REQUIRED CONTROLS

Describe fully all equipment, facility, and/or procedure controls applicable for each
hazard (i.e. if PPE is used as control, it must be specifically describe.) If a control can
only be verified by documentation then it must be available. All controls must be valid
in that they reduce severity, probability of both.

S: PTW System, Toolbox Talk and proper PPE.


Work to be done only at safe sea side, never quay side.
Watchman to standby for look out and inform concerned of any danger.
Life buoy deployed in water and work vest to be worn.
Proper staging erected or good bosun chair to be used.
Safety belt/harness to be worn and condition inspected before use.

Work in good coordination with colleagues to avoid.
Working area and adjacent to be made clean and clear of any
obstruction.
Staging condition must be carefully examined by senior officer to ensure
suitability.
Attachment point carefully tested to ensure adequate strength.
Good safety belt/harness to be worn.
Standby crew to inform working colleagues if any big waves or other
vessel coming towards own vessel.
Notify other approaching vessel that there are crews working over-side
and do not allow to come alongside until working crew clear from the ship
side.
Duty engineer to be notified of men working below ship side discharges
and notices attached to relevant valves.
Work clear of overboard discharges if possible.
