CRIME REPORT
(536-933)

Complete this report for offenses occurring in the City of Oakland, except
for cases involving worthless documents, missing persons, stolen or
recovered vehicles, towed vehicles, lost and found property or matters to
be investigated by other agencies. Complete this report in cases involving
accidental discharge of weapon or dispatch of an injured or vicious animal.
Also complete this report whenever a crime occurs related to domestic
violence and for all incidents involving a domestic dispute or a restraining
order, even if a crime has not occurred.
Complete one copy for each victim of a crime against the person.
Complete one copy for each incident involving crime against property.

Check the appropriate box to route extra copies of this


report to other units.

HEADER

Leave "Assign to" and "RD No." blank.


Complete "Outside Reporting Agency" if not a Department
member or employee, i.e., Oakland Housing Authority,
Oakland Public School Security, etc., otherwise leave blank.
Leave "Reporting Agency Case No.", and "Police Beat"
blank. Enter the Community Policing Beat number for the
location of the occurrence in "CP Beat".
Enter the incident number.

VICTIM

Enter the name of the victim of the incident.


Citizen

Last name first, then first name and middle


name or initial. Indicate Miss, Mrs., Jr. or Sr. if
appropriate.

Military

Include rank, social security number and


branch of

Enter the firm name. For a large firm with


many branches, include the branch name,
store number, warehouse number or
substation number.
If a business, check the "Business Name" box
to indicate that the business is the victim of
the incident.
If another jurisdiction, check the
"Local/State/Federal" box to indicate that a
jurisdiction is the victim (ex. Dept. of Fish
and Game, FBI, Post Office).

School

Enter the name of the school.

Park

Enter the name of the park.

Recreation

Enter the name of the recreation center or


facility.

Center
Gas Station Enter the business name of the station and
include the name of the oil company.
Enter the person's sex, race, date of birth or age.
Enter the person's home address including building, room or
apartment number, and zip code. Enter the name of the city
if outside Oakland. If Oakland address, check the "Oakland"
box. For military personnel, include unit ship or station. For
Department personnel, enter OPD. For a business, leave
blank.
Enter the person's home or message telephone number,
including the area code if other than 510. For a business
leave blank.
Enter the person's business or school address including zip
code. Enter the name of the city if outside Oakland. If
Oakland address, check the "Oakland" box.
Enter the business's phone number or the phone number of
the person's place of employment.

Enter the person's occupation or job title. For a Department


member or employee, leave blank.
Enter the hours and days of the week when the person can
be contacted at the business address.
Enter the victim's driver's license number. Enter the state if
not California.
Check the "Domestic Violence" box if applicable.
If victim of a sexual assault has been notified of the right to
have a victim advocate and /or support person present,
check the "Victim's Support" box.
Check the "Victim Injured" box if the victim sustained
injuries.
If victim is provided with a Resource Card for Victims of
Domestic Violence and Other Violence Crimes (TF-869) or
the Spanish version (TF-869-1), check the "Resource Info
Provided" box.
If victim of a sexual assault requests anonymity, check the
"Sex Assault Victim Requests Conf." box.
ADDITIONAL When a person other than the complainant notified the
police of the

"Local/State/Federal" box under Victim has been checked,
enter the
reporting officer's first initial, last name and serial number.
Indicate whether the additional person is the reporting party
(R/P),

reside, whether or not they are at home,
such as houses, apartments, mobile homes
and houseboats. Travel trailers and boats
used as permanent dwellings and a
residential area in a commercial building are
also included here.

If the incident was gang-related, check the box and, if


known, indicate the gang name and victim's relationship to
the gang in the narrative.
If race, ethnicity, religion, sexual orientation, physical or
mental disability, or gender was a factor in the crime, check
the box. Describe the circumstances in the narrative that
support this conclusion.
CRIME

When a crime has occurred, enter the common name of the


crime followed by the section number (including any
applicable subsection) and the code abbreviation. Enter
which victim(s) the crime pertains to, i.e., VI, V2 etc.
When reporting an incident involving a domestic dispute
and a crime has not occurred, enter "Domestic Dispute." In
addition to the Crime Report, complete a copy of the
Continuation Report (536936) in order to record the
information relevant to a domestic dispute.
When reporting an incident where you advised a suspect of
the terms of a restraining order and a crime has not
occurred, enter "Proof of Service." In addition to the Crime
Report, complete a copy of the Continuation Report
(536-936) in order to record the information relevant to
proof of service of a temporary restraining order.
When reporting the accidental discharge of a weapon and a
crime has not occurred, enter "K-3, Accidental Discharge"
and the narrative should read, "Crime Report prepared for
the issuance of an RD number only."
When reporting the dispatching of an injured or vicious
animal and a crime has not occurred, enter "K-3, Vicious
Animal" or "K-3, Injured Animal" as appropriate and the
narrative should read, "Crime Report prepared for the
issuance of an RD number only."

Includes devices such as typewriters, adding


machines, cash registers, duplicating
machines, computers and computer

and pagers.

TV's, Radios, Includes all items that are designed to

picture cameras and projectors, still picture

Miscellaneous
Includes items not accounted for in the
other categories such as shrubbery, vehicle
parts, boats, trailers, airplanes, books and
household pets.

Pursesnatch The grabbing or snatching of a purse,


handbag, etc., from the custody of an
individual. If more force is used than is
actually necessary to snatch the purse from
the grasp of the person, then a strong arm
robbery has occurred.

The theft of any part or accessory attached


to the

The theft of personal property from an

the vehicle, indicate the investigative unit for which the
