Winter Retreat 2010

Please Review the following, fill out the forms and bring them with you on Sunday! We
must have these forms filled out in order for you to attend the retreat!

Date and Time:

We will meet at Baker on Sunday, January 17th at 10:30. COME DIRECTLY TO THE
YOUTH ROOM! Church will be going on and we cannot create a disturbance.
We will be loading the bus and leaving at 11:00. We are not stopping for lunch so you
will want to pack something! That afternoon we will be at Camp Findley for free time,
Bible study, and Dinner. After Dinner we will be headed to Peek’n Peak ski resort for
Skiing/snowboarding or tubing.

The next morning we will have a closing session and breakfast at Camp Findley and then
ride the bus home again. We will arrive back at Baker at noon.

Winter Retreat Expectations:

Behavior: We will be staying at Camp Findley and skiing/snowboarding/tubing at

Peek’N Peak Resort. We will be guests in these facilities and are expected to treat the
property correctly. At the beginning of the retreat we will go over the rules and safety
stuff for the retreat. It is also expected that you listen to any and all leaders while on this
retreat. We want to make sure everyone has a safe and fun time!

What to Bring?
- Lunch for the ride down.
- Snow Gear for our night out at Peek’n Peak
- Skiing/Snowboarding gear (for those going skiing/snowboarding)
- A Check written to Baker Church for your ticket to Peak’n Peek
(Skiing/snowboarding $24 Tubing $15)
- Sleeping bag and pillow
- Toiletries
- Bible, notebook, and a pen
- Appropriate clothing for a one-night stay including sleepwear.
- Board game or cards for free time
- Medical Release Form/Permission Slip

What not to Bring!

- No fireworks, weapons, knifes, or guns
- NO drugs, alcohol, or tobacco products

I give my permission for my above named child to join the Baker Memorial United
Methodist Youth Group on the Winter Retreat 2010, taking place January 17th- 18th 2010
at Camp Findley. I understand that the group will be traveling by a school bus. I also
understand that they will be participating in winter sports at Peak’n Peek Resort.

I hereby release Baker Memorial United Methodist Church, its staff, sponsors, and
volunteer leaders from responsibility and liability for any injury or illness that my child
may sustain during this activity.

In the event of an emergency, I hereby authorize an adult leader for this activity (J.L.
Miller, Jeff Thompson, Katie Felton), as agent for me to consent to any x-ray
examination; medical, dental, or surgical diagnosis; treatment; and hospital care advised
and supervised by a physician, surgeon or dentist (as appropriate) licensed to practice
under the laws of the state where the services are rendered, either at a doctor’s office or at
any hospital. I expect to be contacted as soon as possible.
