phases: (1) an acute
proinflammatory state resulting from innate immune system recognition of ligands, and (2)
an anti-inflammatory phase that may serve to modulate the proinflammatory phase. Under
normal circumstances, these coordinated responses direct a return to homeostasis2 (Fig. 2-1)

establish phenotypic responses such as fever and anorexia. The vagus nerve has been
described as highly influential in mediating afferent sensory input to the CNS.3
Neural circuit relaying messages of localized injury to the brain (nucleus tractus solitarius).
The brain follows with a hormone release (adrenocorticotropic hormone [ACTH],
glucocorticoids) into the systemic circulation and by sympathetic response. The vagal
response rapidly induces acetylcholine release directed at the site of injury to curtail the
inflammatory response elicited by the activated immunocytes. This vagal response occurs in
real time and is site specific. EPI = epinephrine; IL-1 = interleukin-1; NOREPI =

Cholinergic Anti-Inflammatory Pathways
The vagus nerve exerts several homeostatic influences, including enhancing gut motility,
reducing heart rate, and regulating inflammation. Central to this pathway is the understanding
of neurally controlled anti-inflammatory pathways of the vagus nerve. Parasympathetic
nervous system activity transmits vagus nerve efferent signals primarily through the
neurotransmitter acetylcholine. This neurally mediated anti-inflammatory pathway allows for
a rapid response to inflammatory stimuli and also for the potential regulation of early
proinflammatory mediator release, specifically tumor necrosis factor (TNF).4 Vagus nerve
activity in the presence of systemic inflammation may inhibit cytokine activity and reduce
injury from disease processes such as pancreatitis, ischemia and reperfusion, and
hemorrhagic shock. This activity is primarily mediated through nicotinic acetylcholine
receptors on immune mediator cells such as tissue macrophages. Furthermore, enhanced
inflammatory profiles are observed after vagotomy, during stress conditions.4 Experimental
trials have studied this pathway to develop therapeutic interventions. Specifically, nicotine,
which also activates nicotinic acetylcholine receptors on immune cells, has been shown to
reduce cytokine release after endotoxemia in animal models.5
Pathophysiology of Shock
Regardless of etiology, the initial physiologic responses in shock are driven by tissue
hypoperfusion and the developing cellular energy deficit. This imbalance between cellular
supply and demand leads to neuroendocrine and inflammatory responses, the magnitude of
which is usually proportional to the degree and duration of shock. The specific responses will
differ based on the etiology of shock, as certain physiologic responses may be limited by the
inciting pathology. For example, the cardiovascular response driven by the sympathetic
nervous system is markedly blunted in neurogenic or septic shock. Additionally, decreased
perfusion may occur as a consequence of cellular activation and dysfunction, such as in septic
shock and to a lesser extent traumatic shock (Fig. 5-1). Many of the organ-specific responses
are aimed at maintaining perfusion in the cerebral and coronary circulation. These are
regulated at multiple levels including (a) stretch receptors and baroreceptors in the heart and
vasculature (carotid sinus and aortic arch), (b) chemoreceptors, (c) cerebral ischemia
responses, (d) release of endogenous vasoconstrictors, (e) shifting of fluid into the
intravascular space, and (f) renal reabsorption and conservation of salt and water.

result directly from hemorrhage/hypovolemia, cardiac failure, or neurologic injury.
Decreased tissue perfusion and cellular injury can then result in immune and inflammatory
responses. Alternatively, elaboration of microbial products during infection or release of
endogenous cellular products from tissue injury can result in cellular activation to
subsequently influence tissue perfusion and the development of shock. HMGB1 = high

Furthermore, the pathophysiologic responses vary with time and in response to resuscitation.
In hemorrhagic shock, the body can compensate for the initial loss of blood volume primarily
through the neuroendocrine response to maintain hemodynamics. This represents the
compensated phase of shock. With continued hypoperfusion, which may be unrecognized,
cellular death and injury are ongoing and the decompensation phase of shock ensues.
Microcirculatory dysfunction, parenchymal tissue damage, and inflammatory cell activation
can perpetuate hypoperfusion. Ischemia/reperfusion injury will often exacerbate the initial
insult. These effects at the cellular level, if untreated, will lead to compromise of function at
the organ system level, thus leading to the "vicious cycle" of shock (Fig. 5-2). Persistent
hypoperfusion results in further hemodynamic derangements and cardiovascular collapse.
This has been termed the irreversible phase of shock and can develop quite insidiously and
may only be obvious in retrospect. At this point there has occurred extensive enough
parenchymal and microvascular injury such that volume resuscitation fails to reverse the
process, leading to death of the patient. In experimental animal models of hemorrhagic shock
(modified Wiggers model), this is represented by the "uptake phase" or "compensation
endpoint" when shed blood must be returned to the animal to sustain the hypotension at the
set level to prevent further hypotension and death.10If shed blood volume is slowly returned to
maintain the set level of hypotension, eventually the injury progresses to irreversible shock,
where further volume will not reverse the process and the animal dies (Fig. 5-3).
Hypovolemic/Hemorrhagic
The most common cause of shock in the surgical or trauma patient is loss of circulating
volume from hemorrhage. Acute blood loss results in reflexive decreased baroreceptor
stimulation from stretch receptors in the large arteries, resulting in decreased inhibition of
vasoconstrictor centers in the brain stem, increased chemoreceptor stimulation of vasomotor
centers, and diminished output from atrial stretch receptors. These changes increase
vasoconstriction and peripheral arterial resistance. Hypovolemia also induces sympathetic
stimulation, leading to epinephrine and norepinephrine release, activation of the reninangiotensin cascade, and increased vasopressin release. Peripheral vasoconstriction is
prominent, while lack of sympathetic effects on cerebral and coronary vessels and local
autoregulation promote maintenance of cardiac and CNS blood flow.
Diagnosis
Treatment of shock is initially empiric. A secure airway must be confirmed or established and
volume infusion initiated while the search for the cause of the hypotension is pursued. Shock
in a trauma patient and postoperative patient should be presumed to be due to hemorrhage
until proven otherwise. The clinical signs of shock may be evidenced by agitation, cool
clammy extremities, tachycardia, weak or absent peripheral pulses, and hypotension. Such
apparent clinical shock results from at least 25 to 30% loss of the blood volume. However,
substantial volumes of blood may be lost before the classic clinical manifestations of shock
are evident. Thus, when a patient is significantly tachycardic or hypotensive, this represents
both significant blood loss and physiologic decompensation. The clinical and physiologic
response to hemorrhage has been classified according to the magnitude of volume loss. Loss
of up to 15% of the circulating volume (700 to 750 mL for a 70-kg patient) may produce little
in terms of obvious symptoms, while loss of up to 30% of the circulating volume (1.5 L) may
result in mild tachycardia, tachypnea, and anxiety. Hypotension, marked tachycardia [i.e.,
pulse greater than 110 to 120 beats per minute (bpm)], and confusion may not be evident until
more than 30% of the blood volume has been lost; loss of 40% of circulating volume (2 L) is
immediately life threatening, and generally requires operative control of bleeding (Table 5-5).
Young healthy patients with vigorous compensatory mechanisms may tolerate larger volumes
of blood loss while manifesting fewer clinical signs despite the presence of significant
peripheral hypoperfusion. These patients may maintain a near-normal blood pressure until a
precipitous cardiovascular collapse occurs. Elderly patients may be taking medications that
either promote bleeding (e.g., warfarin or aspirin), or mask the compensatory responses to
bleeding (e.g., beta blockers). In addition, atherosclerotic vascular disease, diminishing
cardiac compliance with age, inability to elevate heart rate or cardiac contractility in response
to hemorrhage, and overall decline in physiologic reserve decrease the elderly patient's ability
to tolerate hemorrhage. Recent data in trauma patients suggest that a systolic blood pressure
(SBP) of less than 110 mmHg is a clinically relevant definition of hypotension and
hypoperfusion based upon an increasing rate of mortality below this pressure (Fig. 5-6).53

The systemic response after trauma, combining the effects of soft tissue injury, long bone
fractures, and blood loss, is clearly a different physiologic insult than simple hemorrhagic
shock. Multiple organ failure, including acute respiratory distress syndrome (ARDS),
develops relatively often in the blunt trauma patient, but rarely after pure hemorrhagic shock
Diagnosis
Attempts to standardize terminology have led to the establishment of criteria for the diagnosis
of sepsis in the hospitalized adult. These criteria include manifestations of the host response
to infection in addition to identification of an offending organism. The terms sepsis, severe
sepsis, and septic shock are used to quantify the magnitude of the systemic inflammatory
reaction. Patients with sepsis have evidence of an infection, as well as systemic signs of
inflammation (e.g., fever, leukocytosis, and tachycardia). Hypoperfusion with signs of organ
dysfunction is termed severe sepsis. Septic shock requires the presence of the above,
associated with more significant evidence of tissue hypoperfusion and systemic hypotension.
Beyond the hypotension, maldistribution of blood flow and shunting in the microcirculation
further compromise delivery of nutrients to the tissue beds.78
Recognizing septic shock begins with defining the patient at risk. The clinical manifestations
of septic shock will usually become evident and prompt the initiation of treatment before
bacteriologic confirmation of an organism or the source of an organism is identified. In
addition to fever, tachycardia, and tachypnea, signs of hypoperfusion such as confusion,
malaise, oliguria, or hypotension may be present. These should prompt an aggressive search
for infection, including a thorough physical examination, inspection of all wounds, evaluation
of intravascular catheters or other foreign bodies, obtaining appropriate cultures, and
adjunctive imaging studies, as needed.
Treatment
Evaluation of the patient in septic shock begins with an assessment of the adequacy of their
airway and ventilation. Severely obtunded patients and patients whose work of breathing is
excessive require intubation and ventilation to prevent respiratory collapse. Because
vasodilation and decrease in total peripheral resistance may produce hypotension, fluid
resuscitation and restoration of circulatory volume with balanced salt solutions is essential.
Empiric antibiotics must be chosen carefully based on the most likely pathogens (gramnegative rods, gram-positive cocci, and anaerobes) because the portal of entry of the
offending organism and its identity may not be evident until culture data return or imaging
studies are completed. Knowledge of the bacteriologic profile of infections in an individual
unit can be obtained from most hospital infection control departments and will suggest
potential responsible organisms. Antibiotics should be tailored to cover the responsible
organisms once culture data are available, and if appropriate, the spectrum of coverage
narrowed. Long-term, empiric, broad-spectrum antibiotic use should be minimized to reduce
the development of resistant organisms and to avoid the potential complications of fungal
overgrowth and antibiotic-associated colitis from overgrowth of Clostridium difficile. IV
antibiotics will be insufficient to adequately treat the infectious episode in the settings of
infected fluid collections, infected foreign bodies, and devitalized tissue. This situation is
termed source control and involves percutaneous drainage and operative management to
target a focus of infection. These situations may require multiple operations to ensure proper
wound hygiene and healing.
After first-line therapy of the septic patient with antibiotics, IV fluids, and intubation if
necessary, vasopressors may be necessary to treat patients with septic shock. Catecholamines
are the vasopressors used most often. Occasionally, patients with septic shock will develop
arterial resistance to catecholamines. Arginine vasopressin, a potent vasoconstrictor, is often
efficacious in this setting.
The majority of septic patients have hyperdynamic physiology with supranormal cardiac
output and low systemic vascular resistance. On occasion, septic patients may have low
cardiac output despite volume resuscitation and even vasopressor support. Mortality in this
group is high. Despite the increasing incidence of septic shock over the past several decades,
the overall mortality rates have changed little. Studies of interventions, including
immunotherapy, resuscitation to pulmonary artery endpoints with hemodynamic optimization
(cardiac output and O2 delivery, even to supranormal values), and optimization of mixed
venous O2 measurements up to 72 hours after admission to the ICU, have not changed
mortality.

Sinyal aferen ke Otak
Sistem saraf pusat (SSP) memainkan peran kunci dalam merancang respon inflamasi. The
SSP mempengaruhi beberapa organ melalui kedua sinyal neurohormonal dan endokrin.
Cedera atau infeksi sinyal diakui oleh SSP melalui jalur sinyal aferen (Gbr. 2-2). The SSP
dapat menanggapi rangsangan inflamasi perifer melalui kedua jalur peredaran darah dan
saraf. Mediator inflamasi mengaktifkan reseptor SSP dan membangun respon fenotipik
seperti demam dan anoreksia. Saraf vagus telah digambarkan sebagai sangat berpengaruh
dalam mediasi masukan sensorik aferen ke CNS.3

Persiapan Kolinergik Anti-inflamasi

Patofisiologi Syok
Terlepas dari etiologi, respon fisiologis awal shock didorong oleh hipoperfusi jaringan dan
defisit energi sel berkembang. Ketidakseimbangan antara pasokan dan permintaan seluler
menyebabkan neuroendokrin dan inflamasi tanggapan, besarnya dari yang biasanya
sebanding dengan derajat dan durasi shock. Tanggapan tertentu akan berbeda berdasarkan

Sebagai contoh, respon kardiovaskular didorong oleh sistem saraf simpatik yang nyata tumpul
di neurogenik atau septic shock. Selain itu, penurunan perfusi dapat terjadi sebagai akibat dari
aktivasi sel dan disfungsi, seperti shock septik dan pada tingkat lebih rendah kejutan

Persiapan yang menyebabkan penurunan perfusi jaringan dan syok. Penurunan perfusi
jaringan dapat mengakibatkan langsung dari perdarahan / hipovolemia, gagal jantung, atau
cedera neurologis. Penurunan perfusi jaringan dan cedera seluler kemudian dapat
mengakibatkan respon imun dan inflamasi. Atau, elaborasi produk mikroba selama infeksi
atau pelepasan produk seluler endogen dari cedera jaringan dapat mengakibatkan aktivasi sel
untuk kemudian mempengaruhi perfusi jaringan dan pengembangan shock. HMGB1 =

Selanjutnya, respon patofisiologis bervariasi dengan waktu dan dalam menanggapi resusitasi.
Syok hemoragik, tubuh dapat mengkompensasi hilangnya awal volume darah terutama
melalui respon neuroendokrin untuk mempertahankan hemodinamik. Ini merupakan tahap
kompensasi shock. Dengan lanjutan hipoperfusi, yang dapat tidak diakui, kematian seluler
dan cedera sedang berlangsung dan fase dekompensasi shock terjadi kemudian. Disfungsi
microcirculatory, kerusakan jaringan parenkim, dan aktivasi sel inflamasi dapat
mengabadikan hipoperfusi. Cedera iskemia / reperfusi akan sering memperburuk penghinaan
awal. Efek ini pada tingkat sel, jika tidak diobati, akan menyebabkan kompromi fungsi pada
tingkat sistem organ, sehingga mengarah ke "lingkaran setan" shock (Gbr. 5-2). Hasil
hipoperfusi Persistent di derangements hemodinamik lebih lanjut dan kolaps kardiovaskular.
Ini telah disebut fase ireversibel shock dan dapat mengembangkan cukup diam-diam dan
hanya mungkin jelas dalam retrospeksi. Pada titik ini ada telah terjadi cukup parenkim luas

Hipovolemik / hemoragik
Penyebab paling umum syok pada pasien bedah atau trauma adalah hilangnya volume
sirkulasi dari perdarahan. Hasil kehilangan darah akut di refleksif penurunan stimulasi
baroreseptor dari peregangan reseptor di arteri besar, yang mengakibatkan penurunan
penghambatan pusat vasokonstriktor di batang otak, meningkatkan stimulasi kemoreseptor
pusat vasomotor, dan output berkurang dari atrium stretch reseptor. Perubahan ini
meningkatkan vasokonstriksi dan resistensi arteri perifer. Hipovolemia juga menginduksi
stimulasi simpatis, yang menyebabkan epinefrin dan norepinefrin rilis, aktivasi kaskade reninangiotensin, dan peningkatan pelepasan vasopressin. Vasokonstriksi perifer adalah menonjol,
sementara kurangnya efek simpatik pada pembuluh darah serebral dan koroner dan
autoregulasi lokal mempromosikan pemeliharaan aliran darah jantung dan SSP.

Pengobatan syok awalnya empiris. Sebuah jalan napas aman harus dikonfirmasi atau
didirikan dan infus volume yang dimulai sementara mencari penyebab hipotensi ini dikejar.
Syok pada pasien trauma dan pasien pasca operasi harus dianggap disebabkan oleh
perdarahan sampai terbukti sebaliknya. Tanda-tanda klinis syok dapat dibuktikan dengan
agitasi, ekstremitas teraba dingin, takikardia, denyut perifer lemah atau tidak ada, dan
hipotensi. Seperti jelas sengatan klinis dari kehilangan setidaknya 25 sampai 30% dari
volume darah. Namun, volume besar darah mungkin akan hilang sebelum manifestasi klinis
klasik syok yang jelas. Dengan demikian, ketika seorang pasien secara signifikan takikardi
atau hipotensi, ini merupakan kedua kehilangan darah yang signifikan dan dekompensasi
fisiologis. Respon klinis dan fisiologis perdarahan telah diklasifikasikan menurut besarnya
penyusutan volume. Kerugian hingga 15% dari volume sirkulasi (700-750 mL untuk pasien
70 kg) dapat menghasilkan sedikit dalam hal gejala yang jelas, sedangkan kerugian hingga
30% dari volume sirkulasi (1,5 L) dapat menyebabkan ringan takikardia, takipnea, dan
kecemasan. Hipotensi, takikardia ditandai [yaitu, pulsa lebih besar dari 110 sampai 120
denyut per menit (bpm)], dan kebingungan mungkin tidak jelas sampai lebih dari 30% dari
volume darah telah hilang; kehilangan 40% dari volume sirkulasi (2 L) segera mengancam
kehidupan, dan umumnya membutuhkan kontrol operasi perdarahan (Tabel 5-5). Pasien sehat
muda dengan mekanisme kompensasi yang kuat dapat mentolerir volume yang lebih besar
kehilangan darah saat mewujudkan tanda-tanda klinis lebih sedikit meskipun kehadiran
hipoperfusi perifer yang signifikan. Pasien-pasien ini mungkin mempertahankan tekanan
darah mendekati normal sampai kolaps kardiovaskular terjal terjadi. Lansia pasien dapat
mengambil obat yang baik mempromosikan perdarahan (misalnya, warfarin atau aspirin),
atau menutupi tanggapan kompensasi untuk perdarahan (misalnya, beta blockers). Selain itu,

Syok trauma
Respon sistemik setelah trauma, menggabungkan efek cedera jaringan lunak, fraktur tulang
panjang, dan kehilangan darah, jelas merupakan penghinaan fisiologis yang berbeda dari syok
hemoragik sederhana. Gagal organ ganda, termasuk akut sindrom gangguan pernapasan
(ARDS), mengembangkan relatif sering pada pasien trauma tumpul, tapi jarang setelah murni
syok hemoragik (seperti perdarahan GI). Defisit hipoperfusi shock traumatik diperbesar oleh
aktivasi proinflamasi yang terjadi setelah induksi shock. Selain iskemia atau iskemiareperfusi, mengumpulkan bukti menunjukkan bahwa bahkan perdarahan sederhana

cepat. Dalam model laboratorium shock traumatik, penambahan jaringan lunak atau cedera
tulang panjang perdarahan menghasilkan lethality dengan kehilangan darah secara signifikan
kurang bila hewan tertekan oleh perdarahan. Pengobatan syok traumatis difokuskan pada
koreksi elemen individu untuk mengurangi kaskade aktivasi proinflamasi, dan termasuk
kontrol cepat dari perdarahan, resusitasi volume yang memadai untuk memperbaiki utang O2,
debridement jaringan nonviable, stabilisasi cedera tulang, dan pengobatan yang tepat lembut
cedera jaringan.
Septic shock (vasodilatasi Syok)
Dalam sirkulasi perifer, vasokonstriksi mendalam adalah respon fisiologis yang khas untuk
tekanan arteri menurun dan perfusi jaringan dengan perdarahan, hipovolemia, atau gagal
jantung akut. Ini bukan respon karakteristik vasodilator shock. Syok vasodilatasi adalah hasil
dari disfungsi endotel dan pembuluh darah sekunder untuk beredar mediator inflamasi dan
sel-sel atau sebagai respon terhadap hipoperfusi berkepanjangan dan berat. Dengan demikian,
shock vasodilatasi, hasil hipotensi dari kegagalan otot polos pembuluh darah mengerut tepat.
Syok vasodilatasi ditandai dengan vasodilatasi perifer dengan hipotensi yang dihasilkan dan
ketahanan terhadap pengobatan dengan vasopressor. Meskipun hipotensi, tingkat katekolamin
plasma meningkat, dan sistem renin-angiotensin diaktifkan di vasodilator shock. Bentuk yang
paling sering ditemui syok vasodilatasi adalah syok septik. Penyebab lain vasodilator syok

Meskipun kemajuan dalam perawatan intensif, tingkat kematian sepsis berat tetap pada 30
sampai 50%. Di Amerika Serikat, 750.000 kasus sepsis terjadi setiap tahun, sepertiga dari
yang fatal.76 rekening Sepsis untuk 9,3% dari kematian di Amerika Serikat, sebanyak

Dalam upaya untuk membasmi patogen, jenis sel kekebalan tubuh dan lainnya (misalnya, sel
endotel) menguraikan mediator larut yang meningkatkan makrofag dan neutrofil membunuh
mekanisme efektor, meningkatkan aktivitas prokoagulan dan aktivitas fibroblast pelokalan
penjajah, dan meningkatkan aliran darah mikrovaskuler untuk meningkatkan pengiriman
membunuh pasukan ke daerah invasi. Ketika respon ini terlalu bersemangat atau menjadi
sistemik daripada lokal, manifestasi sepsis mungkin jelas. Temuan ini termasuk output
ditingkatkan jantung, vasodilatasi perifer, demam, leukositosis, hiperglikemia, dan takikardia.
Pada syok septik, efek vasodilatasi yang disebabkan, sebagian, dengan upregulation isoform
diinduksi nitric oxide synthase (iNOS atau NOS 2) di dinding pembuluh darah. iNOS

Upaya untuk membakukan terminologi telah menyebabkan pembentukan kriteria untuk
diagnosis sepsis dalam dewasa dirawat di rumah sakit. Kriteria ini termasuk manifestasi dari
respon host terhadap infeksi selain identifikasi organisme menyinggung. Istilah sepsis, sepsis
berat, dan syok septik yang digunakan untuk mengukur besarnya reaksi inflamasi sistemik.
Pasien dengan sepsis memiliki bukti infeksi, serta tanda-tanda sistemik peradangan (misalnya,
demam, leukositosis, dan takikardia). Hipoperfusi dengan tanda-tanda disfungsi organ disebut
sepsis berat. Syok septik membutuhkan kehadiran di atas, terkait dengan bukti yang lebih
signifikan hipoperfusi jaringan dan hipotensi sistemik. Di luar hipotensi, salah-distribusi
aliran darah dan shunting dalam mikrosirkulasi pengiriman kompromi lebih lanjut dari nutrisi
ke jaringan beds.78
Menyadari syok septik dimulai dengan mendefinisikan pasien pada risiko. Manifestasi klinis
syok septik biasanya akan menjadi jelas dan meminta memulai pengobatan sebelum
konfirmasi bakteriologis dari suatu organisme atau sumber organisme diidentifikasi. Selain
demam, takikardia, dan takipnea, tanda-tanda hipoperfusi seperti kebingungan, malaise,
oliguria, atau hipotensi mungkin ada. Ini harus segera pencarian agresif untuk infeksi,
termasuk pemeriksaan fisik secara menyeluruh, pemeriksaan semua luka, evaluasi kateter
intravaskular atau badan asing lainnya, mendapatkan budaya yang tepat, dan studi pencitraan
ajuvan, sesuai kebutuhan.
pengobatan
Evaluasi pasien dalam syok septik dimulai dengan penilaian terhadap kecukupan saluran
napas dan ventilasi mereka. Parah pasien obtunded dan pasien yang kerja pernapasan
berlebihan membutuhkan intubasi dan ventilasi untuk mencegah keruntuhan pernapasan.
Karena vasodilatasi dan penurunan resistensi perifer total dapat menghasilkan hipotensi,
resusitasi cairan dan pemulihan volume sirkulasi dengan larutan garam seimbang sangat
penting. Antibiotik empiris harus dipilih dengan cermat berdasarkan patogen yang paling
mungkin (batang gram negatif, gram-positif cocci, dan anaerob) karena portal masuknya
organisme menyinggung dan identitasnya mungkin tidak jelas sampai kembali data yang
budaya atau pencitraan studi yang selesai. Pengetahuan tentang profil bakteriologis infeksi di
unit masing-masing dapat diperoleh dari sebagian departemen pengendalian infeksi di rumah
sakit dan akan menyarankan organisme yang bertanggung jawab potensial. Antibiotik harus
disesuaikan untuk menutupi organisme yang bertanggung jawab setelah data budaya yang
tersedia, dan jika sesuai, spektrum cakupan menyempit. Jangka panjang, empiris, spektrum
luas penggunaan antibiotik harus diminimalkan untuk mengurangi perkembangan organisme
resisten dan untuk menghindari potensi komplikasi pertumbuhan berlebih jamur dan kolitis
terkait antibiotik dari pertumbuhan berlebih dari Clostridium difficile. Antibiotik IV tidak
akan cukup untuk mengobati memadai episode menular dalam pengaturan koleksi terinfeksi
cairan, benda asing yang terinfeksi, dan jaringan devitalized. Situasi ini disebut kontrol
sumber dan melibatkan drainase perkutan dan manajemen operasi untuk menargetkan fokus
infeksi. Situasi ini mungkin memerlukan beberapa operasi untuk memastikan kebersihan luka
yang tepat dan penyembuhan.
Setelah terapi lini pertama pasien septik dengan antibiotik, cairan IV, dan intubasi jika perlu,
vasopressor mungkin diperlukan untuk mengobati pasien dengan syok septik. Katekolamin
adalah vasopressor paling sering digunakan. Kadang-kadang, pasien dengan syok septik akan
mengembangkan resistensi arteri terhadap katekolamin. Vasopressin arginin, suatu
vasokonstriktor kuat, sering manjur dalam pengaturan ini.
Sebagian besar pasien septik memiliki fisiologi hiperdinamik dengan curah jantung di atas
normal dan resistensi vaskular sistemik yang rendah. Pada kesempatan, pasien sepsis
mungkin memiliki curah jantung rendah meskipun resusitasi volume dan bahkan dukungan
vasopressor. Kematian dalam kelompok ini adalah tinggi. Meskipun meningkatnya insiden
syok septik selama beberapa dekade terakhir, tingkat kematian secara keseluruhan telah
berubah sedikit. Studi intervensi, termasuk imunoterapi, resusitasi untuk endpoint arteri
pulmonalis dengan optimasi hemodinamik (cardiac output dan pengiriman O2, bahkan di atas
